o Birtho Serious Illness/Injuryo Death If there has been a death in the family, please provide:o Name of Funeral Home/Mortuary and Telephone Numbero Name of Attending Physician and Telephone Number For births and serious injuries or illness, the individual must give signed consent <strong>to</strong> thephysician for the release of information <strong>to</strong> the American Red Cross. If message concernssomeone other than the service member’s spouse or immediate family, (i.e. In-Laws), pleaseadvise the parties of this need for consent. Be sure <strong>to</strong> provide a telephone number where you can be reached. Once all necessary information has been received by the Red Cross caseworker, a CaseNumber/Message will be issued/assigned <strong>to</strong> you. Please write this number down as it will berequired <strong>to</strong> confirm or update the message being relayed. If you are in contact with yourservice member or their command, be sure <strong>to</strong> pass on the ARC Case Number. The American Red Cross Messaging Center personnel will verify the emergency and initiatecontact with the appropriate military authorities. This notification could take 12-72 hours <strong>to</strong>complete. EMERGENCY LEAVE:o The American Red Cross provides communications and emergency reporting for theservice member and their families in emergency situations. Verification of anexisting emergency through American Red Cross channels is the only way a servicemember can request emergency leave.o The American Red Cross does not grant emergency leave -- that is a commanddecision. The American Red Cross’ responsibility is <strong>to</strong> verify the situation accuratelywith the proper authority (doc<strong>to</strong>r, hospital, coroner) in the area of the emergencyand relay a message <strong>to</strong> service member's command for their action.o No message regarding a medical emergency may leave the Continental United States(CONUS) via the American Red Cross Emergency Communications Center inWashing<strong>to</strong>n, DC without a Doc<strong>to</strong>r's Interpretative Statement (DIS).
