dimensional scales: (a) affective subscale-excessive worry about health, (b) cognitive<br />
subscale-alienation from others due to their disbelief that one is ill, (c) behavioural<br />
subscale-dependency on reassurance seek<strong>in</strong>g from others to reduce illness fears, <strong>and</strong> (d)<br />
perceptual subscale-somatic absorption with body sensations. Sample items <strong>in</strong>clude ―I<br />
wish others took my health compla<strong>in</strong>ts more seriously‖ <strong>and</strong> ―I worry a lot about my<br />
health‖ (p. 6). The scale uses a five-po<strong>in</strong>t response format. It has good <strong>in</strong>ternal<br />
consistency (α = .80 - .89 across various samples), good convergent validity (test-retest r<br />
= .75 - .78), <strong>and</strong> good external validity (r = .86).<br />
2.2.4. Secondary outcome measures<br />
The Centre for Epidemiologic Studies Depression Scale (CES-D; Radl<strong>of</strong>f, 1997)<br />
is a 20-item measure <strong>of</strong> depression symptoms <strong>in</strong> community samples (Radl<strong>of</strong>f & Locke,<br />
2000). Sample items <strong>in</strong>clude, ―I feel depressed‖, <strong>and</strong> ―I thought my life had been a<br />
failure‖ (p. 523). The items are rated on a scale <strong>of</strong> 0 - 4 based on symptoms over the past<br />
week. Total severity scores on the CES-D range from 0 - 60 with higher scores <strong>in</strong>dicat<strong>in</strong>g<br />
more severe depression symptoms. Scores <strong>of</strong> 16 or greater may signify the presence <strong>of</strong><br />
depressive illness. Internal consistency is good (� = .85 <strong>in</strong> community samples),<br />
alternate-forms reliability over 2 to 8 weeks is moderate (r = .51 - .67), <strong>and</strong> convergent<br />
validity ranges from moderate to excellent (r = .49 - .89). The computerized version <strong>of</strong><br />
the CES-D has good <strong>in</strong>ternal consistency (� = .93; Smits, Cuijpers, & van Straten, 2011).<br />
