The Well Site kronit~rlng Fa,- is ergaxtized to facilitate the<br />

entry of data from the Water System Record Book and to collect<br />

infomation on technical and arganfzationaJ problems. It<br />

contains an obsenmtian checklist to monitor the status of the<br />

punp, civil works afid environmental heclth carsdftions at the<br />

well site* ft contains qdestians on the relationship between<br />

the pump operator, and the community water corxuafttee, the WDA,<br />

and athez institutions involved in the operation and maintenance<br />

of the system. One section qderfes the pump operator on<br />

progress made toward the resolution of past problems and actions<br />

needed to resalve current ones,<br />

The Water Point Monitoring For4 is used to survey water sources,<br />

ather than the project well, frequently used by permanent and<br />

temporszy residents of the area. The purpose is t3 determine<br />

the effsct of the drilled well on patterns of water use, numbers<br />

of users and rellabflty of supply at these other watering<br />

paints. A aajar issue is rbe exten* to which the availabiLity<br />

of water at tht drilled well reduces the demand at these other<br />

sources and innreases thair relfabilty; <strong>part</strong>icularly during the<br />

dry season, Data collected a2 fees chargsd at these other<br />

points can be compared with those at the well and should provide<br />

data which. will gsrnit an analysis of the relationship between<br />

costs to consumers and patterns of use. The survey form is<br />

essentially a repeat of the Baseline Water Point Survey.<br />

The Household %anitorfng Fom repeats questions from the<br />

Baseline Household Surrey, including those on time and labor<br />

expended ro ccS2ect water. The data will be analyzed ta<br />

deternine the tine saved by hsusehofds and how it is used. This<br />

suxvey will be administered periodically to the same households<br />

aver the life of the nonitaring period- It should capture the<br />

seasonal pattern cf water use at different sources and should<br />

reveal the changes which result fron the avaflabifty of L-ates at<br />

the drilled well.<br />

he Contmunity Kater Cumittee Monitoring Fom is used to obtain<br />

infomation on changes in the organization and activities of the<br />

water comittees. It contaizs an activities checklist and<br />

questinns related to well ~anagement arzd village <strong>part</strong>icipation,<br />

It is designed to get the villagers perspectives on changes,<br />

problems and haw they may be solved.<br />
