AVDCAssistant Clinical Professor – University of Minnesota College of Veterinary Medicinekstepani@umn.eduINTRODUCTIONNormal dental radiology requires understanding normal dental anatomy.Dogs, cats, horses, cows, etc. are diphyodont [two sets of teeth: primary (deciduous) and adult]. Eachtooth has a crown and a root. The apex of the root is the terminal end of the root where theneurovascular bundle enters the tooth. The cusp is the terminal end of the crown.The tooth is composed of organic and inorganic material. The 3 hard tissues of the tooth include theenamel (crown only), dentin (root and crown) and cementum (root only). The cementum and enamelmeet at the cementoenamel junction (CEJ). A tooth is a living structure and dentin is continuallyproduced throughout the life of an animal, if the tooth remains vital. Odontoblasts produce dentin andare located in the pulp with the blood vessels, lymphatics, and nerves. The pulp (endodontic system) isdivided into the root (pulp) canal (in the root), the pulp chamber (in the crown), and the pulp horns (inthe cusps of the crown).The tooth is anchored in the jaws by the periodontium. The incisive bones, maxillary bone, andmandibular bone anchor the teeth. The periodontium consists of the 1) gingiva, 2) alveolar bone, 3)periodontal ligament, and 4) cementum. Radiographically, the alveolar bone and periodontal ligamentspace are commonly evaluated to assess periodontal disease. Normal cementum cannot be seenradiographically as it consists of just a few cell layers.<strong>NORMAL</strong> RADIOGRAPHIC ANATOMYRadiographs are 2-D representations of 3-D structures. Therefore, overlying structures causingsummation and superimposition frequently create artifacts.In the young patient, the dentin walls are thin and the pulp system is large. The root will not be fullyformed until apexogenesis is complete. As the tooth ages, secondary dentin production continues, theendodontic system becomes smaller, and a root is formed. There is a radiolucent structure around eachtooth (lamina lucida) that represents the space of the periodontal ligament. Immediately adjacent to thelamina lucida is the lamina dura (where the periodontal ligament attaches to the alveolar bone). Thisstructure is a radioopaque structure that loses opacity as the patient ages. The trabecular pattern ofsupporting bone becomes coarser and less distinct with age. The veterinarian should become familiarwith normal structures (e.g., mental foramen, developmental grooves) so as not to mistake them forpathology.Normal anatomical landmarks visualized include the radiolucent mandibular canal, mental foramen(rostral, middle, and caudal), and mandibular symphysis. Particularly, the middle mental foramen can besuperimposed on the apex of the mandibular canine tooth and/or 1 st and 2 nd premolars andkstepani@umn.edu


misinterpreted as pathology. When in doubt, take a second film at a different angle. If the radiolucencystays with the tooth it is likely pathology; if it moves away, it is likely the normal foramen. The correct anatomical names of teeth are (right or left), (maxillary ormandibular), (first, second, third or fourth), (incisor, canine, premolar, molar), as applicable, writtenout in full or abbreviated. The modified Triadan system is presently considered to be the toothnumbering system of choice in veterinary dentistry; gaps are left in the numbering sequence where thereare missing teeth (for example, the first premolar encountered in the feline left maxilla is numbered 206,not 205. The two lower right premolars are 407 and 408, not 405 and 406).Both the use of anatomical names and the modified Triadan system are acceptable for recording andstoring veterinary dental information. The use of anatomical names in publications is required by manyleading journals and is recommended. It offers the advantage of veterinary dental publications beingunderstandable to other health professionals and scientists with an interest in veterinary dentistry.In the cat, the tooth immediately distal to the maxillary canine is the second premolar, the toothimmediately distal to the mandibular canine is the third premolar.kstepani@umn.edu


SURFACES OF TEETH AND DIRECTIONS IN THE MOUTHVestibular is the correct term referring to the surface of the tooth facing the vestibule or lips; buccal andlabial are acceptable alternatives.Lingual: The surface of a mandibular or maxillary tooth facing the tongue is the lingual surface. Palatalcan also be used when referring to the lingual surface of maxillary teeth.Mesial and distal are terms applicable to tooth surfaces. The mesial surface of the first incisor is next tothe median plane; on other teeth it is directed toward the first incisor. The distal surface is opposite fromthe mesial surface.Rostral and caudal are the positional and directional anatomical terms applicable to the head in asagittal plane in non-human vertebrates. Rostral refers to a structure closer to, or a direction toward themost forward structure of the head. Caudal refers to a structure closer to, or a direction toward the tail.kstepani@umn.edu
