As reinforcement to verbal explanation, an information sheet is provided toapplicant explaining procedures for obtaining a taxicab driver’s permit (TaxiDriver Licensing Procedures New Applicant) (CCS-FRM-930) or (Taxi CabDriver Licensing Procedures Renewals) (CCS-FRM-931) .NOTE: By dating and initialing the received section of Chapter 45 Taxi CabsApplication this indicates form CCS-FRM-930 or CCS-FRM-931 was issued tothe driver.7. The Office Assistant then instructs the applicant to call the Transportation RegulationOffice in approximately 5 business days to find out if the application has beenapproved.8. At the end of each day, the Office Assistant types a list of applications received inalphabetical order to perform criminal background checks (Criminal BackgroundCheck Process CCS-WKI-943).9. Once criminal background checks have been received, the Administrative Assistantreviews the application and criminal background to determine whether the applicantqualifies for a license or permit based on the ordinance requirements.10. If the applicant is approved, the Office Assistant indicates approval on the upperright corner of application and also on the background list (Criminal BackgroundCheck Process CCS-WKI-943).10.1. If the Applicant does not qualify, the Administrative Assistant sends a TaxicabDriver Denial Letter (CCS-FRM-944) stating this to the applicant.11. When the applicant telephones for a status report, the Office Assistant informs theapplicant that the application has either been approved or denied.12. Applicant must then return to Transportation Regulation Office in order to bescheduled for next available Taxicab Driver’s Training Course (CCS-WKI-910).3 of 5

Applicant must attend Taxicab Driver’s Training Course and passboth sections of Training Course examination in order to qualify forpermit.12.1.1.2. Renewal drivers have option to obtain DVD recording of TaxicabDriver’s Training Course instead of attending all-day class.12.1.1.3. Once DVD is returned to Transportation Regulation Office,applicant is assigned a renewal test date.12.1.1.4. An information sheet Taxi Driver Licensing Procedures Renewals(CCS-FRM-931) is provided to driver explaining procedures,including date and location of renewal test.13.
