78 Marine Accident ReportRecommendati<strong>on</strong>sAs a result of its investigati<strong>on</strong> of this accident, <strong>the</strong> Nati<strong>on</strong>al Transportati<strong>on</strong> Safety<strong>Board</strong> makes <strong>the</strong> following recommendati<strong>on</strong>s:New Recommendati<strong>on</strong>sTo <strong>the</strong> U.S. Coast Guard:During c<strong>on</strong>trol verificati<strong>on</strong> examinati<strong>on</strong>s, review a drill scenario in which<strong>on</strong>e or more main vertical z<strong>on</strong>es are inaccessible and evaluate <strong>the</strong>procedural effectiveness of <strong>the</strong> crew in crowd c<strong>on</strong>trol, crisis management,lifejacket distributi<strong>on</strong>, and passenger accountability. (M-01-1)To Carnival Corporati<strong>on</strong>:For <strong>the</strong> ships in your fleets, revise <strong>the</strong> safety management system to includeprocesses for preventing unauthorized flame cutting, grinding, or o<strong>the</strong>ractivities that might ignite a fire. (M-01-2)For <strong>the</strong> ships in your fleets, develop plans to account for passengers andcrewmembers in comm<strong>on</strong> emergency scenarios, in particular, a situati<strong>on</strong>involving <strong>the</strong> inaccessibility of <strong>on</strong>e or more main vertical z<strong>on</strong>es and/ormuster stati<strong>on</strong>s. (M-01-3)To Carnival Cruise Lines:For <strong>the</strong> ships in your fleet, engineer, design, and implement systemmodificati<strong>on</strong>s to mitigate <strong>the</strong> spread of fire and smoke from <strong>the</strong> laundryrooms through ventilati<strong>on</strong> ducts to o<strong>the</strong>r areas of <strong>the</strong> vessel. (M-01-4)Examine <strong>the</strong> propulsi<strong>on</strong> systems <strong>on</strong> <strong>the</strong> ships in your fleet and, if necessaryto ensure redundancy, modify <strong>the</strong> arrangement of <strong>the</strong> auxiliary voltagecircuitry to <strong>the</strong> high-speed breakers where a single source supplies bothport and starboard propulsi<strong>on</strong> systems. (M-01-5)Revise <strong>the</strong> safety informati<strong>on</strong> disseminated to passengers to include acti<strong>on</strong>sto take if <strong>the</strong>y encounter smoke or fire and/or if <strong>the</strong>ir muster stati<strong>on</strong> is notavailable.
