D. Vision Benefit1. Program DescriptionThe employer agrees to provide a vision benefit to eligible employees anddependents. The vision benefit provided by the <strong>State</strong> shall have anemployee co-payment <strong>of</strong> $10 for the comprehensive annual eye examinationand $25 for materials.2. Employee EligibilityEmployee eligibility for vision benefits will be the same as that prescribed forhealth benefits under subsection B.1.3. Family Member EligibilityFamily member eligibility for vision benefits will be the same as thatprescribed for health benefits under subsection B.2.E. Pre-retirement Death Continuation <strong>of</strong> Benefits ProposalThe union agrees to support legislation that would add Government Code Section19849.15:19849.15. Not<strong>with</strong>standing any other provision <strong>of</strong> law, the <strong>State</strong> employer shall,upon the death <strong>of</strong> an employee while in <strong>State</strong> service, continue to pay employercontributions for health, dental and vision benefits for a period not to exceed <strong>12</strong>0days beginning the month <strong>of</strong> the employee’s death. The surviving dependent(s), ifany, shall be advised <strong>of</strong> all rights and obligations during this period regarding thecontinuation <strong>of</strong> health and dental benefits as an annuitant by the <strong>California</strong> PublicEmployees’ Retirement System. The surviving dependent(s) shall also be notifiedby the department during this period regarding COBRA rights for the continuation <strong>of</strong>vision benefits. This section shall apply to represented <strong>State</strong> employees inbargaining units that have agreed to this provision.3.2 Rural Subsidy ProgramA. The <strong>State</strong> shall establish a rural subsidy program for bargaining <strong>Unit</strong> <strong>12</strong> members, whichmay be administered in conjunction <strong>with</strong> a similar program for state employees in otherbargaining units, for excluded employees, and for annuitants. DPA shall administer anyfund involving Bargaining <strong>Unit</strong> <strong>12</strong> members.B. The program shall operate in the following fashion:1. The <strong>State</strong> shall contribute $1,500 per year on behalf <strong>of</strong> each bargaining unitmember (employee) who lives in a defined rural area, as more definitely described inSenate Bill 514 (Senator Chesbro), incorporated herein by reference. Forbargaining <strong>Unit</strong> <strong>12</strong> members, because a substantial number <strong>of</strong> them are seasonalemployees, payments shall be on a monthly basis.For permanent employees, as in the “Medical Reimbursement Account”situation, the employee does not have to wait for reimbursement <strong>of</strong> coveredmedical expenses until the full amount has been deposited.28BU <strong>12</strong>(99-01)
