QuestionnairesFour sets <strong>of</strong> questionnaires were used in the survey forboth samples: 1) a household questionnaire which wasused to collect information on all de jure householdmembers (usual residents), the household, and thedwelling; 2) a women’s questionnaire administered ineach household to all women aged between 15–49 years;3) an under-5 questionnaire, administered to mothers orprimary caretakers for all children under 5 living in thehousehold; and 4) a men’s questionnaire administered ineach household to all men aged 15–29 years.The Household Questionnaire included the followingmodules:• Household Listing Form• Education• Water and Sanitation• Household <strong>Characteristics</strong>• Child Discipline• HandwashingThe Questionnaire for Individual Women wasadministered to all women aged 15–49 years living in thehouseholds, and included the following modules:• Woman’s Background• Access to Mass Media and ICT• Child Mortality• Desire for Last Birth• Maternal and Newborn Health — only selectedquestions• Illness Symptoms• Contraception• Unmet Need• Attitudes Toward Domestic Violence• Marriage/Union• Sexual Behaviour• HIV/AIDS• Life satisfactionThe Questionnaire for Children Under Five wasadministered to mothers or caretakers <strong>of</strong> children under5 years <strong>of</strong> age 1 living in the households. Normally, thequestionnaire was administered to mothers <strong>of</strong> under-5children; in cases when the mother was not listed in thehousehold roster, a primary caretaker for the child wasidentified and interviewed. The questionnaire includedthe following modules:• Child’s Age• Birth Registration• Early Childhood Development• Breastfeeding• Care <strong>of</strong> Illness• AnthropometryThe Questionnaire for Individual Men was administeredto all men aged 15–29 years living in the households, andincluded the following modules:• Man’s Background• Access to Mass Media and ICT• Marriage/Union• Contraception• Attitudes Toward Domestic Violence• Sexual Behaviour• HIV/AIDS• Life satisfactionThe questionnaires are based on the MICS4 modelquestionnaire 2 .
