PCAB License and Registration – under Eligibility Documents – item v, Certified True Copyof renewal application of PCAB license/registration signed by the owner/bidder may beallowed to be supported by Official Receipt (OR) as proof of their registration.2. In relation to item no. VII on Bid Security, page 89, of the Terms of Reference/Guidelines,attached is a form of the “ Bid-Securing Declaration” (Annex A)3. Paragraph No. 4, item no. XVI. Progress Payments. Section XI. Terms of Reference /Guidelines (p. 93) should be read as follows :”The first progress payment may be paid by the Procuring Entity to the Contractor providedthat at least twenty five percent (25%) (not twenty percent) of the work has beenaccomplished as certified by the Procuring Entity’s Representative.”4. The Bidder must submit a Certificate of Site Inspection of the project at NFA-Bulacan to beissued by the Provincial Manager concerned. This shall be included in the TechnicalDocuments under Project Requirements (ii.4 of the BDS).5. In addition to Specifications. Section VI, Rotary Ventilators’ dimension is 100 cm dia. x 100cm high (1 m x 1 m) and Stainless Gutters sheets with 0.50 mm thickness, 202 series.6. Item XVIII, paragraph no. 16,a – c, page 95, The Contractor’s Obligations, of the Terms ofReference/Guidelines, All Risk and other Insurance Coverage shall no longer be required.7. Sealing and Marking of Bid :a) The prospective bidder/s shall submit its bid/s using the forms specified in theBidding Documents in two (2) separate sealed envelopes, and which shall besubmitted simultaneously. The first envelope shall contain the technicalcomponent of the bid, and the second envelope shall contain the financialcomponent of the bid in order to qualify for the bid.b) Bidder/s shall enclose the original technical component in one sealed envelopemarked “ORIGINAL/COPY NO. 1 – TECHNICAL COMPONENT” and the originalfinancial component in another sealed envelope marked “ORIGINAL/COPY NO. 1 –FINANCIAL COMPONENT.”

c) The bidder/s shall also reproduce and submit two (2) photocopies of the technicaland financial component of the bid which shall be enclosed in their correspondingsealed envelopes marked as “COPY NO. 2 - TECHNICAL COMPONENT”, “COPYNO. 2 – FINANCIAL COMPONENT”, “COPY NO. 3 - TECHNICAL COMPONENT”and “COPY NO. 3 – FINANCIAL COMPONENT.”d) All copies (envelopes) of the Technical Component shall be sealed in one envelopemarked as “FIRST ENVELOPE - TECHNICAL COMPONENT.”e) All copies (envelopes) of the Financial Component shall be sealed in one envelopemarked as “SECOND ENVELOPE - FINANCIAL COMPONENT.”f) The FIRST (TECHNICAL) and SECOND (FINANCIAL) Envelopes shall then becontained in one (1) sealed envelope for submission on or before the deadline.g) All photocopied documents shall be duly certified as TRUE COPY of the originalby the owner or his authorized representative.– to be included in the Terms of Reference/Guidelines and Bid Data Sheet as specified inInstruction to Bidders, Clause 20. Section II.
