Message from <strong>the</strong> President <strong>of</strong> <strong>the</strong> Malaysian Society <strong>of</strong> Allergy and <strong>Immunology</strong> (MSAI)Dear colleaguesIt is a great privilege for <strong>the</strong> Malaysian Society <strong>of</strong> Allergy and <strong>Immunology</strong> (MSAI) to organise <strong>the</strong> <strong>6th</strong> <strong>edition</strong> <strong>of</strong> <strong>the</strong> <strong>National</strong><strong>Clinical</strong> <strong>Immunology</strong> <strong>Symposium</strong> (NACLIS) with <strong>the</strong> <strong>the</strong>me “Creating Awareness <strong>of</strong> PID in South East Asia and Beyond”. Theplatform for 2013 NACLIS 6 has expanded into an international and global one from when it first started in 2009. The origin <strong>of</strong>NACLIS spawned from <strong>the</strong> need to organise educational programmes to increase awareness <strong>of</strong> Primary Immunodeficiencies(PID) amongst <strong>the</strong> medical fraternity, both clinical and scientific branches. This was <strong>the</strong> natural decision by a group <strong>of</strong>dedicated clinicians and scientists from both Health Ministry and Universities at 2 workshops organised by Universiti SainsMalaysia (USM) in 2007 as <strong>the</strong> <strong>National</strong> Initiative for Primary Immunodeficiencies (NPII) which eventually became <strong>the</strong>Malaysian Primary Immunodeficiencies Network, or MyPIN in 2009.Since 2009 NACLIS was held once or twice a year covering various aspects <strong>of</strong> PID, including <strong>the</strong> need for a subspecialty in<strong>Clinical</strong> <strong>Immunology</strong> and Allergy to address <strong>the</strong> need for better and optimal care <strong>of</strong> patients with PID and Allergy. Eventually,MSAI took up <strong>the</strong> challenge <strong>of</strong> continuing discussions with <strong>the</strong> Health Ministry and Academy <strong>of</strong> Medicine Malaysia inhighlighting <strong>the</strong> importance <strong>of</strong> <strong>the</strong> subspecialty by forming a subcommittee on Training and Credentialing <strong>of</strong> <strong>Clinical</strong> <strong>Immunology</strong>. This will be a feature <strong>of</strong>this conference with a forum to discuss pressing issues directly related to PID patient care and <strong>the</strong> subspecialty.It is <strong>the</strong> commitment and vision to ensure <strong>the</strong> best possible standards <strong>of</strong> management for PID patients that is continually emphasied, and measures areneeded for a comprehensive approach in handling those outstanding issues in <strong>the</strong> healthcare <strong>of</strong> PID patients in Malaysia and <strong>the</strong> region. As such, we arehonoured to be able to share experiences with eminent colleagues from Europe and Asia in managing <strong>the</strong>ir PID patients, including administrative matters.We are also please to have <strong>the</strong> International Patient Organisation for Primary Immunodeficiencies (<strong>IPOPI</strong>) participating in this conference to share <strong>the</strong>irexpertise as <strong>the</strong> global advocate for PID patient community and MSAI intends to collaborate with <strong>IPOPI</strong> to form <strong>the</strong> Malaysian PID patient care support group.It is envisaged key PID stakeholders including <strong>the</strong> Health Ministry and Universities will play integral roles towards realisation <strong>of</strong> optimal care for PID patients.Toge<strong>the</strong>r with our co-organisers UKM, UPM and USM, sponsors and <strong>the</strong> endorsement <strong>of</strong> this conference by <strong>the</strong> European Society for Immunodeficiencies(ESID), <strong>IPOPI</strong> and World Allergy Organization (WAO), we hope all participants will benefit from <strong>the</strong> sharing <strong>of</strong> knowledge and experience for <strong>the</strong> sake <strong>of</strong><strong>the</strong>ir patients. However recent epidemiologic studies suggest PID are more commonthan usually thought <strong>of</strong>. Reports are filtering from South East Asia that PID do exist and constitute an emerging disease and needs to beaddressed. These disorders receive less attention compared to <strong>the</strong> o<strong>the</strong>r established diseases. It is estimated that <strong>the</strong>re are 680,000PID patientsin Europe although from <strong>the</strong> many registries only 15,052 (2.27 %) are actually recorded; in Africa, it is estimated at 902,631 cases but only 1016detected (0.1%) Boushifa AA et al. JACI Jan 2013. This is because awareness is lacking <strong>the</strong>refore <strong>the</strong> disease is <strong>of</strong>ten underreported. The possibilitiesthat <strong>of</strong> <strong>the</strong> published reports <strong>of</strong> PID in South Asia, it is only between 0.1% - 2.27 % <strong>of</strong> its true prevalence. Published reports by Noh et al.Med J Malaysia, Feb 2013 identified 51PID cases between 1987- 2007.Based on <strong>the</strong> estimate by Boushifa AA <strong>the</strong> true prevalence could have been 2,157 - 51,000 in Malaysia within that period <strong>of</strong> 20 years. There is<strong>the</strong>refore more PID cases within <strong>the</strong> community and need to be identified.The problem is compounded by delay <strong>of</strong> diagnosis, hence delays in treatment with consequent high morbidity and mortality. It is more so in developingcountries by lack <strong>of</strong> awareness in <strong>the</strong> community. As long as <strong>the</strong> gravity <strong>of</strong> <strong>the</strong> problem is not felt, effort to improve patient care would bedelayed.
