The state report on BednetDelivery and Impregnation should reach <strong>NVBDCP</strong> within 45 days of completion all activity in the state.E. District Annual Stock report on Insecticides (VC 5)The district should furnish the detailed PHC wise stock report on insecticide usage during the year in VC5. The report corresponds with the Calendar year (1 st January to 31 st December). The columns are selfexplanatory. The report should be compiled by the district from PHC stock registers within 15 days ofcompletion of the reporting year. The state should compile and <strong>for</strong>ward the report within 30 days ofcompletion of reporting year to <strong>NVBDCP</strong>. The Annual Stock Report on Insecticides <strong>for</strong> the year 2008should reach <strong>NVBDCP</strong> by 31 st January of 2009.F. District LLIN Log (VC 6)Data on annual distribution of LLINs is entered into District LLIN Log (VC6) at the end of each year fromVC4. For the annual planning, the cumulated number of LLINs is calculated from VC6. For LLINs withan expected effective life of 3 years sum the numbers distributed over the last 2 years is taken. Whenplanning <strong>for</strong> 2011, the numbers distributed in 2009 and 2010 should be used (LLINS distributed in 2008will expire during 2011 and must be replaced). For LLINs with an expected effective life of 5 years sumof the numbers distributed over the last 4 years is taken. When planning <strong>for</strong> 2011, the numberdistributed in 2007-2010 are added. LLINs delivered through ANC must also be included. If LLINs withtwo different durations are included, use two separate <strong>for</strong>ms <strong>for</strong> keeping log. Besides when planning <strong>for</strong>LLIN distribution, the village level bednet surveys undertaken to enumerate the nos existing in eachvillage also needs to be undertaken.Programme Management Monitoring Report (PMMR)This report is to monitor progress made on different programme processes and other managementissues. Update on Training status of the staff as well as the <strong>training</strong>s conducted, field visits & reviewsconducted and reviews undertaken as well as situation of logistics & stock outs are to be provided on aquarterly basis. The <strong>for</strong>mat is given in the Operational Manual. It has the following sections:Part A: Field visits & reviewsPart B: Quality of service deliveryPart C: Training ActivityPart D: BCC Activity <strong>for</strong> Malaria ControlPart E: Status of LogisticsA new cadre of Monitoring and Evaluation (M & E) staff in the <strong>for</strong>m of Malaria Technical Supervisor(MTS) is being appointed at sub-district level. It is also envisaged to implement Lot Quality AssuranceSampling (LQAS) based system of annual/ biannual/ quarterly surveys to obtain quality data onavailability of diagnosis & treatment within 24 hours, on utilization of bed-nets and quality of IRScoverage and reasons <strong>for</strong> non-acceptance.A network of sentinel sites are being established which will provide data on trends of severe malaria anddeaths due to malaria; 1-2 sentinel sites will be established in each high endemic district.55
