D. Service Guidelines: OBSTETRICS SERVICE<strong>Resident</strong>s are responsible for:• Managing prenatal care at the Mercy Maternity based on the schedule definedabove.• Managing labor and delivery and postpartum care of patients from the MercyMaternity Clinic or unassigned patients.• Notifying the attending preceptor of patients being admitted in labor and alldischarges. <strong>Resident</strong>s shall also contact the attending preceptor aboutmanagement plans including when to call the attending with updates on laborand for the delivery. <strong>Resident</strong>s must present cases in a standard format andinterns should discuss with upper level residents before calling the attendingafter hours and on weekends:o Gravida _ Para _ AB _o Ageo EDCo Presenting condition• Onset of labor• Contraction pattern and intensity• Membranes• Dilatation• Station• Vitals• Strips or other tests• Complications if anyo Prenatal courseo Plan• All deliveries will be done with preceptor in attendance. It is the resident’sresponsibility to communicate appropriately with attendings on the progressof labor and the expected time (as much as this is possible!) of delivery.• The delivery note on the mother's chart should comply with Mercy MedicalCenter operative note guidelines.• The OB residents are expected to pre-round and be prepared for dailyinpatient obstetrics rounds with the attending of the day by having seen eachpatient and formulated a management plan prior to the attending's arrival at 8am.• In the Maternity Clinic, residents will provide patient care along with thenurse practitioners under the supervision of the OB attending.• When the need to perform an emergency C-Section arises, the followingprocedure should be pursued:o Call the OB attending regarding the case.o If the OB attending agrees C-section is needed, notify the OB nursingstaff and make sure they contact the anesthesiologist.o Dictate an H&P for the patient before the C-section unless it is a"crash" C-section.Page 61 of 153C:\Documents and Settings\dhutak\Desktop\rshb13.doc
