17.35 Commercial drivers who have declined an ICD or have an ICD implanted asprimary or secondary prophylaxisApplicationAssessment guidelinesFitness guidelinesOSMV determinationguidelinesConditionsRestrictionsRe-assessmentguidelinesThese guidelines apply to driver fitness determinations forcommercial drivers who: have had an implantable cardioverter defibrillator (ICD)implanted as a primary prophylaxis, or have declined an ICD recommended as primary prophylaxisWhen implanted as a primary prophylaxis, the ICD is implanted toprevent sudden cardiac death in individuals considered to be at highrisk but who have not had an episode of ventricular arrhythmia.Individuals whose ICD also regulates pacing for bradycardia mustalso meet the guidelines for permanent pacemakers outlined in 1.30.If further information regarding an individual’s medical condition isrequired, OSMV will request an assessment from a cardiologist.Individuals generally may not drive. However, an ICD maysometimes be implanted in an individual with a low risk of suddenincapacitation. Where this is the case, individuals may drive if anassessment by a cardiologist indicates that the annual risk of suddenincapacitation is 1% or less.OSMV may find individuals fit to drive if an assessment by acardiologist indicates that the annual risk of sudden incapacitation is1% or less.No conditions are required.No restrictions are required.OSMV will re-assess annually.195
