Patient and Family Handbook


Welcome<br />

At Henry Ford Macomb Hospitals, our goal is to help you<br />

become as independent as possible while achieving your<br />

physical, emotional, psychological, social and work-related<br />

potential. This may require learning or relearning skills. Your<br />

abilities, not disabilities, are emphasized. Our program is<br />

designed to focus on three areas:<br />

• increasing your ability to perform everyday skills<br />

• educating you and your family<br />

• enhancing the quality of your future<br />

We also encourage participation from family and friends<br />

in the treatment process. As a member of the Henry Ford<br />

Health System, our mission is to improve human life through<br />

excellence in the science and art of health care and healing.<br />

Helping you heal in body, mind and soul is at the core of our<br />

mission.<br />

About Your <strong>Rehabilitation</strong> Team<br />

Upon your arrival at Henry Ford Macomb, the rehabilitation<br />

team evaluates your physical, medical, psychological,<br />

vocational and social needs. Working with you and your<br />

family, they develop an individualized goal plan. The team then<br />

meets weekly to review your progress, revise goals and plan for<br />

your successful discharge from the program.<br />

Your involvement and the involvement of your family are<br />

critical to your progress. In light of this, a team member will<br />

always involve you in plan recommendations and changes. If<br />

you or a family member has questions about your care, please<br />

direct them to a team member or the program manager. The<br />

following are descriptions of team members and their roles.<br />

family conferences, as requested. Your family or caregiver will<br />

be scheduled to attend therapy and nursing instruction sessions<br />

to aid in understanding your individual needs and prepare for<br />

discharge.<br />

Physiatrist<br />

The physiatrist is a physician who specializes in Physical<br />

Medicine and <strong>Rehabilitation</strong> and is generally responsible for the<br />

