Although Fife provides home care services to more older people than most otherauthorities, it provides fewer hours per week than many others. Some of its services, forexample, shopping delivery <strong>and</strong> meals on wheels are provided without using home carers.The Council has made a number of changes by investing in:• improvements in home care services, in equipment <strong>and</strong> in adaptations;• more nursing home places; <strong>and</strong>• (in collaboration with the health board) integrated response teams, which have begun tohave an impact on the relatively high numbers of older people unable to be dischargedfrom hospital.The Oakley integrated team is a pilot project designed to enhance the care provided by homecarers, to assist with hospital discharges <strong>and</strong> avoid the need for admissions. Trained homecarers provide the service <strong>and</strong> are supported by nursing staff to deal with problems such aseye care, dermatology, sensory loss, continence <strong>and</strong> dementia. Professional nursing staff canconcentrate on higher-level tasks. The project evaluation has confirmed that key objectiveshave been achieved <strong>and</strong> older people now have the kind of service they want. In addition, thework of home carers is now more highly valued <strong>and</strong> they manage personal care tasks withgreater confidence.Learning disability services are underpinned by a Partnership in Practice Agreement (PiP)between the Council <strong>and</strong> the NHS <strong>and</strong> by joint decision-making on the allocation of ChangeFund resources. Responsibility for implementing the review of services for people with learningdisabilities, ‘The same as you?’ rests with senior managers from social work <strong>and</strong> from FifeNHS. People with learning disabilities <strong>and</strong> their families are well engaged in planning <strong>and</strong>implementation, through stakeholder conferences, membership of all 11 implementationgroups, <strong>and</strong> employment in the implementation project (SAY project). Fifteen people withlearning disabilities receive direct payments to pay for their own services.128
