Report of the NIMHANS - <str<strong>on</strong>g>WHO</str<strong>on</strong>g> <str<strong>on</strong>g>India</str<strong>on</strong>g> <str<strong>on</strong>g>Workshop</str<strong>on</strong>g><strong>on</strong>'<strong>Psychosocial</strong> <strong>Support</strong> <strong>in</strong> <strong>Disaster</strong>'BackgroundMore than <strong>on</strong>e year has passed s<strong>in</strong>ce the Tsunami struck and caused devastati<strong>on</strong> <strong>in</strong> variousparts of the world, leav<strong>in</strong>g a severe impact <strong>on</strong> the lives of <strong>in</strong>dividuals, families andcommunities. Mental health professi<strong>on</strong>als and allied experts have been extend<strong>in</strong>g theirexperience through paraprofessi<strong>on</strong>als, n<strong>on</strong> professi<strong>on</strong>als and n<strong>on</strong> governmental organizati<strong>on</strong>sto help survivors of the Tsunami <strong>in</strong> Tamil Nadu, P<strong>on</strong>dicherry, Andhra Pradesh andAndaman and Nicobar Islands <strong>in</strong> rebuild<strong>in</strong>g their lives.As a nodal centre for Tsunami psychosocial care, the Nati<strong>on</strong>al Institute of Mental Healthand Neuro Sciences (NIMHANS), Bangalore, had the resp<strong>on</strong>sibility of tak<strong>in</strong>g stock of thequantity and quality of psychosocial services that were provided and their impact <strong>on</strong>survivors' lives. Al<strong>on</strong>g with discussi<strong>on</strong>s <strong>on</strong> the various strategies adopted to help survivors,an important aim was to c<strong>on</strong>tribute to the formulati<strong>on</strong> of disaster preparedness programmesof the Government of <str<strong>on</strong>g>India</str<strong>on</strong>g>.With these c<strong>on</strong>cepts <strong>in</strong> m<strong>in</strong>d, a Nati<strong>on</strong>al <str<strong>on</strong>g>Workshop</str<strong>on</strong>g> was planned and jo<strong>in</strong>tly organized byNIMHANS, Bangalore and <str<strong>on</strong>g>WHO</str<strong>on</strong>g> Country Office, <str<strong>on</strong>g>India</str<strong>on</strong>g>, New Delhi <strong>on</strong> '<strong>Psychosocial</strong><strong>Support</strong> <strong>in</strong> <strong>Disaster</strong>' <strong>on</strong> the 3rd & 4th of February, 2006 at NIMHANS.ObjectivesThe specific objectives of the workshop were:• To share experiences of provid<strong>in</strong>g psychosocial care to survivors of the Tsunami, ata nati<strong>on</strong>al level.• To review the observati<strong>on</strong>s and percepti<strong>on</strong>s about difficulties encountered bysurvivors of the Tsunami.• To collate the strategies adopted by professi<strong>on</strong>als <strong>in</strong> reach<strong>in</strong>g out to victims of theTsunami.• To understand the activities undertaken at the field level by paraprofessi<strong>on</strong>als andn<strong>on</strong> - professi<strong>on</strong>als from voluntary agencies.• To arrive at models of psychosocial support service provisi<strong>on</strong> <strong>in</strong> disaster situati<strong>on</strong>s.• To provide recommendati<strong>on</strong>s <strong>on</strong> <strong>in</strong>terventi<strong>on</strong>s <strong>in</strong> disaster situati<strong>on</strong>s.1


MethodologyIn order to make the workshop an <strong>in</strong>teractive process, the structure (Appendix 1) providedspace for participants to present their experiences <strong>in</strong> a broad as well as <strong>in</strong>-depth mannerand to aid <strong>in</strong>formal shar<strong>in</strong>g.The workshop began with presentati<strong>on</strong>s from agencies and experts from various parts ofthe country <strong>on</strong> efforts and service provisi<strong>on</strong> regi<strong>on</strong>-wise - Tamil Nadu, Andaman & NicobarIslands, P<strong>on</strong>dicherry, Kerala and Andhra Pradesh.After these presentati<strong>on</strong>s, participants were divided <strong>in</strong>to groups for focused discussi<strong>on</strong>s <strong>on</strong>four key areas:• Capacity build<strong>in</strong>g and tra<strong>in</strong><strong>in</strong>g• Collaborati<strong>on</strong> and co-ord<strong>in</strong>ati<strong>on</strong>• Relief and