14Standards for female and male Sterilization services1.6.2. Post-operative complicationsa)b)c)d)e)Wound sepsis: Small stitch abscess is to be treated with drainage and dressings.However, severe sepsis needs opening of the incision and drainage of pus.Further, treatment should be done with dressings, antibiotics, and analgesics.Haematoma in the abdominal wall: A small non-expanding, non-infectedhaematoma will resolve with no therapy. A large one, particularly if infected, mayneed drainage and treatment with antibiotics.Intestinal obstruction, paralytic ileus, and peritonitis: The client shouldbe hospitalized if she is not already in hospital. Keep the patient on nothingby mouth, put nasogastric suction, give IV fluids, antibiotics, and analgesics asindicated, and refer to a higher centre, if required.Tetanus: If tetanus is diagnosed, the patient must be transferred immediately toa proper centre for treatment.Incisional hernia: A rare complication that needs surgical treatment.1.6.3. Failure of operation leading to pregnancyThis may be due to either technical deficiency in the surgical procedure or spontaneousre-canalization. To detect failure leading to pregnancy at the earliest, the client should beadvised to report to the facility immediately after missed periods. The client should be offeredMTP and repeat sterilization surgery or should be medically supported throughout thepregnancy if she so wishes. Ectopic pregnancy must be ruled out as tubectomy predisposesto this condition. Each case of sterilization failure should be reported to the District QualityAssurance Committee. The District Quality Assurance Committee will conduct a preliminaryinvestigation and report to the State Quality Assurance Committee.All cases of failure and complications, major or minor, arising during surgery or postsurgerymust be documented. The complications that required hospitalization and all casesof failure must be reported to the district quality assurance committee. The district qualityassurance committee will in turn be responsible for communicating such information tothe concerned insurance service providers for compensation.

But these are not complications of sterilization. Reassuranceand treatment according to the cause are required in most cases.Chronic pelvic inflammatory disease: This usually presents itself as lowerabdominal pain and requires treatment with bed rest, antibiotics, and analgesics.Psychological problems (e.g. depression): Discussing the problem, clarifyingthe role of sterilization, and answering questions are important steps.
