<strong>Key</strong> <strong>Executives</strong>Michael L. Cotton – CEO, <strong>Alere</strong> HealthMr. Cotton became CEO of <strong>Alere</strong> Health in January 2012. Previously, he joined<strong>Alere</strong> in September 2010 as president of <strong>Alere</strong>’s Health Improvement Division.With more than 22 years of managed care and healthcare consultingexperience, Mr. Cotton came to <strong>Alere</strong> from WellCare Health Plans, Inc., wherehe served as President of National Health Plans, overseeing the company’shealth plan markets, operations, national provider contracting and businessdevelopment.Prior to WellCare, Mr. Cotton served as a worldwide partner and market leaderfor Mercer Human Resources Consulting from October 2001 to December2005. Before Mercer, he held the positions of president and chief executiveofficer of Mid-Valley CareNet, a physician hospital organization (PHO) withmore than 600 physician members. Earlier in his career, Mr. Cotton was aregional president and chief executive officer for AMERIGROUP Corporationand senior vice president and chief operating officer of QualChoice HealthPlans, Inc., a managed care plan of the University Hospital’s Health System.Mr. Cotton earned his undergraduate degree from Franklin University and hismaster’s degree in business administration from Cleveland State University.Gregg E. Raybuck – President, <strong>Alere</strong> HealthMr. Raybuck became President of <strong>Alere</strong> Health in February 2012. He isresponsible for overall company strategy, performance and growth, providingleadership and direction to operations, sales, marketing, program developmentand reimbursement. He has total problem-solving responsibility for <strong>Alere</strong>Health’s operations and expansion strategies. Previously, he served asPresident of the Women's and Children's Health Division beginning in


November 2006. Prior thereto, Mr. Raybuck served as Senior Vice President ofSales for Women's and Children's Health beginning in August 2006. Mr.Raybuck was associated with Matria Healthcare from 1992 to January 2001,last serving as President of the Cardiology business unit of the company. Priorto his appointment as President of the Cardiology business, Mr. Raybuck heldthe position of Vice President of Sales and Marketing for Women’s Health.Prior to his return to Matria before it was acquired, Mr. Raybuck served as VicePresident of Sales and Marketing for CardioNet, Inc., a technology and serviceprovider of remote wireless monitoring technology for cardiac patients from2004 to 2006. Prior thereto, he served as President and CEO of NorthAmerican operations for Card Guard Inc., a global provider of medical devicesand cardiac monitoring technology.Mr. Raybuck earned a bachelor’s degree in business from Thomas EdisonState College.Sanford Becker – President, <strong>Alere</strong> WellbeingMr. Becker joined <strong>Alere</strong> Wellbeing (formerly Free & Clear ® ) in February 2005and has served as CFO and COO prior to becoming President in June 2011.His professional experience includes more than 35 years in management,operations, accounting and finance.Prior to <strong>Alere</strong>, Mr. Becker held senior management positions at severalprivately-held, venture-backed companies, including Radiant Research (aleading clinical research organization); Applied Discovery (a leader in providingWeb-enabled business services to the legal industry, sold to LexisNexis); andPointshare Corporation (a provider of online services to the healthcare industry,sold to Siemens Medical Solutions). He was a managing member of CompassCapital Advisors, a private merchant-banking firm that specialized in financinghigh-growth, privately held companies. Before that, Mr. Becker was a partner atMoss Adams CPAs.Mr. Becker’s particular areas of expertise include operations, financialmanagement, strategic business growth and development, mergers andacquisitions and structuring debt and equity financings. During his career, Mr.Becker has arranged more than $1.4 billion in debt and equity transactions.He earned a bachelor’s degree in business administration from the University ofWashington, and he is a Certified Public Accountant.Page | 2