medical management of patients involved in the rehabilitation<br />

program. The physiatrist evaluates, advises and directs the total<br />

care of the patient and utilizes the expertise of a variety of<br />

professionals.<br />

<strong>Rehabilitation</strong> Nurse<br />

Our specially trained nurses provide support and care on a<br />

24-hour basis. They also provide you and your family with<br />

education.<br />

Physical Therapist<br />

The physical therapist addresses areas such as strengthening,<br />

balance and coordination, wheelchair activities, walking and<br />

stair climbing.<br />

Occupational Therapist<br />

The occupational therapist helps you use your current<br />

physical capabilities to become as independent as<br />

possible. Therapists focus on self-care skills, daily living<br />

activities, work activities, and perceptual and cognitive tasks.<br />

Speech/Language Pathologist<br />

The speech/language pathologist evaluates and treats problems<br />

with understanding and judgment, gesturing, speaking, reading,<br />

writing, hearing and swallowing.<br />

Recreational Therapist<br />

At our Clinton Township program, a recreational therapist is<br />

available to provide therapy in the form of leisure activities,<br />

focusing on memory skills, understanding and judgment,<br />

leisure interests, socialization and coordination functions.<br />

Social Worker<br />

The social worker provides counseling and emotional support<br />

to you and your family. The social worker is also responsible<br />

for coordinating the discharge planning process, which may<br />

include outpatient and/or home care services, equipment,<br />

transportation and utilization of community resources.<br />

About Your Stay<br />

What to Bring<br />

Our goal is to help you achieve independence as soon as<br />

possible, and part of that work includes getting dressed every<br />

day. In addition to personal hygiene products, you should bring<br />

the following clothes:<br />

• comfortable clothing, such as jogging suits or loose-fitting<br />

pants (5 sets)<br />

• robe, pajamas<br />

• undergarments (5 sets)<br />

• regular or athletic shoes<br />

• jacket or sweater<br />

Family members should make arrangements to do your personal<br />

laundry in a timely manner.<br />

Daily Schedule<br />

A routine for your daily activities will be developed. Physical,<br />

speech and occupational therapy sessions are available seven<br />

days a week, and the minimum expectation for program<br />

participation is that you participate in therapy three hours<br />

per day, five days a week, or a minimum of 15 hours per<br />

week. Therapy schedules are established by the individual<br />

therapists and are posted on the units.<br />

Services available<br />

We have a variety of services available to you during your<br />

stay on the rehabilitation unit. Some services are provided<br />

on-site while others are provided on a referral basis. Examples<br />

of services that are provided on-site include nutrition services,<br />

foreign language interpreter services, laboratory, pharmacy,<br />

radiology, neuro-psychological services, durable medical<br />

equipment (walkers, wheelchairs, hospital bed, non-custom<br />

orthotics, etc.) and assistive technology to modify your feeding<br />

utensils. If you feel you need a service that is not being<br />

provided, please ask your nurse, social worker, or therapist.<br />

Although we provide comprehensive rehabilitation services,<br />

some services may be provided by outside partners upon discharge.<br />

Examples of these services include, but are not limited<br />

to, custom prosthetics and orthotics, vocational rehabilitation,<br />

driver’s assessment, sexual counseling and smoking cessation<br />

support.<br />

Visiting Hours<br />

The Clinton Township hospital’s weekday visiting hours are<br />

from 4 to 8:30 p.m. and weekend and holiday visiting hours are<br />

from 9 a.m. to 8:30 p.m. The Warren location’s visiting hours<br />

are 10 a.m. to 8 p.m. daily. Exceptions to these hours are made<br />

for immediate family members; however, we ask that therapy<br />

sessions not be interrupted by visitors. Children visiting patients<br />

in the program must be supervised at all times.<br />

Smoking Restrictions<br />

Smoking is not allowed anywhere in the hospital or on hospital<br />

property. If you are having difficulty complying with the “no<br />

smoking” rule, please talk with your physician about receiving<br />

assistance during your stay.<br />

Safety<br />

Promoting a safe environment is a priority. Belts called<br />

transfer or gait belts are used with all patients to assist in safe<br />

walking or movements from sitting or standing positions. Your<br />

family can help with your safety by maintaining the use of<br />

safety devices put in place by staff; by requesting assistance<br />

in moving you; and by reminding you not to grab anything<br />

that is not secure. Of course, we also want to make sure that<br />

patients and family members use the call light in the rooms to<br />

seek assistance whenever needed.<br />

Television, DVD/VCR and Stereo System<br />

A television, DVD/VCR and stereo system are available<br />

in the activity room to promote socialization with<br />

others. Individual TV and phone service are available for all<br />

hospitalized patients. We provide a wireless environment for<br />

our patients and visitors.<br />

Preparing for Your Discharge<br />

Family Outings<br />

As you approach discharge, we encourage you to take “practice<br />

trips” outside the hospital. These outings provide both you<br />

and your family an opportunity to discover some of the<br />

challenges that may occur after discharge, when staff members<br />

are not available to help. The outings range from a short<br />

car ride to a trip home for the day. Following each outing,<br />

the staff will meet with you and your family to assess the<br />

outing and work to resolve any issues. Prior approval by your<br />

rehabilitation physician is required for these outings.<br />

Discharge Plans<br />

Plans for your discharge are actually put into place as soon<br />

as you are admitted to the <strong>Rehabilitation</strong> Units in Clinton<br />

Township or Warren. We encourage family support in<br />

preparing for your discharge. If further therapy, home care<br />

or special adaptive equipment is needed, arrangements will be<br />

made. You will be encouraged to make follow-up appointments<br />

with your personal physician.<br />

Home Evaluation<br />

Depending on your needs, a home evaluation may be<br />

scheduled. A home evaluation is an assessment of your living<br />

environment. This evaluation will be scheduled during your<br />

stay in the <strong>Rehabilitation</strong> Program. The therapist meets you and<br />

your family at your home. While you are at home, the therapist<br />

will evaluate your mobility skills, address safety issues, and<br />

recommend proper equipment. Any questions or concerns will<br />

be addressed prior to discharge.<br />

Graduation Day Celebration<br />

While in our rehab program, you will re-learn the things<br />

necessary for your successful return home. You will<br />

demonstrate your success on Graduation Day. This will occur<br />

the day before you leave the hospital. On this day, you will<br />

perform the tasks necessary to show that you are ready to<br />

eturn home, and have achieved your goals. Graduation Day is<br />

truly your day to “show off” your achievements!<br />

Follow-up Services<br />

Henry Ford Macomb Hospitals and Henry Ford Health System<br />

offer comprehensive outpatient services to help you continue<br />

your recovery. Recommendations and arrangements for many<br />

of these services will be made prior to your discharge.<br />

Outpatient Therapy<br />

Henry Ford Macomb Hospitals offers an extensive array of<br />

physical, occupational and speech therapy programs on an<br />

outpatient basis. Therapy is offered on the campuses of our<br />

Warren physical rehab center and Clinton Township hospital as<br />

well as several conveniently located outpatient sites throughout<br />

Macomb County.<br />

Home Care<br />

Henry Ford Home Health Care offers an in-home team of<br />

certified health care professionals providing skilled nursing;<br />

physical, occupational and speech therapies; home health<br />

aide services; medical social work services; and nutritional<br />

counseling.<br />

Medical Equipment<br />

Henry Ford Health Products provides medical equipment,<br />

including ambulatory aids and wheelchairs; hospital beds,<br />

bedside commodes or shower benches; and traditional products<br />

such as blood pressure and diabetes supplies.<br />

Stroke Support Group<br />

This group meets monthly at the Clinton Township hospital.<br />

It offers support and socialization to you and your family as<br />

you adjust to lifestyle changes brought on by a stroke.<br />

Insurance & Payment Information<br />

Medicare (with a secondary insurance) pays for the cost of<br />

inpatient rehabilitation provided you meet criteria at admission<br />

and during your stay. This criteria includes a medical condition<br />

that necessitates acute care in an inpatient rehabilitation program,<br />

along with the ability to participate in at least 15 hours<br />

of therapy per week. The Medical Directors at each program will<br />

make the decision whether you meet criteria, but it is always<br />

subject to review from Medicare. Private insurance, Medicaid,<br />

and managed care benefits will be verified prior to admission<br />

and pre-authorization for admission will be obtained by<br />

hospital staff. This is not a guarantee of payment. Throughout<br />

your stay, the rehabilitation staff will work with your insurance<br />

provider to obtain continued authorization and we will provide<br />

information regarding co-pays and deductibles.<br />

Private payment and reduced cost arrangements for patients<br />

who qualify for financial assistance can be arranged through<br />

the billing department of Henry Ford Macomb Hospitals. Third<br />

party funding sources such as Worker’s Compensation can be<br />

negotiated as needed.<br />

You are entitled to and will receive a written disclosure of your<br />

specific insurance information and coverage that is available to<br />

us at the time of your admission to the inpatient rehabilitation<br />

unit. Our admission referral coordinators will help you with this<br />

information.
