11/22/2011Laboratory evaluation of liver disease• Hepatocyte integrity: cytosolic hepatocellular enzymes; AST, ALT,LDH• Hepatocyte functions:– Proteins secrete into blood: Serum albumin, Prothrombin time– Hepatocyte metabolisms: Serum ammonia etc.• Biliary excretory functions:– Substance secrete in bile: Serum bilirubin – Conjugated andUnconjugated, Urine bilirubin, Serum bile acids– Plasma membrane enzymes of bile duct cell: Serum alkalinephosphatase• ภาวะตับแข็ง/Cirrhosis – เปนสาเหตุตนๆ (Top 10) ที่เปนสาเหตุใหเสียชีวิตในประเทศตะวันตก• สาเหตุที่พบบอยที่ทําใหเกิดภาวะตับแข็ง คือ ดื่มแอลกอฮอล และ ตับอักเสบจากเชื้อไวรัส สวนสาหตุอื่นๆเชน โรคของทอน้ําดี และภาวะเหล็กเกินCirrhosisตับแข็ง/Cirrhosis เปนจุดสิ้นสุดของโรคตับเรื้อรังCirrhosisลักษณะทางพยาธิวิทยาที่พบ– Bridging fibrous septae มี fibrous bands หรือ broad scarsเชื่อมระหวาง portal tracts และ portal tracts กับ terminalhepatic veins– Parenchymal nodules เปน proliferating hepatocytes ที่ถูกลอมรอบดวย fibrous bands ถาขนาด 3 mm จะเรียกวา macronodules– โครงสรางเดิมของตับเสียไป ขนาดจะเล็กลง23
