FORMATS FOR SUBMISSION OF FIRMS CREDENTIALSThe proposal should contain the following information in enclosed formatattached at Appendix A.• Year of Establishment of Firm• Average annual turnover (last three years)Note: The Firm shall submit Certificate of Incorporation and audited balancesheet for the last three years (FY 2008-09 , FY 2009-10 and FY 2010-11). Forclaiming experience of Road projects completion ongoing certificate fromemployer should be enclosed. The proposal should also contain the details ofthe key personnel viz. their name, qualification, expertise area, experience andyears of association with the firm.Appendix AThe following information related to the firm should be provided in the proposal.i.
