-2-Remunerati<strong>on</strong>: Rs.20000.00 per m<strong>on</strong>th will be paid as fees <strong>on</strong> c<strong>on</strong>tract, apart from this,incentives at the rate <str<strong>on</strong>g>of</str<strong>on</strong>g> Rs.20.00 for every Artificial Inseminati<strong>on</strong> d<strong>on</strong>e,Rs.25.00 for every Pregnancy cases verified and found to be positive andRs.50.00 for every calf birth verified will be paid, subject to a maximum totalclaim <str<strong>on</strong>g>of</str<strong>on</strong>g> Rs.5000.00 per m<strong>on</strong>th. Rs.50.00 for every emergency case treated will be paid, subject to amaximum claim <str<strong>on</strong>g>of</str<strong>on</strong>g> Rs.500.00 per m<strong>on</strong>th as pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al charges. Medicines to treat both general and emergency cases will be provided by theimplementing Milk Uni<strong>on</strong>s. Other equipment to provide Artificial Inseminati<strong>on</strong> services and treatment willbe provided by the implementing Milk Uni<strong>on</strong>s.AGREEMENT:Selected veterinarian shall enter into an agreement agreeing to all thec<strong>on</strong>diti<strong>on</strong>s menti<strong>on</strong>ed there<str<strong>on</strong>g>of</str<strong>on</strong>g> with the Milk Uni<strong>on</strong>.CONTRACT CONDITIONS:- Each unit will cover around 30 MPCS in a week. (around 10 MPCS per day).Each MPCS to be visited twice a week. The veterinarian should arrange for his own mode <str<strong>on</strong>g>of</str<strong>on</strong>g> transport ( Two wheeler/Four wheeler) to visit the MPCS and attend emergency cases. Should carry the required medicines at the time <str<strong>on</strong>g>of</str<strong>on</strong>g> visit. Rs.4.00 per Km. will be paid as propulsi<strong>on</strong> charges. The total distance covered for regular visit shall be approximately 1000 km.per m<strong>on</strong>th. The total distance covered to attend emergency cases shall be approximately500 km. per m<strong>on</strong>th. Incentives to a maximum <str<strong>on</strong>g>of</str<strong>on</strong>g> Rs.5000.00 per m<strong>on</strong>th and Rs.500.00 per m<strong>on</strong>thas pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al charges will be provided depending <strong>on</strong> the performance. Mobile veterinary route are to be operated as per the time schedulesprescribed by the Milk Uni<strong>on</strong>.


-3-Must operate from the Headquarters assigned by the Respective Milk Uni<strong>on</strong>s.Milk Uni<strong>on</strong> will supply the required veterinary medicines.The doctor should strictly adhere to the time schedule approved by theGeneral Manager <str<strong>on</strong>g>of</str<strong>on</strong>g> the c<strong>on</strong>cerned Milk Uni<strong>on</strong>s.The doctor should arrange for any physical assistance required to carry outthe job assigned without any deficiency at his own cost. No separateattendants will be provided.The engagement <str<strong>on</strong>g>of</str<strong>on</strong>g> the veterinarian <strong>on</strong> c<strong>on</strong>tract is purely temporary.DUTIES AND RESPONSIBILITIES:1. Treating the milch animals owned by the members <str<strong>on</strong>g>of</str<strong>on</strong>g> the MPCS.2. Attending emergency cases <strong>on</strong> call <strong>basis</strong>.3. Inseminate milch animals.4. Visit the allotted MPCS as per the time schedule.5. Coordinate Vaccinati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> all animals covered by the MPCS.6. Treatment <str<strong>on</strong>g>of</str<strong>on</strong>g> infertility cases as special case and follow up by maintaining aregister.7. C<strong>on</strong>duct health camps whenever required.8. Prepare animal census pertaining to each MPCS with the help <str<strong>on</strong>g>of</str<strong>on</strong>g> EOs/ MPCSstaff.9. M<strong>on</strong>itor AI coverage <str<strong>on</strong>g>of</str<strong>on</strong>g> the said MPCS.10. Enrol MPCS employees for AI worker (VLW) training wherever necessary.11. M<strong>on</strong>itor identificati<strong>on</strong>, Artificial Inseminati<strong>on</strong>, pregnancy verificati<strong>on</strong> and calfbirth.12. Maintain work d<strong>on</strong>e registers (list furnished separately).13. Inspect records <str<strong>on</strong>g>of</str<strong>on</strong>g> the MPCS.14. Collecti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> data <str<strong>on</strong>g>of</str<strong>on</strong>g> infertility cases in the MPCS through VLW/ Secretaries/E.Os.15. Maintain cow cards issued to the individual farmer members.16. Submit periodical reports as stipulated by the <str<strong>on</strong>g>of</str<strong>on</strong>g>ficials <str<strong>on</strong>g>of</str<strong>on</strong>g> the Milk Uni<strong>on</strong>s.17. Implement various schemes allotted to the Milk Uni<strong>on</strong>s, pertaining to the area.18. C<strong>on</strong>duct members meeting (Night meetings al<strong>on</strong>g with team <str<strong>on</strong>g>of</str<strong>on</strong>g>ficials andtechnical people from the Uni<strong>on</strong>).19. Submit m<strong>on</strong>thly PDLS/ MIS report.20. Submit m<strong>on</strong>thly Infertility reports and <strong>on</strong> follow up <str<strong>on</strong>g>of</str<strong>on</strong>g> such cases.PERFORMANCE ASSESSMENT:The Performance <str<strong>on</strong>g>of</str<strong>on</strong>g> the doctors will be c<strong>on</strong>tinuously assessed by the DGM(P&I)/AGM (P&I) <str<strong>on</strong>g>of</str<strong>on</strong>g> the Milk Uni<strong>on</strong>s. Criteria for performance assessment:1. Regularity in visiting the MPCS as per schedule.2. Random checking route operati<strong>on</strong>.

Performance in terms <str<strong>on</strong>g>of</str<strong>on</strong>g> cases treated, Artificial Inseminati<strong>on</strong>s d<strong>on</strong>e,Pregnancy cases verified, calf birth verified, vaccinati<strong>on</strong>s d<strong>on</strong>e, etc.4. Submissi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> fortnightly and m<strong>on</strong>thly performance reports.5. Evaluati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the progress in implementati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the schemes in his area.6. Periodical review every m<strong>on</strong>th.Claim and Settlement <str<strong>on</strong>g>of</str<strong>on</strong>g> fees/ Scheme expenses:1. Claim in duplicate shall be made <strong>on</strong>ce in a m<strong>on</strong>th by the doctor.2. Claim should not exceed Rs.20000.00 per m<strong>on</strong>th towards salary, Rs.4000.00per m<strong>on</strong>th towards propulsi<strong>on</strong> charges for regular visits, Rs.5000.00/ m<strong>on</strong>thtowards incentives, Rs.2000.00/m<strong>on</strong>th towards propulsi<strong>on</strong> chares to attendemergency cases and Rs.500.00/m<strong>on</strong>th towards pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al charges forattending emergency cases at any cost.5. Claim should be submitted <strong>on</strong> or before 5 th <str<strong>on</strong>g>of</str<strong>on</strong>g> the succeeding m<strong>on</strong>th.6.
