These visits will assist the resident in betterunderstanding the obstacles to, as well as resources for, improving their patients’ overall healthand well-being.Description:Each resident must perform at least two home visits during his/her residency training, one ofwhich must be an older adult continuity patient. It is recommended that the second be a 1-6week post-partum mother-baby home visit for a Mercy Family <strong>Health</strong> Center (MFHC) continuityobstetric patient.An additional way that residents can fulfill the Adult Home Visit requirement is by seeinghis/her own continuity panel patient during a C1 Mission Provider Home Visit and logging it as aHome Visit.Duties:Each resident will identify patients from his /her continuity clinic panel, who would beappropriate for, and accepting of, a home visit.A faculty member must supervise all home care either on site or by prompt chart review, as isappropriate based on a resident’s level of expertise and competence. If a faculty member isunable to accompany the resident to the patient’s home, it is recommended that he/she takeanother resident. Home Visit Packets with the appropriate forms documenting pertinentsocial/clinical information and education can be found in the MFHC Preceptor Room file. It ispreferable that these visits be recorded on the forms provided, opposed to dictating, as this willensure prompt chart reviews and verification/tracking of these educational experiences.Completed home visit forms will be filed into the patients MFHC clinic record. Each home visitshould also be entered by the resident into the New Innovations on-line tracking program.Home Visit Instructions1) Identify an appropriate patient for home visit and contact the patient to discuss the homevisit, schedule a time, and get directions. Coordinate with a faculty member’s schedule ifhe/she is to attend the home visit with you.2) Obtain the Home Visit Packet from MFHC Preceptor Room file prior to yourscheduled visit.a.
