Any other children <str<strong>on</strong>g>for</str<strong>on</strong>g> whom aSupplementary In<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> Form hasbeen completed.11. Any other children.Priority within the Oversubscripti<strong>on</strong>Criteria:The governors will apply the following orderof priorities within any of the above criteriawhen applicati<strong>on</strong>s exceed the number ofplaces available <str<strong>on</strong>g>and</str<strong>on</strong>g> it is necessary <strong>to</strong> decidebetween applicati<strong>on</strong>s through a priorityranking:i. Social or medical need which make theschool particularly suitable <str<strong>on</strong>g>for</str<strong>on</strong>g> thechild in questi<strong>on</strong>. Str<strong>on</strong>g <str<strong>on</strong>g>and</str<strong>on</strong>g> relevantevidence must be provided by anappropriate professi<strong>on</strong>al authority (e.g.qualified medical practiti<strong>on</strong>er,educati<strong>on</strong> welfare officer, social workeror priest) (see note c)).ii. For Oversubscripti<strong>on</strong> Criteria 2 or 3above - The strength of evidence ofpractice of the faith as dem<strong>on</strong>stratedby the level of the child’s Massattendance <strong>on</strong> Sundays (see notes f)<str<strong>on</strong>g>and</str<strong>on</strong>g> g)). This evidence must beprovided by the parent/carer <str<strong>on</strong>g>and</str<strong>on</strong>g> beendorsed by a priest at the church(es)where the child normally worships.Applicati<strong>on</strong>s will be ranked in the ordershown <strong>on</strong> the SupplementaryIn<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> Form i.e. firstly thoseattending Mass weekly or at least 3times a m<strong>on</strong>th, then <strong>on</strong>ce or twice am<strong>on</strong>th, then less than <strong>on</strong>ce a m<strong>on</strong>th,then those who do not attend.iii. For Oversubscripti<strong>on</strong> Criteria 5, 6 or 7above - the strength of evidence ofpractice of the faith as dem<strong>on</strong>stratedby the level of the child's attendanceat services. This evidence must beprovided by the parent/carer <str<strong>on</strong>g>and</str<strong>on</strong>g> beendorsed by the minister/faith leaderat the child’s designated place ofworship. Applicati<strong>on</strong>s will be ranked inthe order shown <strong>on</strong> theSupplementary In<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> Form i.e.firstly those who attend servicesweekly or at least three times a m<strong>on</strong>th,then <strong>on</strong>ce or twice a m<strong>on</strong>th, then lessthan <strong>on</strong>ce a m<strong>on</strong>th, then those who d<strong>on</strong>ot attend.iv.Distance from home (the address atwhich the child resides) <strong>to</strong> the school(closest proximity receives highestpriority) (see note e)).