Craig Apolinsky – Executive Vice President & General CounselMr. Apolinsky has overall management responsibility for <strong>Alere</strong>’s LegalDepartment. Previously, Mr. Apolinsky served as Vice President, GeneralCounsel and Corporate Secretary for Merge Healthcare, Inc., a publicly-tradedcompany that develops, licenses, implements and supports medical imagingand information software. He led Merge’s worldwide legal affairs for their U.S.,Canadian, Indian and European operations.Prior to Merge, he served as Deputy General Counsel and Assistant Secretaryfor Gold Kist, Inc. He was responsible for securities compliance, corporategovernance, contracting and licensing matters for this publicly-traded companywith annual revenues in excess of $2 billion. He began his legal career withAlston & Bird LLP, a prominent national legal firm, and practiced law with thefirm until joining Gold Kist. Craig was a partner with Alston & Bird in itsCorporate Health Care and Technology Practice Groups.Mr. Apolinsky earned his law degree from the University of Texas at Austin,School of Law and earned his bachelor’s degree from the University of Virginia.Joseph Blankenship – Executive Vice President & ChiefFinancial OfficerMr. Blankenship became Chief Financial Officer of <strong>Alere</strong> in March 2010.Previously, he served as Vice President and Corporate Controller, first forMatria Healthcare (acquired by Inverness Medical Innovations, Inc. in May2008) beginning in December 2004, and then for <strong>Alere</strong> beginning in May 2008.Prior to joining <strong>Alere</strong>, Mr. Blankenship held various senior executive financialpositions for both private and public companies. He served as Controller for AirServ Corporation from 2002 to 2004 and held the positions of Senior VicePresident, Chief Financial Officer, Treasurer and Assistant Secretary while atHealthcare.com from 1999 to 2001. Mr. Blankenship served as CorporateController of Decorative Home Accents, Inc., a textile manufacturer, fromOctober 1996 to December 1998, and as Director of Accounting of AllegiantPhysician Services, Inc., a healthcare services company, from April 1994 toPage | 3

A Certified Public Accountant, Mr. Blankenship earned abachelor’s degree from West Virginia University.Pete Desai – Executive Vice President, Health ManagementStrategyMr. Desai has more than 10 years of health industry experience with anextensive background in developing and implementing innovative healthcareprograms and services. In his current role, Mr. Desai is responsible for drivingstrategy and business development, while supporting the global healthmanagement function, working with leadership to grow services and revenue intargeted geographies. Previously, Mr. Desai served in the roles of Senior VicePresident of Health Strategy & Global Solutions beginning in 2011, Director ofStrategy & Business Development beginning in 2008, and Director of StrategicProjects beginning in 2007.Previously, Mr. Desai served in various roles in Accenture’s Health & LifeScience’s practice from 1999 to 2007, where he developed a broad healthcarebackground including experience working with health insurance payers,providers and behavioral health vendors. While at Accenture, Mr. Desai’sresponsibilities ranged from defining corporate strategy and growth plans forclients to managing large-scale multi-million dollar implementation projects, allof which helped him build a core foundation in managing complex initiatives.Mr. Desai is a graduate of Vanderbilt University, where he earned a Bachelor ofScience degree in biomedical engineering.Julie Griffin – Executive Vice President, Culture & PerformanceAs Executive Vice President of Culture & Performance, Ms. Griffin isresponsible for organizational development and human resource activitiesacross <strong>Alere</strong>’s business units. She is accountable for creating a culture built on<strong>Alere</strong>’s six core values of teamwork, excellence, health, accountability, trustand innovation – a culture in which associates can develop and succeed. Ms.Griffin also works closely with the C&P Business Partners to build aperformance strategy consistent with <strong>Alere</strong>’s leadership.Ms. Griffin first joined <strong>Alere</strong> as Vice President of Human Resources when it wasstill Matria Healthcare in September 2006. ole in September 2009. Ms. Griffin offers 15 years of progressive managementexperience in human resources with a wide range of experience in the areas ofaccounting, administration and operations. She has served in a variety ofleadership roles for Loyaltyworks, Inc., Benecorp, Quovadx, Inc. (a spin-off ofMatria Healthcare), Scottish Rite Children’s Medical Center and AllegiantPhysician Services, Inc.Ms. Griffin attended Georgia State University and is certified by the Society forHuman Resource Management.Garrett Hale – Executive Vice President, Clinical OperationsMr. Hale has served as Executive Vice President of Clinical Operations for<strong>Alere</strong> Health since March 2012. Previously, he served in executive leadershiproles for the Women’s and Children’s Health Division, starting with VicePresident when he joined the company in 2005, followed by Senior VicePresident, and later Executive Vice President of Operations beginning in 2008.His diversified experience in process improvement, operations management,change leadership, organizational design, Six Sigma process reengineering(Black Belt), complex project management, and call center leadership provideshim with the insight and experience necessary to work with and understand theneeds of clients. Mr. Hale works both externally with clients and internally withthe product development team to ensure the products and services <strong>Alere</strong>delivers are not only in line with current trends, but also anticipate the future ofthe marketplace.Prior to <strong>Alere</strong>, Mr. Hale served as Senior Manager with The North HighlandCompany, where he consulted with Fortune 500 companies specializing inorganizational change management, process improvement, organizationaldesign and complex program management. Steven Kelley – Executive Vice President, TechnologySolutionsSteven Kelley has served as Executive Vice President of Technology Solutionsfor <strong>Alere</strong> Health since February 2012. He is responsible for all aspects of IT,including application development and support, IT operations andinfrastructure, and network and telecom. Steven joined <strong>Alere</strong> in February2011. Previously, Mr. Kelley was the Owner and Principal of Standard Door, alocal-Atlanta independent manufacturing company. Prior to Standard Door,Steven was an Associate Partner with Accenture, where he spent 20 yearsdelivering IT consulting services to Fortune 500 clients.Mr. Kelley brings more than 25 years of leadership experience deliveringoperational results both as a small business owner and in high-performancecorporate environments. He has a strong background in business operationsas well as extensive experience delivering large-scale strategic changeprograms and IT projects across a broad range of industries, includinghealthcare, financial services, telecommunications, and manufacturing.Steven earned a Bachelor of Science degree from Vanderbilt University and amaster’s degree in business administration from the University of NorthCarolina at Chapel Hill.Craig <strong>Key</strong>es, MD, MBA – Executive Vice President & ChiefMedical OfficerDr. <strong>Key</strong>es joined <strong>Alere</strong> Health in March 2011 as Executive Vice President andChief Medical Officer. In this role, he is responsible for the direct oversight ofour enterprise Quality and Accreditation programs, the clinical solutions that wedeliver through our various programs, and the measurement of theeffectiveness of the programs we deliver to our clients.A lifelong advocate of health and well-being, Dr. <strong>Key</strong>es most recently foundedSportXcel, a youth sports performance and adult fitness enterprise in MetroDenver, which provides both commercial and charitable research-based fitness,conditioning and injury-prevention programs for youth, adults, seniors and eliteathletes. Prior to SportXcel, Dr. <strong>Key</strong>es held senior executive positions over a10-year span with United Healthcare from 1998-2008, including CEO-NewYork, CEO-Colorado/New Mexico, National SVP of Mid-Market Sales, andChief Medical Officer of the 16 markets in the North Division.Page | 6