rehabilitati<strong>on</strong>• M<strong>on</strong>itor<strong>in</strong>g and evaluati<strong>on</strong>Each group was moderated by participants with expertise <strong>in</strong> aspects of disaster managementrelevant to the areas under deliberati<strong>on</strong>. The groups facilitated detailed discussi<strong>on</strong>s andspecific shar<strong>in</strong>g of learn<strong>in</strong>gs, lead<strong>in</strong>g to formulati<strong>on</strong> of recommendati<strong>on</strong>s <strong>on</strong> the key areas.These were then presented to the larger group. A collati<strong>on</strong> of f<strong>in</strong>al recommendati<strong>on</strong>s waspresented prior to the clos<strong>in</strong>g of the workshop. An open sessi<strong>on</strong> c<strong>on</strong>cluded the proceed<strong>in</strong>gs.In additi<strong>on</strong>, a display of tra<strong>in</strong><strong>in</strong>g materials that were used by professi<strong>on</strong>als, paraprofessi<strong>on</strong>alsand n<strong>on</strong>-professi<strong>on</strong>als from various parts of the country for psychosocial care services <strong>in</strong>disaster management, was organized. This display <strong>in</strong>cluded artwork created by childsurvivors of Tsunami. The display area acted as an <strong>in</strong>formal space for discussi<strong>on</strong>s <strong>on</strong>creati<strong>on</strong> of the materials and experiences that c<strong>on</strong>tributed to it. The space also aidednetwork<strong>in</strong>g am<strong>on</strong>g <strong>in</strong>dividuals and organizati<strong>on</strong>s work<strong>in</strong>g <strong>in</strong> the field of disastermanagement.Materials presented and used <strong>in</strong> the workshop were provided <strong>in</strong> the form of kits to participants,which <strong>in</strong>cluded copies of the presentati<strong>on</strong>, NIMHANS publicati<strong>on</strong>s like IEC materials,posters and <strong>in</strong>formati<strong>on</strong> pamphlets <strong>on</strong> 'techniques of psychosocial care for survivors ofdisaster, psychosocial care for children'; leaflets <strong>on</strong> 'psychosocial care for the disastersurvivors, disaster <strong>in</strong>formati<strong>on</strong> for Individuals/ Families / Community Level Workers;<strong>in</strong>formati<strong>on</strong> manuals 1,2,3,4 for <strong>in</strong>dividuals and families, community level workers,work<strong>in</strong>g with children and women respectively; tra<strong>in</strong><strong>in</strong>g tool kits like 'My Workbook' andTOT Informati<strong>on</strong> manual'. All these materials were provided <strong>in</strong> English and regi<strong>on</strong>allanguages H<strong>in</strong>di, Tamil, Telugu, and Malayalam. The <str<strong>on</strong>g>WHO</str<strong>on</strong>g> report <strong>on</strong> Tsunami 'MentalHealth and <strong>Psychosocial</strong> Relief Efforts after the Tsunami <strong>in</strong> South-East Asia' and '<str<strong>on</strong>g>WHO</str<strong>on</strong>g>Framework for Mental Health and <strong>Psychosocial</strong> <strong>Support</strong> after the Tsunami' was alsodistributed to the participants.2


For evaluati<strong>on</strong> of the workshop, participants were requested to fill <strong>in</strong> feedback forms withsuggesti<strong>on</strong>s for improvement.ParticipantsThere were 152 Professi<strong>on</strong>als, Policy makers, Adm<strong>in</strong>istrators, Representative of CivilSociety Organizati<strong>on</strong>s, Students and Survivors of Tsunami participated the workshop(Appendix 2).Proceed<strong>in</strong>gsInaugurati<strong>on</strong>The <strong>in</strong>augural sessi<strong>on</strong> commenced with an <strong>in</strong>vocati<strong>on</strong> s<strong>on</strong>g. Dr. Parthasarathy, Professorand Head of Department of Psychiatric Social Work, NIMHANS, extended a welcome toall the participants of the workshop and press pers<strong>on</strong>s, with a special menti<strong>on</strong> of thecommunity level workers and survivors of the Tsunami who were attend<strong>in</strong>g the event. He<strong>in</strong>troduced all the dignitaries present <strong>on</strong> the stage and expressed appreciati<strong>on</strong> for their<strong>in</strong>puts <strong>in</strong>to the <strong>on</strong>go<strong>in</strong>g psychosocial care programmes <strong>in</strong> Tsunami affected areas and towardsthe workshop.Dr. Cherian Varghese, Co-ord<strong>in</strong>ator (N<strong>on</strong> Communicable Diseases & Mental Health), <str<strong>on</strong>g>WHO</str<strong>on</strong>g>Country Office <str<strong>on</strong>g>India</str<strong>on</strong>g> representative to <str<strong>on</strong>g>India</str<strong>on</strong>g>, provided the background to the workshop.He stressed <strong>on</strong> the need to suggest feasible community based <strong>in</strong>terventi<strong>on</strong>s <strong>in</strong> collaborati<strong>on</strong>with the Nati<strong>on</strong>al <strong>Disaster</strong> Management Authority, which should be the best mix of socialand medical models available for psychosocial care. The emphasis of such models must be<strong>on</strong> disaster preparedness.Dr. D<strong>in</strong>esh Bhugra, Dean, Royal College of Psychiatrists, UK, delivered the felicitati<strong>on</strong>address, highlight<strong>in</strong>g the need for cultural specificity <strong>in</strong> psychosocial care and c<strong>on</strong>textualiz<strong>in</strong>gthe needs of survivors. The less<strong>on</strong>s learnt regard<strong>in</strong>g the cultural <strong>in</strong>fluences <strong>on</strong> help-seek<strong>in</strong>gbehaviors, k<strong>in</strong>ds of services and modes of delivery for the community must be analyzed <strong>in</strong>order to develop culture specific <strong>in</strong>terventi<strong>on</strong>s.Dr. Vijay Chandra, Regi<strong>on</strong>al advisor, Mental Health, <str<strong>on</strong>g>WHO</str<strong>on</strong>g> Regi<strong>on</strong>al Office for South EastAsia, delivered a special address <strong>on</strong> 'Tsunami <strong>Disaster</strong> and Mental Health - Regi<strong>on</strong>al Picture'.He brought attenti<strong>on</strong> to the process of evaluati<strong>on</strong> of the impact of work that has takenplace. He po<strong>in</strong>ted out the need for def<strong>in</strong>iti<strong>on</strong>s of psychosocial care to prevent trivializati<strong>on</strong>of efforts.The keynote address was delivered by Prof. V<strong>in</strong>od Chandra Men<strong>on</strong>, Member, Nati<strong>on</strong>al<strong>Disaster</strong> Management Authority. He felt the workshop was a celebrati<strong>on</strong> of efforts to reachout to each other, the strength of survivors, values such as compassi<strong>on</strong> and professi<strong>on</strong>alism.He emphasized the need for an <str<strong>on</strong>g>India</str<strong>on</strong>g>n network for psychosocial emergencies and disasters.The magnitude of work dur<strong>in</strong>g the Tsunami would have been much better had such anetwork existed. He also touched up<strong>on</strong> gender dimensi<strong>on</strong>s <strong>in</strong> psychosocial care.3


The presidential address was delivered by Prof. D. Nagaraja, Director / Vice Chancellor,NIMHANS. He highlighted the role of NIMHANS <strong>in</strong> disaster management over the last 25years, especially <strong>in</strong> Latur, Bhopal and Gujarat. Outl<strong>in</strong><strong>in</strong>g the details of plann<strong>in</strong>g whichtook place <strong>in</strong> NIMHANS for Tsunami psycho social care, he expla<strong>in</strong>ed how the resourcesand field work lead to the recogniti<strong>on</strong> of the <strong>in</strong>stitute as a nodal centre. He appreciated theefforts of the Department of Psychiatric Social Work and spoke of the need for governmentaland NGO collaborati<strong>on</strong> for susta<strong>in</strong>ability of <strong>in</strong>terventi<strong>on</strong>s.Dr. K Sekar, Professor, Department of Psychiatric Social Work, NIMHANS, proposed thevote of thanks. He thanked the dignitaries for provid<strong>in</strong>g <strong>in</strong>puts <strong>in</strong>to the formulati<strong>on</strong> of theworkshop and participat<strong>in</strong>g <strong>in</strong> the exercise. He also thanked all the participants, departmentsof NIMHANS and the media for participat<strong>in</strong>g.Sessi<strong>on</strong> 1This sessi<strong>on</strong> c<strong>on</strong>centrated <strong>on</strong> resp<strong>on</strong>ses to the Tsunami <strong>in</strong> Tamil Nadu. Al<strong>on</strong>g with present<strong>in</strong>g data <strong>on</strong> the psychosocialcare delivery through Community Level Workers (CLW) <strong>in</strong> Tamil Nadu, Andhra Pradesh,P<strong>on</strong>dicherry and Kerala he stressed <strong>on</strong> aspects such as social and cultural sensitivity of<strong>in</strong>terventi<strong>on</strong>s and us<strong>in</strong>g active outreach approach rather than more traditi<strong>on</strong>al medicalstance. He highlighted a generic model of provid<strong>in</strong>g psychosocial support <strong>in</strong> disastersituati<strong>on</strong>s and spoke about work<strong>in</strong>g with CLWs <strong>in</strong> this c<strong>on</strong>text. The UN team for recoveryled by <str<strong>on</strong>g>WHO</str<strong>on</strong>g> <str<strong>on</strong>g>India</str<strong>on</strong>g> and other partners has developed a model for susta<strong>in</strong>ed communitiesbased psychosocial support and has tra<strong>in</strong>ed 13,000 CLWS, who have provided care tomore than 300,000 <strong>in</strong>dividuals.Dr. K Sekar presented a report <strong>on</strong> the '<strong>Psychosocial</strong> support <strong>in</strong> Tsunami disaster - NIMHANSresp<strong>on</strong>se <strong>in</strong> Tamil Nadu, P<strong>on</strong>dicherry and Andhra Pradesh'. He began by stat<strong>in</strong>g that theresp<strong>on</strong>ses of survivors to disasters are normal reacti<strong>on</strong>s to abnormal situati<strong>on</strong>s. Theperspective of <strong>in</strong>terventi<strong>on</strong>s should therefore be <strong>on</strong> normaliz<strong>in</strong>g cop<strong>in</strong>g patterns andencourag<strong>in</strong>g mastery of the situati<strong>on</strong>, not of mental illness. In this c<strong>on</strong>text, communitylevel workers often have to c<strong>on</strong>centrate <strong>on</strong> survivors need for ventilati<strong>on</strong> rather than therapy.He presented the needs assessment results from Tamil Nadu and Andhra Pradesh and data<strong>on</strong> at-risk populati<strong>on</strong>s, changes <strong>in</strong> family structures and gender dimensi<strong>on</strong>s. He also presentedcomparis<strong>on</strong> data of six m<strong>on</strong>ths after the disaster. Highlight<strong>in</strong>g the normal-deviancy model,he spoke of process of tra<strong>in</strong><strong>in</strong>g of CLWs where handhold<strong>in</strong>g is a must as simply impart<strong>in</strong>gtra<strong>in</strong><strong>in</strong>g would not help <strong>in</strong> the l<strong>on</strong>g run. Capacity build<strong>in</strong>g tools which were used <strong>in</strong> thetra<strong>in</strong><strong>in</strong>g were specific to community needs. He c<strong>on</strong>cluded with a few po<strong>in</strong>ts <strong>on</strong> NIMHANSpolicy <strong>in</strong>itiatives, especially with the district mental health programs and the IIadm<strong>in</strong>istrative reform committee meet<strong>in</strong>g where psychosocial care was added as a part ofthe agenda.4


Dr. Vivek Benegal, Associate Professor, Department of Psychiatry, NIMHANS, made apresentati<strong>on</strong> <strong>on</strong> 'Aftermath of Tsunami - Handl<strong>in</strong>g the psychosocial impact <strong>in</strong> Andamanand Nicobar Islands'. He followed the format of visual rather than text slides to br<strong>in</strong>gdisaster situati<strong>on</strong>s to life. Some of the barriers for workers <strong>in</strong> the Island were destructi<strong>on</strong>,loss of life, displacement, disability, loss of <strong>in</strong>stituti<strong>on</strong>s and poor communicati<strong>on</strong> network.He spoke about a rift between local populati<strong>on</strong>s, especially tribal and settlers <strong>in</strong> terms ofresettlement, land and alternative livelihood plans. The NIMHANS team began withc<strong>on</strong>centrati<strong>on</strong> <strong>on</strong> assessment and medical care. They so<strong>on</strong> started tra<strong>in</strong><strong>in</strong>g for CLWs andcounsel<strong>in</strong>g services. He presented data <strong>on</strong> levels