Before AdmissionToursTours of the facility are provided Monday through Friday. The best times to tour are from 9:30 a.m.-11:00 a.m. and 1:00 p.m.-3:00 p.m., as they allow you to see patients participating in therapy. Sothat we can have tour guides available, please call (214) 820-9330 to schedule the day and time thatwould be most convenient for you. If special arrangements need to be made for evening or weekendtours, please let us know.What to Bring With YouClothingWe want you to be as active as possible during your stay, so we recommend that you wear nonbindingclothes during daytime hours. Shorts, sweat pants, and t-shirts with tennis shoes areespecially comfortable. Dresses are not appropriate for therapy activities.We recommend that you bring at least six changes of clothing and an adequate supply of socksand undergarments. Pajamas, gowns, bathrobe and slippers are suggested for the evening, andswim suits are needed for aquatic activities.Please use an indelible ink pen to mark all clothing with your complete name for easy identification.Personal ItemsWe recommend that you bring the following:• Brush & comb• Toothbrush & toothpaste• Shampoo• Deodorant• Shaving supplies (electric razor if you are on blood thinners)• Non-skid shoes (e.g. tennis shoes)• Eye glasses/contactsAdditional items to include may be:• Hair dryer• Hair curlers• Radio/tape player (All electrical appliances must be checked by our engineering department before useand should be labeled with your name.)We ask that you leave all non-essential items at home.Room AssignmentYour room will be assigned according to your medical need and bed availability at the time ofadmission. Most patients will be admitted to a semi-private room.Please feel free to bring personal items to decorate your room, including plants, flowers, etc.There is a bulletin board in each room for you to display cards, posters, and photographs.Family and Guest HousingFor out-of-town family members, we have limited access to a variety of nearby, lower-cost housingfacilities. These include hotels and motels that can be reserved for nightly, weekly or monthly stays.Please call 214-820-9330 for a list or ask your social worker.


About Our HospitalParkingParking lot #28 adjacent to the hospital on Gaston Avenue is designated for the family members ofinpatients and for outpatients and participants in our community exercise and aquatic classes. Thecharge for this lot is $2.00. So that we can have adequate parking space for these persons, please askfriends and other visitors to park in lot #27 or #9, on the corner of Gaston and Hall. Parking forthese lots is anywhere from $2.00 to $4.00 depending on the length of time in the lot.Parking is not allowed in the front and side driveways that are designated for patient transportationdrop-off and pick-up. Please ask your family members and visitors to honor this request.Hospital Hours and AccessThe lobby entrance on Gaston Avenue and the Floyd Street entrance, near the Aquatics Center willbe open from 6:00 a.m. to 6:00 p.m. The west entrance will be open from 6:00 a.m. to 9:00 p.m.Persons can exit all doors at any hour, but will not be able to re-enter the hospital after locking until6:00 a.m.Telephone CallsLocal telephone calls may be made from patient rooms by dialing nine (9), then the number desired.Metro numbers beginning with 817 and long distance calls may be made by dialing nine, zero, thearea code and the number desired. These calls must be charged to a calling or credit card.Family members and friends may call directly to the room at any time. However, we recommendthat calls not be placed to the room after 9:00 p.m. as a courtesy to patients who may have alreadyretired for the evening.SmokingSmoking is not permitted in the hospital by employees, patients, family members or visitors.<strong>Baylor</strong> facilities and all outdoor areas of the <strong>Baylor</strong> campus grounds (including parking areas andsidewalks) are designated smokefree. Smoking is not permitted anywhere on our campus. Thispolicy is for the protection of all who come to <strong>Baylor</strong> for their healthcare, to visit or to work.LaundryAs a convenience to our patients and their family members, washers, and dryers are located in thebasement. There is no charge to use the equipment, however, washing detergents are not provided.Patients who are not able to do laundry and who have no family members available can make specialarrangements through the nursing department.PetsThere is an Animal Assisted Therapy Program that is part of our inpatient treatment program.These animals have been through extensive obedience and temperament testing before beingallowed into the hospital. We regret that personal pets are not allowed inside the hospital.If you would like to visit with your pet, please do so outside the facility.


