How to Obtain Health and Social Care Services in Your AreaIntroductionThis leaflet will help to explain about health and socialcare services in your area. It will also give practicaladvice about some of the things that you need to do,and on the help that you and your family can receive. Itwill tell you about the services that you and your familycan get from:• family doctors (general practitioner – GP)• hospital services• district nurses• community midwives• health visitors• school health nurses• family planning services• dentists• mental health services• disability services• social work services• pharmacists (chemists)• opticians• community development workers• health promotion• other health-related services you can get throughyour GPPage 2
How to Obtain Health and Social Care Services in Your AreaIf you need an interpreter during your appointmentplease tell a member of staff as soon as you can,before you go. In an emergency please tell staffimmediately that you want an interpreter.Family doctor (general practitioner – GP)To get services from a GP Practice you first need toregister with them. To do this you must have a medicalcard, which you do not have to pay for. You can get themedical card registration form (HS22X) from a healthcentre or GP’s surgery. Fill in the form and take it withyou when you go to register with the GP Practice. Ifyou need help to fill in the form or need a list of localGPs, you can contact the Southern Health and SocialServices Board, the Southern Health and SocialServices Council or any Citizens Advice Bureau. If youhave difficulty in registering with a GP Practice pleasecontact the Central Services Agency and they willallocate one for you (see ‘Useful contact numbers’).When you have filled the form in take it to a GP Practicewho can accept you as a patient. They will sign theform that allows you to get your medical card. If youwant, you can choose to have a male or female GP in aPractice.Page 3