Dr. <strong>Key</strong>es has also held prior positions as a Physician Surveyor for the NationalCommittee for Quality Assurance and as Medical Director for IPRO, a nationallyrecognized healthcare assessment and improvement organization. Dr. <strong>Key</strong>esbegan his career in the practice of medicine serving under- and un-insuredpeople with HIV/AIDS in New York City.Dr. <strong>Key</strong>es earned his undergraduate and medical degrees, both with Honors, atthe University of Wisconsin, and completed his internship and residency atColumbia University and Lenox Hill Hospital in New York City. He is BoardCertified in both Internal Medicine and Geriatrics. He also earned a master’sdegree in business administration from Columbia University School ofBusiness.Scott McClintock – Executive Vice President & Chief MarketingOfficerMr. McClintock has led Marketing for <strong>Alere</strong> since January 2011. Previously, heserved as Senior Vice President of Marketing for <strong>Alere</strong> beginning in 2008. Priorto this role, he served as Chief Marketing Officer of Matria Healthcare (acquiredby Inverness Medical Innovations, Inc. in May 2008) beginning in December2006. Prior to joining Matria, Mr. McClintock served as Senior Vice President,Head of Global Marketing for Premiere Global Services, a $500 millioninternational leader in teleconferencing services. He was responsible for allaspects of marketing for PGI. Prior thereto, he headed up branding, marketingand communications for Equant (now Orange Business Services), a $3 billioninternational leader in providing data communication solutions to Global 500companies. He joined Equant at its inception and was instrumental in growingthe company from startup, through its IPO and finally to its sale to FranceTelecom.Prior to Equant, Mr. McClintock served as Vice President, ManagementSupervisor for several advertising agencies including BBDO, Ogilvy & Mather,Rubin Postaer, and WayneWest, where he worked on the Shell Oil, Compaq,MediaOne (now Comcast), Pizza Hut and BellSouth accounts. He began hiscareer as a systems engineer with Raytheon and brings more than 20 years ofmarketing and communications experience to <strong>Alere</strong>.Mr. McClintock earned his bachelor’s degree from the University of Texas,Austin, where he double-majored in computer science and marketing.Page | 7

Norman, MD, MBA – Executive Vice President &Chief Innovation OfficerDr. Norman joined <strong>Alere</strong> in 2005 when <strong>Alere</strong> purchased PacifiCare's diseasemanagement business and became their exclusive disease managementvendor. In 2004, Dr. Norman was named among the Top 25 Most InfluentialPeople in Disease Management by Managed Healthcare Executive magazine.In that year, he also joined the Disease Management Association of America’s(DMAA) Board of Directors. Prior to joining <strong>Alere</strong>, Dr. Norman served asExecutive Director of PacifiCare's dedicated disease management unit. He alsoserved as Vice President of Health Care Quality for PacifiCare, as well asRegional Medical Director and Senior Plan Medical Director for PacifiCareof California.Before joining PacifiCare, he earned a master’s degree in businessadministration from Stanford University, where he previously earned degrees inpsychology and medicine.
