Applicants will be provided the option to complete the HUD Form-92006, Supplement toApplication for Federally Assisted <strong>Housing</strong>, to identify an individual or organization thatthe PHA may contact and the reason(s) the individual or organization may be contacted.To provide specific accommodation to persons with disabilities, upon request, theinformation may be mailed to the applicant and, if requested, it will be mailed in anaccessible format.The full application is completed at the eligibility appointment in the applicant’s ownhandwriting, unless assistance is needed, or a request for accommodation is requested bya person with a disability. Applicants will then be interviewed by PHA staff to review theinformation on the full application form. Verification of disability as it relates to 504, Fair<strong>Housing</strong>, or ADA reasonable accommodation will be requested at this time. The fullapplication will also include questions asking all applicants whether reasonableaccommodations are necessary.When the Waiting List is open, families may obtain application forms at locationsdesignated in the PHA’s Public Notice.Completed pre-applications must be returned to the PHA by mail or as specified in thePHA’s public notice. Applications must be complete to be accepted by the PHA forprocessing. If an application is incomplete, the PHA will notify the family of theadditional information required.4-I.C. ACCESSIBILITY OF THE APPLICATION PROCESSElderly and Disabled Populations [24 CFR 8 and HCV GB, pp. 4-11 – 4-13]The PHA must take a variety of steps to ensure that the application process is accessible to thosepeople who might have difficulty complying with the normal, standard PHA application process.This could include people with disabilities, certain elderly individuals, as well as persons withlimited English proficiency (LEP). The PHA must provide reasonable accommodation to theneeds of individuals with disabilities. The application-taking facility and the application processmust be fully accessible, or the PHA must provide an alternate approach that provides full accessto the application process. Chapter 2 provides a full discussion of the PHA’s policies related toproviding reasonable accommodations for people with disabilities.Limited English ProficiencyPHAs are required to take reasonable steps to ensure meaningful access to their programs andactivities by persons with limited English proficiency [24 CFR 1].
