A recent ten-year follow-up of a cohort of 296 adults in a remote central Australian Indigenouscommunity from 1995-2004 found significantly lower CVD and all-cause mortality andhospitalisation rates compared to other <strong>NT</strong> Indigenous adults. CVD standardised mortality ratio(SMR) was 0.57 and SHR was 0.34 (88) . This finding was surprising in the context of a recentreview of that particular health service, which found that chronic disease care systems (registers,care planning and recall systems) were relatively poorly developed, but the health service modelsupported the preferred decentralised lifestyle of the population. This was a communitycontrolledhealth service, funded by OATSIH (89) .In summary, there is very little published and routinely reported data which can directly linkactivities identified in the <strong>NT</strong><strong>PCD</strong>S with clinical care processes at a primary care level,intermediate clinical outcomes relevant to the <strong>PCD</strong>S and longer term outcomes such as avoidablehospitalisations and mortality in the <strong>NT</strong> to date.Chapter 4: Clinical Outcomes – <strong>Evaluation</strong> of the <strong>NT</strong> Preventable Chronic Disease <strong>Strategy</strong> 2007 77
