1. Martha Francis is 39 years old. Her mo<strong>the</strong>r had ovarian cancer and was found<br />

to have a deleterious BRCA1 mutation. She has two sisters, a husband, and a<br />

daughter. She is not <strong>of</strong> Ashkenazi Jewish descent.<br />

2. She makes an appointment at a risk clinic. The clinic instructs her to use <strong>the</strong><br />

Surgeon General’s Family History (Yoon, 2002) <strong>Web</strong>-based tool to prepare for<br />

<strong>the</strong> visit. She brings up <strong>the</strong> Surgeon General’s Family History tool and enters<br />

her family history.<br />

3. She <strong>the</strong>n sends <strong>the</strong> data to <strong>the</strong> risk clinic prior to her appointment with that<br />

clinic, where a CAGENE application receives <strong>the</strong> data.<br />

4. The counselor at <strong>the</strong> risk clinic (nurse geneticist, nurse practitioner, genetic<br />

counselor, doctor, etc.) uses <strong>the</strong> CAGENE application (a pedigree drawing<br />

program that runs risk models), where <strong>the</strong> patient’s family history already has<br />

been received. The counselor edits <strong>the</strong> data after confirming and clarifying<br />

various issues with <strong>the</strong> patient and adds additional information that was not<br />

entered at home.<br />
