Message from the President of the Malaysian Society of Allergy and Immunology (MSAI)Dear colleaguesIt is a great privilege for the Malaysian Society of Allergy and Immunology (MSAI) to organise the 6th edition of the NationalClinical Immunology Symposium (NACLIS) with the theme “Creating Awareness of PID in South East Asia and Beyond”. Theplatform for 2013 NACLIS 6 has expanded into an international and global one from when it first started in 2009. The origin ofNACLIS spawned from the need to organise educational programmes to increase awareness of Primary Immunodeficiencies(PID) amongst the medical fraternity, both clinical and scientific branches. This was the natural decision by a group ofdedicated clinicians and scientists from both Health Ministry and Universities at 2 workshops organised by Universiti SainsMalaysia (USM) in 2007 as the National Initiative for Primary Immunodeficiencies (NPII) which eventually became theMalaysian Primary Immunodeficiencies Network, or MyPIN in 2009.Since 2009 NACLIS was held once or twice a year covering various aspects of PID, including the need for a subspecialty inClinical Immunology and Allergy to address the need for better and optimal care of patients with PID and Allergy. Eventually,MSAI took up the challenge of continuing discussions with the Health Ministry and Academy of Medicine Malaysia inhighlighting the importance of the subspecialty by forming a subcommittee on Training and Credentialing of Clinical Immunology. This will be a feature ofthis conference with a forum to discuss pressing issues directly related to PID patient care and the subspecialty.It is the commitment and vision to ensure the best possible standards of management for PID patients that is continually emphasied, and measures areneeded for a comprehensive approach in handling those outstanding issues in the healthcare of PID patients in Malaysia and the region. As such, we arehonoured to be able to share experiences with eminent colleagues from Europe and Asia in managing their PID patients, including administrative matters.We are also please to have the International Patient Organisation for Primary Immunodeficiencies (IPOPI) participating in this conference to share theirexpertise as the global advocate for PID patient community and MSAI intends to collaborate with IPOPI to form the Malaysian PID patient care support group.It is envisaged key PID stakeholders including the Health Ministry and Universities will play integral roles towards realisation of optimal care for PID patients.Together with our co-organisers UKM, UPM and USM, sponsors and the endorsement of this conference by the European Society for Immunodeficiencies(ESID), IPOPI and World Allergy Organization (WAO), we hope all participants will benefit from the sharing of knowledge and experience for the sake oftheir patients. However recent epidemiologic studies suggest PID are more commonthan usually thought of. Reports are filtering from South East Asia that PID do exist and constitute an emerging disease and needs to beaddressed. These disorders receive less attention compared to the other established diseases. It is estimated that there are 680,000PID patientsin Europe although from the many registries only 15,052 (2.27 %) are actually recorded; in Africa, it is estimated at 902,631 cases but only 1016detected (0.1%) Boushifa AA et al. JACI Jan 2013. This is because awareness is lacking therefore the disease is often underreported. The possibilitiesthat of the published reports of PID in South Asia, it is only between 0.1% - 2.27 % of its true prevalence. Published reports by Noh et al.Med J Malaysia, Feb 2013 identified 51PID cases between 1987- 2007.Based on the estimate by Boushifa AA the true prevalence could have been 2,157 - 51,000 in Malaysia within that period of 20 years. There istherefore more PID cases within the community and need to be identified.The problem is compounded by delay of diagnosis, hence delays in treatment with consequent high morbidity and mortality. It is more so in developingcountries by lack of awareness in the community. As long as the gravity of the problem is not felt, effort to improve patient care would bedelayed. That would be unfortunate as specific treatment modalities are available and in some cases even cure if diagnosed early.Malaysian Society of Allergy and Immunology with the endorsement by European Society for Immunodeficiencies (ESID), and the InternationalPatient Organisation of Primary Immunodeficiencies (IPOPI) attempt to create awareness of PID for the region via this conference, addressing onthe relevant contemporary issues.The objectives of the Conference are:• To create awareness for Primary Immunodeficencies (PID) in the region of SE Asia• To appreciate the extent of the problem by sharing the experience of guest speakers with expertise in their field• To uplift quality of patient care through better diagnosis and management so as to recognize the disease earlier for early treatment• To develop training program towards creating the sub specialist with capabilities• To provide optimum diagnostic and therapeutic skills abreast with time relevant for developing countries• To engage the care givers, allied health workers, policy makers and most importantly the patients and their parents to providecomprehensive facilities for optimum patient care and laboratory support with policies favorable to patient well being• To form Parent-Patient support group who will then provide feedback to the professionals, policy makers, serving as advocacytowards their needs and aspirationTarget AudienceTarget Audience will include the medical fraternity (trainees, residents, specialists, public health doctors, etc), allied health care providers,nurses, laboratory immunologists, scientists, social workers, medical educators and policy makers from Malaysia, Singapore, Thailand,Philippines, Indonesia, China, India etc.
