<strong>Recent</strong> OH&S <strong>Audit</strong> FindingsA recent OH&S audit of <strong>Yarra</strong> <strong>Valley</strong> <strong>Water</strong> capital works projects highlighted a number of issues that warrantyour consideration.Issue date: December 2009<strong>Yarra</strong> <strong>Valley</strong> <strong>Water</strong> periodically conducts internal OH&S audits of either our contractors engaged to undertake our capitalworks projects or contractors engaged to undertake land development works.<strong>Yarra</strong> <strong>Valley</strong> <strong>Water</strong> has recently completed an OH&S audit and identified the following issues –1. Material Safety Data Sheets- MSDSs are required for all dangerous goods and hazardous substances- MSDSs also need to remain current for the duration of all works- Need to ensure MSDSs are available for all dangerous goods and hazardous substances used on-site- Also applies to dangerous goods and hazardous materials used by any sub-contractors engaged on-site2. Hazardous substance / dangerous goods risk assessment- Risk assessment of hazardous substances & dangerous goods use, handling and storage, needs to becompleted and readily available on site- Also applies to dangerous goods and hazardous materials used by any sub-contractors engaged on-site3. Maintenance records- Maintenance records for all plant and equipment needs to comply with the requirements of the WorkSafeVictoria regulations- <strong>Recent</strong> discussion with WorkSafe Victoria indicates maintenance records need to be maintained and readilyavailable within 24 hours of request- Also applies to plant & equipment used by any sub-contractors engaged on-site4. Inadequate or no load restraint of equipment- All load restraint needs to comply with the National Transport Commission guidelines. Use the attachedhyperlink to access these guidelines- Also applies to load restraint required by any sub-contractors engaged on-site5. Plant Hazard Identification- All hazard control documentation, including hazard checklists, need to remain current and available within 24hours of request- Any identified corrective actions as a result of a plant risk assessment need to be addressed as closed outand documented within an appropriate time-frame- Particular attention shall be given to mobile plant, warning devices- The principal contractor needs to ensure all engaged contractors comply with the above requirements


pPagePage6. Induction Records- Induction records for all contract and sub-contract employees and site visitors need to be completed andreadily available on-site upon request7. Formwork and Concreting Safe Work Method Statements- SWMSs are required for all high risk activities, in particular for erection/removal of formwork and associatedconcrete works- SWMSs are also required to be present on-site and readily available on request8. Faulty Fire Extinguisher- Please check the condition of all on-site emergency equipment to ensure it is ready & available for immediateuse9. Site Housekeeping- Please ensure all sites, including crib huts, storage sheds are well maintained10. Manual Handling- It is recommended that heavy equipment and materials are stored at the front of storage facilities for ease ofhandling and to minimise the need for difficult manual handling11. First Aid Kits- All First Aid kits need to be regularly inspected and replenished, where necessary. Particular care is to betaken to discard items that are past their use by date.12. Safety Inspection Checklist- A safety inspection checklist needs to be completed in accordance with the adopted project managementplanWhilst all of the above issues are currently being addressed with the respective contractors involved, all contractors needto note that the above items will be the focus of future audits of our capital works and land development projects and youare requested to ensure you have adequately addressed each of these items.For more informationContact........................................................
