Features ofjoint care services are:• an out-of-hours home care service, co-located with the Night Nursing Service;• a Rapid Response Team operating 7 days a week <strong>and</strong> providing support for up to 4 weeks;• a joint hospital admissions <strong>and</strong> discharge planning team; <strong>and</strong>• a Rehabilitation <strong>and</strong> Assessment Pilot Scheme (RAPS) at Croft House, a residential home,where a multi-disciplinary team carries out assessments <strong>and</strong> arranges <strong>and</strong> co-ordinatesactivities <strong>and</strong> resources; most older people return to the community after their spell in CroftHouse.Expansions are proposed for sheltered housing, the Crossroads attendant scheme <strong>and</strong> Care<strong>and</strong> Repair.At the centre of Aberdeen, Aberdeenshire, Moray Council <strong>and</strong> the Health Board’s plans toimprove learning disability <strong>and</strong> mental health services is the closure of Woodl<strong>and</strong>s <strong>and</strong>Ladysbridge, but immediate practical problems are transitional services <strong>and</strong> housing. A housingprocurement group is looking at support models. The Change Fund has provided resources toimprove local area co-ordination, develop autism services, introduce direct payments <strong>and</strong>restructure day services.8