What to Expect on Your First DayAdmissionAfter arriving at <strong>Baylor</strong> Institute for Rehabilitation, you and your family will be greeted in your roomby the Patient Admissions Coordinator. The Coordinator will provide bedside admission and orientyou and your family members to your room and the hospital. After this process is complete, yournurse will visit with you and do a medical assessment.On the day of admission, we ask that patients be accompanied by a family member. This is especiallyimportant if the patient is not able to complete the admission forms, verify information and sign the“Consent to Treat” form. If no family member is available at the time of admission, a family membermust complete the admission documents within 24 hours. This can be done by checking in withthe Admissions Desk on the first floor of the hospital, between 8:00 a.m. and 5:00 p.m., or with theNursing Supervisor, after 5:00 p.m. weekdays and on weekends.We also ask that patients bring their insurance card, prescription drug card, Medicare card, driver’slicense and social security card at the time of admission. Although financial information has beenverified prior to admission, we will use this information to arrange for insurance assignment to thehospital, permitting direct billing to your insurance company.Treatment ProgramA rehabilitation hospital is very different from an acute care hospital. The types of activities that youwill be participating in and the assistance that you receive will be different. We have developed thisportion of the brochure to help explain these differences and why rehabilitation services are necessaryto help your continued progress.What is the purpose of a rehabilitation hospital and why have you been referred to one? Following yourinjury, illness or surgery, you may have experienced loss of strength, endurance and/or the abilityto perform activities in the way you once did. Your deficits may be in the areas of speech, thinking/cognitive abilities, and movement/mobility. You have been referred to our facility because it offersa treatment program to help you regain independence and functioning. This is the goal of all ourpatients.


Your Treatment TeamOur interdisciplinary clinical team, led by physiatrists and including licensed physical therapists,licensed occupational therapists, speech-language pathologists, certified recreation therapists,neuropsychologists, certified rehabilitation registered nurses, social workers and other specializedclinicians, work with each patient to design a custom-tailored program of rehabilitation care.PhysiatristThis is a physician who specializes in physical medicine and rehabilitation. He/she will be yourattending physician, meaning the physician who is primarily responsible for your care. Thephysiatrist may also call in consulting physicians to address specific medical problems that may arise.Clinical Nurse CoordinatorThe clinical nurse coordinator works with all the people involved in planning and implementingyour treatment program. The coordinator will help with issues related to financial resources and isthe primary contact and source of information about your progress in the treatment program.Social WorkerThe social worker problem solves issues related to financial resources and insurance benefits asit relates to discharge options and services. She provides you and your family with dischargeinformation and makes referrals for continued services for you upon discharge from the hospital.Rehabilitation NurseThe responsibilities and involvement of the rehabilitation nurses vary, depending on the level ofcare required by each patient. The nurse will monitor your physical condition, nutrition, boweland bladder function, response to medications and vital signs, similar to the nurse’s role in an acutecare hospital. However, nurses in our rehabilitation hospital will also help ensure that therapyrecommendations are carried out and provide training to you and your family members regardinghow to manage medical issues after discharge.Patient <strong>Care</strong> Technician (PCT)Like the nurse, the patient care technician provides varying degrees of support based on your level ofindependence. The PCT’s primary role is to assist you with self-care activities in your hospital room. As yourprogress in the treatment program and become capable of performing these activities more independently,the PCT provides less hands-on care.Occupational Therapist (OT)The OT works with you in the areas of self-care. This includes dressing, grooming, bathing, eating,and advanced self-care activities such as home management, cooking, cleaning, money management,and the use of upper extremities (arms). The OT may also work on assessing and treating visiondeficits and prescribing assistive technology.Physical Therapist (PT)The PT works with you in the use of your lower extremities (legs), in the areas of mobility, includingbed, or walking, with or without the use of assistive equipment, and/or using a wheelchair. Thephysical therapist will also make recommendations on your equipment needs in preparation fordischarge.


