SAMPLE FORM FOR OBTAINING AN ITALIAN TAX IDENTIFICATION CODE WITHINSTRUCTIONS ON HOW TO FILL IN EACH SECTIONAs stated in the <strong>Italian</strong> <strong>Tax</strong> <strong>Booklet</strong>, a <strong>Tax</strong> Identification Code (made up of 16 alphanumeric symbols <strong>–</strong>numbers and letters) is a means of identifying each citizen for the purpose of managing his/herrelationship with <strong>Italian</strong> public offices and administrations. In order to be valid, this code must beregistered in the <strong>Tax</strong> Register under the domain of the <strong>Italian</strong> Revenue Agency (“Agenzia delle Entrate”).An <strong>Italian</strong> <strong>Tax</strong> Identification Code may be obtained through the local <strong>Italian</strong> Consulate. A non-<strong>Italian</strong> resident may, in special circumstances, also request aplastic-coated card containing the <strong>Tax</strong> Identification Code (which is delivered to the local <strong>Italian</strong>consulate and then, in turn, to the applicant).As an alternative, the <strong>Italian</strong> <strong>Tax</strong> Identification Code may be obtained through an <strong>Italian</strong> Chartered <strong>Tax</strong>Advisor.In order to request a <strong>Tax</strong> Identification Code, non-<strong>Italian</strong> resident Shareholders can submit to the local<strong>Italian</strong> consulate the form set out in the next page, together with the original and copies of theiridentification documents, such as the passport or identity card (in order to allow the <strong>Italian</strong> consulate tocheck the accuracy of the data filled in the form).For Hong Kong resident Shareholders, please refer to instructions starting from page 46.39