of psychosocial care provisi<strong>on</strong>, spectrumof emoti<strong>on</strong>al and behavioral disorders am<strong>on</strong>g survivors. The team focused <strong>on</strong> needs ofspecial populati<strong>on</strong>s such as women, children and pers<strong>on</strong>s with substance abuse. He c<strong>on</strong>cludedwith appreciati<strong>on</strong> of preparedness of adm<strong>in</strong>istrati<strong>on</strong> <strong>in</strong> the island.Dr. Dhanasekara Pandian, Assistant Professor, Department of Psychiatric Social Work,NIMHANS, presented <strong>on</strong> '<strong>Psychosocial</strong> care services for survivors of Tsunami <strong>in</strong> TamilNadu - NIMHANS resp<strong>on</strong>se'. He provided details of the different k<strong>in</strong>ds of tra<strong>in</strong><strong>in</strong>g programsthat were c<strong>on</strong>ducted based <strong>on</strong> the needs of particular target groups. The model for thetra<strong>in</strong><strong>in</strong>g was to strengthen exist<strong>in</strong>g local groups such as panchayats, mahila mandals, youthclubs, teachers and anganwadi workers. He gave details of various programs <strong>in</strong> terms ofdurati<strong>on</strong> of tra<strong>in</strong><strong>in</strong>g, number of participants, resource pers<strong>on</strong>s, dates and different k<strong>in</strong>ds ofgroups that the NIMHANS team reached out to such as colleges, teachers, NGO workers,survivors. He displayed clipp<strong>in</strong>gs of media coverage of NIMHANS efforts. He ended thepresentati<strong>on</strong> with an appraisal of impact of tra<strong>in</strong><strong>in</strong>gs - better outreach, tra<strong>in</strong>ed pers<strong>on</strong>stra<strong>in</strong><strong>in</strong>g others, <strong>in</strong>creased efficiency <strong>in</strong> psychosocial efforts.Ms. Supriya Sahoo, IAS, Additi<strong>on</strong>al Secretary to Government, Health & Family Welfare,Tamil Nadu, presented a paper <strong>on</strong> the government of Tamil Nadu's resp<strong>on</strong>se to the Tsunamidisaster. She gave a brief<strong>in</strong>g <strong>on</strong> the c<strong>on</strong>cerns and issues that the government faced dur<strong>in</strong>gefforts at rescue, relief and rehabilitati<strong>on</strong>. A major c<strong>on</strong>cern was quick and effective disposalof dead bodies <strong>in</strong> order to reduce epidemics as well as the trauma of survivors. Mobilizati<strong>on</strong>of different departments was important <strong>in</strong> deal<strong>in</strong>g with the issue. Mobilizati<strong>on</strong> of men andmaterial was the next major c<strong>on</strong>cern. Operati<strong>on</strong>aliz<strong>in</strong>g medical <strong>in</strong>stituti<strong>on</strong>s which hadbeen affected by the Tsunami was of utmost importance. A less<strong>on</strong> that the adm<strong>in</strong>istrati<strong>on</strong>learnt was the need for ready lists of resources for every stage of efforts. She identifiedsteady flow of <strong>in</strong>formati<strong>on</strong> and communicati<strong>on</strong> and the restorati<strong>on</strong> of basic <strong>in</strong>frastructureas vital to disaster management for the government. She admitted that the realizati<strong>on</strong> ofmental health needs of the people was delayed <strong>on</strong> the part of the government, but theengagement of social workers and CLWs was immediately put <strong>in</strong> place for this purposerather than send<strong>in</strong>g psychiatrists to fulfill this need. She provided data <strong>on</strong> the strength ofmental health staff <strong>in</strong> the state and ended with the less<strong>on</strong>s that the government has learntfrom the entire effort.Ms. Gemma De Silva, Assistant Director, Department of Social Welfare, Government ofTamil Nadu, presented a report <strong>on</strong> the resp<strong>on</strong>se of the Department of Social Welfare to theTsunami disaster. The department was identified as the agency to provide psychosocialsupport, which was developed with the UN recovery team. She focused <strong>on</strong> details of the5