Rehabilitation TechnicianThe rehabilitation technician is specially trained and carries out therapeutic activities prescribed byyour license therapist to address your rehabilitation goals.. The rehabilitation technician is speciallytrained and supervised by a licensed therapist to supervise activities that address your physicalrehabilitation goals.Speech-Language PathologistThe speech-language pathologist will help you improve communication skills, as well as cognition,in the areas of listening, speaking, reading and writing. He/she may also work on assessing andtreating swallowing disorders.NeuropsychologistNeuropsychology provides assessment and treatment in cognition, which includes thinking, newlearning, memory, emotional well-being (coping skills, mood), and behavior. Services are providedin individual, family and group therapy settings. Adjustment counseling and sexuality, includingrelationships, self-esteem, body image and fertility are also addressed.Therapeutic Recreation Specialist (TR)The TR works with you on the physical, cognitive, social and emotional issues which may interferewith leisure and community activities. The TR may adapt leisure activities so you can continueto participate in pastimes you enjoy. The TR can also help increase your awareness of leisure andcommunity resources, which may include taking you on a community outing prior to discharge.Respiratory TherapistA respiratory therapist provides treatment and education to patients who are on a ventilator, havea tracheotomy or have any pulmonary dysfunction.Your Daily Therapy SchedulePatients will receive approximately three hours of therapy a day Monday thru Friday. Within a dayor two of admission, you will begin a routine of therapy activities based on your individual needs.You will receive a printed therapy schedule daily to help you know what times you need to beprepared to participate in therapy.While most of the time your schedule will remain the same, sometimes it is necessary for changesto be made to it. This will occur if you progress into new treatment activities, diagnostic tests ormedical procedures are scheduled, or one of your treatment team members is absent. A printed copywill be delivered to your room the evening before by nursing staff. If schedule changes occur duringthe day, a new schedule will be printed and put in place of the previous schedule. A copy of thetherapy schedule is also available at the nurse’s station on each floor.TherapiesThe therapy gym and therapy offices are located on the second floor. Aquatic therapy is provided inthe Aquatics Center on the first floor. Therapy is provided on both a one-on-one basis and in a groupformat.Hospital transporters are provided to take you to each therapy session. At the end of your therapies, youwill be asked to wait near the second floor elevator for the transporter to take you back to your roomwhere they can assist you back to bed as needed. As you progress, you will be asked to become more


independent in your mobility. For example, you may be asked to propel your own wheelchair insteadof having the transporter push it for you. Family members, who have been trained in transfers and whoare able to help you get in and out of bed, may also transport you to and from therapies.Hospital PassesDuring your rehabilitation program the treatment team may recommend that you go out of thehospital on a pass. In addition to providing a break from being in the hospital, the pass will help youprepare for going back into the community and the activities that are important to you. Initially, youmay be scheduled for an outing with the therapeutic recreation specialist. This will allow you to be inthe community with the support of the <strong>Baylor</strong> Rehab staff.You may also be provided with a pass to leave the hospital with family members. Before this type ofpass is granted, your family members must complete training with the PT on how to safely assist youand manage any equipment while out in the community. One training is completed, your physicianwill issue the order for a community pass.Patient <strong>Care</strong> ConferencesThroughout your stay, patient care conferences will be held. During this conference your treatmentteam will meet to discuss your treatment plan, therapy goals, progress and anticipated discharge date.Your and your family members will have the opportunity to attend some of these conferences andwill be notified of the day and time the conference will be held. Since space and time are limited,we ask that you be on time, suggest that you prepare your questions in advance, and that you limitthe number of family members to two. This will help us accommodate all patients and familiesscheduled during the allotted time period.Family Observation and ParticipationIt is our philosophy that support and involvement from family and friends enhance the rehabilitationprocess. Therefore, your family members and friends are invited to observe and participate indaytime therapies, evening activities and training sessions with therapists. Visitors are also welcomein your hospital room until 9:00 p.m.Observation Area, Room 307For the welfare of patients, we limit the number of visitors in room 307 to two per patient, andchildren under age 12 are allowed only with special permission. Visitors are not allowed to enterroom 307 when the door is closed for a procedure. Additionally, guests are not allowed to stayovernight.Overnight GuestsIn our private rooms, one adult over the age of 16 may stay overnight. We are not able toaccommodate overnight adult guests in semi-private rooms except under the followingcircumstances:• The patient is critically ill• The patient is extremely agitated• The patient is under age 16These exceptions are also contingent upon space requirements to perform patient care activities androommate consideration. Overnight stay in a semi-private room must have staff approval and theroommate must be in agreement.


Discharge PlanningA discharge date is set for each patient at the first patient care conference and reassessed through thehospital stay. Your social worker will inform you and your family members well in advance of thedate so that adequate preparation can be made.Your treatment team will make recommendations regarding plans for continued therapy or othercare options following discharge. Recommendations will also be made for any home modifications,equipment, community resources and other services.Additionally, you may be scheduled for a follow-up visit with your physician in the OutpatientServices Clinic one month after discharge. At this time, your medical, physical and functional statuswill be evaluated and recommendations will be made for modifications in equipment, medicationand/or home program.Discharge DayYour discharge plan, including the day and time, will be clearly communicated to you. We ask thatyou make advance arrangements with your family or caregiver to leave your room at the time specifiedby your treatment team so that we can make it ready for the admission of another patient.
