Instructions to <strong>Bid</strong>derssecure Supplemental/<strong>Bid</strong> Bulletins that may be issued by the BAC. However,<strong>Bid</strong>ders who have submitted bids before the issuance <strong>of</strong> the Supplemental/<strong>Bid</strong>Bulletin must be informed and allowed to modify or withdraw their bids inaccordance with ITB Clause 2<strong>3.</strong>1<strong>1.</strong> Language <strong>of</strong> <strong>Bid</strong>C. Preparation <strong>of</strong> <strong>Bid</strong>sThe bid, as well as all correspondence and documents relating to the bid exchangedby the <strong>Bid</strong>der and the Procuring Entity, shall be written in English. Supportingdocuments and printed literature furnished by the <strong>Bid</strong>der may be in another languageprovided they are accompanied by an accurate translation in English certified by theappropriate embassy or consulate in the Philippines, in which case the Englishtranslation shall govern for purposes <strong>of</strong> interpretation <strong>of</strong> the bid.1<strong>2.</strong> Documents Comprising the <strong>Bid</strong>: Eligibility and TechnicalComponents1<strong>2.</strong><strong>1.</strong> Unless otherwise indicated in the BDS, the first envelope shall contain thefollowing eligibility and technical documents:(a) Eligibility Documents –Class “A” Documents:(i)(ii)(iii)(iv)(v)Registration certificate from the Securities and ExchangeCommission (SEC), Department <strong>of</strong> Trade and Industry (DTI)for sole proprietorship, or Cooperative Development Authority(CDA) for cooperatives, or any pro<strong>of</strong> <strong>of</strong> such registration asstated in the BDS;Mayor’s permit issued by the city or municipality where theprincipal place <strong>of</strong> business <strong>of</strong> the prospective bidder is located;Statement <strong>of</strong> all its ongoing and completed government andprivate contracts within the period stated in the BDS, includingcontracts awarded but not yet started, if any. The statementshall include, for each contract, the following:(iii.1) name <strong>of</strong> the contract;(iii.2) date <strong>of</strong> the contract;(iii.3) kinds <strong>of</strong> Goods;(iii.4) amount <strong>of</strong> contract and value <strong>of</strong> outstanding contracts;(iii.5) date <strong>of</strong> delivery; and(iii.6) end user’s acceptance or <strong>of</strong>ficial receipt(s) issued for thecontract, if completed.Audited financial statements, stamped “received” by theBureau <strong>of</strong> Internal Revenue (BIR) or its duly accredited andauthorized institutions, for the preceding calendar year, whichshould not be earlier than two (2) years from bid submission;NFCC computation or CLC in accordance with ITB Clause5.5; andPage 7 <strong>of</strong> 40