tra<strong>in</strong><strong>in</strong>g programmes and spoke of the process of formulat<strong>in</strong>g tra<strong>in</strong><strong>in</strong>gs as well aspsychosocial support programmes. She also threw light <strong>on</strong> the process of organizati<strong>on</strong> andco-ord<strong>in</strong>ati<strong>on</strong> at district level. Currently, the agency has started a cell to m<strong>on</strong>itor progressof programmes and has appo<strong>in</strong>ted staff to c<strong>on</strong>t<strong>in</strong>ue efforts. The role of CLWs has beenmodified where they act as resource pers<strong>on</strong>s for <strong>in</strong>formati<strong>on</strong> <strong>on</strong> where to go and whom toapproach for various purposes. In future, the department <strong>in</strong>tends to strengthen referralcare, develop skill build<strong>in</strong>g <strong>in</strong> the health system to deal with disasters and ensurec<strong>on</strong>t<strong>in</strong>uati<strong>on</strong> of services.Mr. Sujith John, Schizophrenia Research Foundati<strong>on</strong> (SCARF), Chennai, presented a paper<strong>on</strong> 'SCARF's resp<strong>on</strong>se to the Tsunami disaster'. He provided details of tra<strong>in</strong><strong>in</strong>g programmesc<strong>on</strong>ducted and activities undertaken by the organizati<strong>on</strong>. Most tra<strong>in</strong><strong>in</strong>g programmes werec<strong>on</strong>ducted <strong>in</strong> phases as CLWs were not available <strong>in</strong> <strong>on</strong>e stretch. He highlighted the use ofmedia such as films to create awareness am<strong>on</strong>g survivors about Tsunami to reducesuperstiti<strong>on</strong>, fear and to foster c<strong>on</strong>fidence. He spoke about the various reacti<strong>on</strong>s of survivorsthat were seen, rang<strong>in</strong>g from blame and guilt to death wishes and state of shock. Hepresented data <strong>on</strong> needs assessment that was carried out <strong>in</strong> terms of variables of survivorssuch as age and sex and also, the major diagnoses. The future agenda of the organizati<strong>on</strong><strong>in</strong>cludes expansi<strong>on</strong> of mental health services and creati<strong>on</strong> of a network of agencies fordisaster preparedness.Mr. Saji Thomas presented a paper <strong>on</strong> psychosocial care and support for children as part ofthe UNICEF resp<strong>on</strong>se to the Tsunami disaster. He detailed the goals of a psychosocialsupport programme as not just provid<strong>in</strong>g counsel<strong>in</strong>g, but creat<strong>in</strong>g an empathetic envir<strong>on</strong>mentthat supports the needs of survivors. He also spoke about the activities undertaken, which<strong>in</strong>cluded provisi<strong>on</strong> of play materials, counsel<strong>in</strong>g, form<strong>in</strong>g children's groups and life skillstra<strong>in</strong><strong>in</strong>g. He further highlighted the larger framework of protecti<strong>on</strong> that UNICEF workswith. A journalist was <strong>in</strong>vited to objectively observe the children's activities and presenttheir voices <strong>on</strong> the disaster and its aftermath. Dr. RameelaShekar, Professor of Social Work, School of Social Work, Roshni Nilaya, Mangalore andDr. Sudha Bhogle, Professor& Head, Department of Psychology, Bangalore University,chaired the sessi<strong>on</strong>.Dr.Chandrashekar, Professor, Department of Psychiatry, JIPMER, P<strong>on</strong>dicherry, provideda brief picture of the Government of P<strong>on</strong>dicherry's resp<strong>on</strong>se to the Tsunami disaster. Hedetailed the c<strong>on</strong>tent, methodology and implementati<strong>on</strong> of tra<strong>in</strong><strong>in</strong>g programmes. He endedwith clipp<strong>in</strong>gs <strong>in</strong> local media <strong>on</strong> the psychosocial care efforts.Dr. Raju, Professor of Psychiatry, Medical College, Thiruvananthapuram and Secretary ofthe Kerala Mental Health Authority (KSHMA) presented <strong>on</strong> KSHMA's resp<strong>on</strong>se to theTsunami disaster <strong>in</strong> Kerala. He spoke of the creati<strong>on</strong> of mental health teams and discussi<strong>on</strong>s6


