DOCUMENTS FOR BIDDING AND ELIGIBILITY CRITERIA6.1. İsteklilerin teklif verebilmek için aşağıda sayılan belgeleri teklifleri kapsamında sunmaları gerekir:For bidding, the Bidders shall present the following documents:6.1.1. Tebligat için adres beyanı ve ayrıca irtibat için telefon ve varsa belgegeçer numarası ile elektronikposta adresi. Türkiye’de temsilciliği bulunmayan istekliler de Türkiye’de tebligat yapılacak adresibelirteceklerdir (mümkünse).For the purposes of notification an address statement and for contact a telephone and faxnumber as well as an electronic mail. Foreign bidders, who do not have representatives in Turkey, shallalso report an address in Turkey for notification (if possible).6.1.2. Kayıtlı olduğu Ticaret ve/veya Sanayi Odası veya Meslek Odası belgesi;A registration certificate issued by the Chamber of Commerce and/or /Industry or Professionwhere the Bidder is registered to,6.1.2.1. İsteklinin, gerçek kişi olması halinde, ilk duyuru veya son teklif verme tarihinin içerisindebulunduğu yılda alınmış, Apostille kaşeli veya ilgili ülkenin Türk Konsolosluğunca veya Türk DışişleriBakanlığınca onaylanmış, Ticaret ve/veya Sanayi Odasından ya da Meslek Odasından, odaya kayıtlıolduğunu gösterir belge verilecektir. In case the Bidder is a real personality, a certificate with Apostillecachet or confirmed by Consulate of the Republic of Turkey in the related country or Turkish Ministry ofForeign Affairs provided from the competent Chamber of Commerce/Industry or Vocation in the yeareither the tender or the first announcement is made and proving that he is registered thereto,6.1.2.2. İsteklinin, tüzel kişi olması halinde, tüzel kişiliğin siciline kayıtlı bulunduğu Ticaret ve/veya SanayiOdasından veya ilgili ticaret sicil memurluğundan veya istekli ülkesindeki yer mahkemesinden veyabenzeri bir makamdan, ilk duyuru veya son teklif verme tarihinin içerisinde bulunduğu yılda alınmış,Apostille kaşeli veya ilgili ülkenin Türk Konsolosluğunca veya Türk Dışişleri Bakanlığınca onaylanmış,tüzel kişiliğin sicile kayıtlı olduğuna dair belge verilecektir.In case the Bidder is a artificial person, a certificate with Apostille cachet or confirmed byConsulate of the Republic of Turkey in the related country or Turkish Ministry of Foreign Affairs receivedfrom the Chamber of Commerce and/or Industry that the artificial personality is registered or from thelocal court in tenderer’s country or from a similar authority in the year either the tender or the firstannouncement is made and proving that he is registered thereto,6.1.3. Teklif vermeye yetkili olduğunu gösteren imza beyannamesi veya imza sirküleri,A statement of signature or letter of signature circular indicating that he is authorized to bid,6.1.3.1. İsteklinin, tüzel kişi olması halinde;a. Apostille kaşeli veya ilgili ülkenin Türk Konsolosluğunca veya Türk Dışişleri Bakanlığıncaonaylanmış Tüzel Kişiliğin İmza Sirkülerini ve,b. İlgisine göre, tüzel kişiliğin ortakları, üyeleri veya kurucuları ile tüzel kişiliğin yönetimindekigörevlileri belirten son durumu gösterir Ticaret Sicil Gazetesi,bu bilgilerin tamamının bir Ticaret SicilGazetesinde bulunmaması halinde, bu bilgilerin tümünü göstermek üzere ilgili Ticaret Sicil Gazeteleriveya bu hususları gösteren belgeleri (yabancı isteklinin ülkesi mevzuatı gereği karşılığı belge veyabelgeler) verecektir.In case the Bidder is an artificial personality;a. Authorized Signatory List of the artificial personality with Apostille cachet or confirmed byConsulate of the Republic of Turkey in the related country or Turkish Ministry of Foreign Affairs andb. Trade Register Gazette showing the partners, members or founders and the administrativepersonnel of the artificial personality according to its reference. In case the necessary information doesnot show up in one gazette, tenderer can submit related gazettes of other necessary documents (if thetenderer is foreigner, he shall submit document/s equivalent to this according to his country’s legislation)confirming this issue.6.1.4.
