E. BENEFITS1) AGMA Medical Coveragea) Medical CoverageBSJ will provide to all ARTISTs who are engaged for fifteen (15) weeks or more, HealthInsurance that will not fall below the level <strong>of</strong> coverage and range <strong>of</strong> providers <strong>of</strong> theHealth Insurance BSJ ISTS. BSJ will demonstrate that the current Health Insuranceprovided to ARTISTs is equivalent or superior to the AGMA Health Plan A. Suchcoverage will provide basic health benefits and major medical benefits both inside andoutside the city <strong>of</strong> origination. As <strong>of</strong> the effective date <strong>of</strong> this Basic Agreement BSJ shalldemonstrate to AGMA that the Medical Coverage available to ARTIST meets therequirement <strong>of</strong> this Section. At least sixty (60) days prior to any change in levels <strong>of</strong>coverage or medical service providers, BSJ shall meet and confer with AGMA so as todemonstrate compliance with this Section prior to any final decision taking place.b) Dental CoverageBSJ will provide to all ARTISTs who are engaged for fifteen (15) weeks or more, DentalInsurance for no less than twelve (12) months from first day <strong>of</strong> coverage per contractyear. Such insurance shall begin on the first day <strong>of</strong> the month after the first day <strong>of</strong> eachARTIST’s contract.c) Vision CoverageBSJ will make available to all ARTISTs who are engaged for fifteen (15) weeks or more,Vision Insurance for ARTISTs entire contracted period from the first day <strong>of</strong> employmentper contract year. BSJ shall make such contributions on behalf <strong>of</strong> each ARTIST whochooses to participate in such Vision plan and which shall be deducted from eachARTIST’s paycheck on a pre-taxed, pro-rated basis, upon written authorization fromeach ARTIST. Such insurance shall begin on the first day <strong>of</strong> the month after the first day<strong>of</strong> each ARTIST’s contract.d) BSJ will provide to all ARTIST who are engaged for fifteen (15) weeks or more, HealthInsurance for no less than twelve (12) months* from first day <strong>of</strong> coverage per contractyear, that will not fall below the level <strong>of</strong> coverage and range <strong>of</strong> providers <strong>of</strong> the HealthInsurance BSJ currently provides ARTIST. Such insurance shall begin on the first day<strong>of</strong> the month after the first day <strong>of</strong> each ARTIST’s contract.*For the 2010-11 contract year only, see Addendum 3.e) Family CoverageBSJ agrees to deduct the difference between the cost <strong>of</strong> Individual Coverage and the cost<strong>of</strong> Family Coverage from the compensation <strong>of</strong> any ARTIST described in Section IV A 1,above, who chooses to elect such additional dependent coverage. BSJ agrees to remit thispayment to the applicable insurance provider along with the required BSJ contributionson a monthly basis.f) Plan BBSJ shall contribute seventy dollars ($70.00) for the 2010-11contract year per month foreach month ARTIST is engaged per dancer to the AGMA Health Plan B.26
