Leader Packet 7) Here is a list of some things you will need to bring; bible and notebook, sleeping bag, pillow, sleeping mat, towel, shoes for activities and sports, warm clothes, snow jacket, gloves, hat, toiletries, flashlight (optional). If you want to buy any late night snacks or burgers and shakes please send no more than $15. Sack dinner for the drive over. NOTE: newlife church is not responsible for any lost or stolen items. ATTN: If you bring any weapons of any kind or any alcohol, you will be asked to leave camp immediately and you will be responsible for transportation. 8) There will be a theme night and we encourage you to wear something to celebrate with us! Winter Camp 2012 WAIVER, RELEASE AND INDEMNIFICATION AGREEMENT I wish to participate in recreational activities to be made available to participants at Winter Camp 2012 including such activities as sledding, snow ball fights and/or other activities that may be hazardous or otherwise involve a risk of physical injury or death to participants (the “Activities”). I expressly assume any and all risks of injury or death arising from or relating to the Activities and waive and release any and all actions, claims, suits or demands of any kind or nature whatsoever against New Life Church on the Peninsula, its corporate affiliates, contractors, vendors, officers, agents, sponsors, volunteers or representatives of any kind (collectively “Releasees”) arising from or relating in any way to my voluntary participation in the Activities. I understand that this Waiver, Release and Indemnification Agreement means, among other things, that if I am injured or die as a result of my participation in any of the Activities, I and/or my family or heirs cannot under any circumstances sue Releasees or any of them for damages relating to or caused by my injuries or death. I agree to indemnify Releasees or any of them, and their subrogees, if any, in the event of any loss, damage or claim arising from or relating in any way to my participation in any of the Activities. I understand and agree that I would not have been permitted to participate in any of the Activities had I not executed this Waiver, Release and Indemnification Agreement. I have read this Waiver, Release and Indemnification Agreement, have asked and received answers to any questions I had concerning its meaning, and execute it freely, without duress, and in full complete understanding of its legal effect, and of the fact that it may affect my legal rights. CONSENT TO TREATMENT I represent that I am legally responsible for __________________________ (“Camper”), whose date of birth is _________________________. By signing below, I hereby consent and authorize any necessary medical treatment for Camper including, but not limited to, the following: I consent and authorize such medical care encompassing routine evaluation and care by New Life Church on the Peninsula (“newlife”). I authorize the Camp Nurse to provide Camper with over-the-counter medication as deemed necessary for minor concerns such as headache, muscle soreness, cough, sore throat, etc. I confirm that I have accurately completed the attached Medical Information Form and that all medication routines have been properly documented and reported to newlife. I will ensure that all prescription medications Camper is routinely taking at home will be provided to the newlife. I understand that these will be held in the care of newlife and I authorize newlife to administer such medications to Camper. I hereby authorize the medical personnel chosen by newlife to secure and administer treatment for Camper in the event of a medical emergency. Camper may be transferred to a hospital or facility for care or treatment, including but not limited to, x-rays, routine tests and other necessary treatments. I also authorize the billing of Camper’s personal medical insurance, if any, in the event of treatment and transportation by the secured facilities, and agree to assume responsibility for any costs not covered by insurance.
