COs and Officers in Charge (OICs) are a reporting senior byvirtue of their command authority regardless of their rank.An Enlisted OIC in the grade of E9 and civilians in commandpositions who hold the grade of GS-9 through GS-12 maysign reports on E5 and below.Reporting senior authority for enlisted OICs in the grade ofE7 and E8 may be delegated to sign E5 reports with the priorwritten approval of NAVPERSCOM (PERS-32).A CPO or SCPO may sign reports on E4 and below ONLY.GS-13 or equivalent may sign reports for E1 to E9.


E1-E4 can be an E6 or civilian equivalent.E5-E6 SHOULD BE a Navy CPO whenever possible.If none is available within the Command, the ratermay be a military or civilian supervisor who is anE7 equivalent or higher.Typically, the senior rater will be the membersdivision officer or department head.

The member shall receive a copy of every report fromthe reporting senior at the time it is signed.The member has the right to submit a statement to therecord concerning their report, either at the time of thereport or within 2 years thereafter. Such a statementShall be endorsed by the original reporting senior andForwarded to NAVPERSCOM (PERS-32).If you wish to submit a statement before the report isfiled, you must do so within 10 days after you receivethe report. You may request assistance in preparingyour statement. Your statement may not exceed twopages and may not have enclosures.

For reporting period in which a member has failed3 or more PFAs in the most recent 4-year period,Enlisted members shall receive:* A grade no greater than 1.0 in "Military Bearing"or Professionalism (CHIEFEVAL block 35 or EVALblock 36).* Marks of "Significant Problems" and "RetentionNot Recommended” (Eval block 45 and block 47),Respectively.

Non-participationin entire PFA (BCA and PRT) due to deployment/operational(Dep/Op), Individual Augmentee Manpower Management (IAMM),Global Support Assignment (GSA), Overseas ContingencyOperation Support Assignment (OSA), pregnancy, temporaryadditional duty (TEMADD) or excused. (Note: BCAS shall beaccomplished on all members even if onboard less than 10weeks or unable to participate in PRT due to Dep/Op. Dep/Opfor BCA must be approved by the immediate superior incommand (ISIC). Use of this code does not requirejustification for PRT non-participation in the comments onperformance block.

(Should appear on most reports.)Member of the basic group if subcategories are used.Example: Training activities that subcategorize instructors andstudents will enter “BASIC” for administrative staff.Mandatory requirement to use “Approved” in block 21 for anyonewho has a Resignation or Retirement or Fleet Reserve dateapproved.Mandatory requirement to use “INDIV AUG” in block 21 foractive duty (including FTS) members under NAVPERSCOM(PERS-4G) orders who have been at their parent command forless than 240 days or for periodsof performance less than 240 days. If the member was at theparent command for greater than or equal to 240 days of thereporting period, this billet subcategory cannot be used. Regularreporting seniors must also use this billet subcategory for anyregular performance evaluations required (e.g., periodic,detachment of individual, detachment of reporting senior) with areporting period end date less than or equal to 180 days after amember has returned to their parent command from an IAassignment. The 180 day period does not apply if a Sailortransfers to a new parent command. IA supported commandswill enter “INDIV AUG” code on all concurrent performance


60 percent combined early and must promoteupper limits will become effective 15 March2011 for E5s.If the member is not expected to return to or visitthe command within 15 days (active duty) or 30days (inactive duty) and the report is notadverse, the reporting senior shall enter in thesignature block of the member “certified, copyprovided.”Mailing Procedures. Mail reports within 15 daysof the ending date for active duty members andwithin 30 days for inactive duty members.


Letter of ExtensionA Periodic report may beextended by letter for up to 3months in place of a Detachmentof Individual, Detachment ofReporting Senior, of a Frockingreport.


Used to fulfill reporting requirements when graded reportsare inappropriate. Submitted for short periods of duty orTEMDU (no more than 3 months) and is purely foradministrative or training purposes, academic DUINS, dutiesthat require a member's complete independence from anyappearance of command influence, and Regular reportperiods that have been fully evaluated in a Concurrent orOperational Commander report. Under uniquecircumstances, a reporting senior can submit a NOB reportfor any period if the reporting senior does not feel that therehas been enough observation to grade with confidence, i.e.,member has been onboard for 6 months but member wasTEMADD for a majority of the reporting period, etc.Justification and explanation must be provided in theComments field.


Provides performance information to another command forconsideration in a FITREP, CHIEFEVAL or EVAL. A FITREP,CHIEFEVAL, or EVAL is the preferred report for significant,observed performance. A PIM may be prepared for thefollowing situations:a. Duty, DUINS, TEMDU, or TEMDUINS for a period of 3months or less, in place of a Regular FITREP, CHIEFEVAL,or EVAL report.b. ADDU, TEMADD, or CAI of any length, as an alternative toa Concurrent report.c. Performance not under written orders that should bebrought to the reporting senior's attention.Forward within 15 days to the command that will preparethe Regular FITREP, CHIEFEVAL, or EVAL report for theperiod covered.
