All new warfarin patients will be given an Rx for Vitamin K 5 mg to be filled only as advised bya MFHC physician. The following guidelines may be used for the management of significantlyelevated warfarin:1. For PT/INR 5.0 to 8.9 and no significant bleeding: Omit 1 to 2 doses of warfarin; reducedose 10 to 20 percent; monitor frequently. Alternately consider Vitamin K: 2.5 mgorally.2. For PT/INR >/= 9.0 and no significant bleeding: Hold warfarin therapy; give Vitamin K:5 mg to 10 mg orally; monitor frequently. Resume at lower dose when INR istherapeutic.3.
