Ms.No 145 Dt; 25-04-1998-*-*-*-*-The attention of the all Jo<strong>in</strong>t Directors, Deputy Directors and Asst.Directors are drawn to the subject cited. The follow<strong>in</strong>g observations are noticed <strong>in</strong>respect of ACR’s received by this office.1) Self <strong>ap</strong>praisal reports <strong>in</strong> the format enclosed are not submitted by most ofthe Officers/Super<strong>in</strong>tendents to the report<strong>in</strong>g officer.2) Self <strong>ap</strong>praisal reports are not submitted every year, the self <strong>ap</strong>praisalreports are to be submitted to the report<strong>in</strong>g officer by 10 th April of everyyear.3) Report<strong>in</strong>g Officers are not prepar<strong>in</strong>g and forward<strong>in</strong>g CR’s <strong>in</strong> the format by15 th April. The Report<strong>in</strong>g Officers should take <strong>in</strong>itiation to obta<strong>in</strong> self<strong>ap</strong>praisal reports from all concerned and prepare the CR’s and forward itto the Counter sign<strong>in</strong>g authority / next higher authority by 15 th April.4) Counter sigh<strong>in</strong>g authority are also not submitt<strong>in</strong>g C.R’s <strong>in</strong> time. They areexpected to pass on the ACR’s wiith their remarks to the Head of thedepartment with<strong>in</strong> a week of its receipt.5) It is noticed that, most of the <strong>ACRs</strong> received are not <strong>in</strong> proper format.Copies of the G. Os ref. 2 nd & 3 rd cited along with format enclosed. Inrespect of Gazetted officers, the report should be written <strong>in</strong> Form-A(enclosed)6) The report<strong>in</strong>g / counter sigh<strong>in</strong>g officers are not writ<strong>in</strong>g their names <strong>in</strong> BlockLetters and also the date on which <strong>ACRs</strong> is prepared / counter-signed isnot <strong>in</strong>dicated, thus identification of Officer reported / countersigned hasbecome difficult. Therefore they should <strong>in</strong>variably write their name <strong>in</strong> blockLetters and <strong>in</strong>dicate the date.7) It is observed that, some of the reports were prepared / counter-signed bythe retired officer which is not admissible. (Memo.No.631/Ser.C/76-1,dt.11th Mary, 1976).8) The report<strong>in</strong>g / countersign<strong>in</strong>g officer are not submitt<strong>in</strong>g <strong>ACRs</strong> <strong>in</strong> a sealedcover. They are requested to send <strong>ACRs</strong> by name <strong>in</strong> a sealed cover.P.T.O.


2//9) The guidel<strong>in</strong>es issued by the Government <strong>in</strong> respect of writ<strong>in</strong>g the <strong>ACRs</strong>are enclosed for <strong>in</strong>formation. All are requested to adhere to the guidel<strong>in</strong>esand prepare / counter signed the <strong>ACRs</strong> <strong>in</strong> time. The follow<strong>in</strong>g are thesome of the important guidel<strong>in</strong>es for prepar<strong>in</strong>g <strong>ACRs</strong>.i) The report should be written by the officer him self. To facilitateidentification, the name of the officer writ<strong>in</strong>g the report should bewritten <strong>in</strong> block letters below his signature and clearly <strong>in</strong>dicate thedesignation.ii) In the case of gazetted officers, the reports will be written <strong>in</strong>duplicate.iii) Name & designation of officers must be given <strong>in</strong> full.iv) Confidential report should be prepared and forwarded by firstreport<strong>in</strong>g officer by 15 th April and the immediate authorities will passon the report with their remarks to the next higher authorities with <strong>in</strong>one week of its receipt. They should reach the Head of thedepartment not later than the 15 th May.v) The report<strong>in</strong>g officer should not write CR on the officer who haveworked under him for a period of less than two months.vi) If the report<strong>in</strong>g officer is transferred after July he should leave areport on each of the officer under him to enable his successor totranscribe these remarks <strong>in</strong> to the CR stat<strong>in</strong>g the name of theauthor.vii) If any adverse remarks should be supported by specific evidentiaryexamples or <strong>in</strong>stances as for as possible.ToAll the JDS/ DDS,/ ADS <strong>in</strong> the state.Sd/B.CHANDRASEKARFor Commissioner of <strong>Seri</strong>culture