<strong>in</strong> the state level meet<strong>in</strong>g <strong>on</strong> co-ord<strong>in</strong>ati<strong>on</strong>, c<strong>on</strong>stituti<strong>on</strong> of committees and c<strong>on</strong>duct<strong>in</strong>gtra<strong>in</strong><strong>in</strong>g programmes. He touched up<strong>on</strong> the ethics of research while collect<strong>in</strong>g data <strong>in</strong>disaster situati<strong>on</strong>s.Dr. Mohan Roy, Nodal Officer, <str<strong>on</strong>g>WHO</str<strong>on</strong>g> - PSS Project, Allepey, presented <strong>on</strong> the AllepeyMedical College's resp<strong>on</strong>se to the Tsunami disaster. Al<strong>on</strong>g with a profile of the district, hegave details of the logistical aspects of emergency care start<strong>in</strong>g from the day the Tsunamistruck. He provided an overview of the tra<strong>in</strong><strong>in</strong>g provided and co-ord<strong>in</strong>ati<strong>on</strong> at state andnati<strong>on</strong>al levels.Dr. Gowrie Devi, Professor of Psychiatry, Super<strong>in</strong>tendent, Institute of Mental Health andMember Secretary, Andhra Pradesh State Mental Health Authority, Hyderabad, made apresentati<strong>on</strong> <strong>on</strong> the PSS programme for Tsunami affected victims <strong>in</strong> Andhra Pradesh. Sheacknowledged that the extent of damage was not extensive <strong>in</strong> Andhra and the governmentwas c<strong>on</strong>cerned more with relief measures rather than mental health aspects. The localpopulati<strong>on</strong>s were also more <strong>in</strong>terested <strong>in</strong> material help rather than psychosocial support.She provided details of the tra<strong>in</strong><strong>in</strong>g at community and district levels that were c<strong>on</strong>ductedal<strong>on</strong>g with <str<strong>on</strong>g>WHO</str<strong>on</strong>g>.In the discussi<strong>on</strong> sessi<strong>on</strong>, some po<strong>in</strong>ts that came up were regard<strong>in</strong>g the need for shar<strong>in</strong>gbetween agencies as there is a lot of overlap <strong>in</strong> tra<strong>in</strong><strong>in</strong>g, <strong>in</strong>terventi<strong>on</strong>s and materials produced.The shar<strong>in</strong>g would lead to mak<strong>in</strong>g best of available resources and prevent<strong>in</strong>g mistakes <strong>in</strong>the field. The ethics of research were discussed where it was felt that research for the sakeof writ<strong>in</strong>g papers or collecti<strong>on</strong> of irrelevant data <strong>in</strong> disaster situati<strong>on</strong>s was unethical. Thetools for data collecti<strong>on</strong> must not be cumbersome such that they imp<strong>in</strong>ge <strong>on</strong> psychosocialcare efforts. At the same time, it was acknowledged that data about the field situati<strong>on</strong> wasimportant for understand<strong>in</strong>g the needs of survivors and assess<strong>in</strong>g the impact of <strong>in</strong>terventi<strong>on</strong>s.Sessi<strong>on</strong> 3 - C<strong>on</strong>current Presentati<strong>on</strong> & Group Discussi<strong>on</strong>Participants were divided <strong>in</strong>to four c<strong>on</strong>current groups and experts chaired the discussi<strong>on</strong>s.Group 1 deliberated <strong>on</strong> Capacity Build<strong>in</strong>g and Tra<strong>in</strong><strong>in</strong>g. Dr. S Gururaj, Professor & Headof the Department of Epidemiology, NIMHANS and Dr. Kiran Rao, Professor, Departmentof Mental Health & Social Psychology, NIMHANS chaired the sessi<strong>on</strong>. The sessi<strong>on</strong> beganwith shar<strong>in</strong>g of experiences of Mr. Muniandi from Velankanni. He spoke about his pers<strong>on</strong>alloss, <strong>in</strong>ability to help others, dump<strong>in</strong>g of relief materials and the help he received fromNGOs. Dr. Poppy Kannan, Professor, Department of Social Work, Stella Maris College,Chennai, made a presentati<strong>on</strong> <strong>on</strong> tra<strong>in</strong><strong>in</strong>g programme for volunteers <strong>on</strong> psychosocial carefor survivors of Tsunami. Dr. K. Redamma, Professor and Head of Department of Nurs<strong>in</strong>g,NIMHANS, made a presentati<strong>on</strong> <strong>on</strong> capacity build<strong>in</strong>g <strong>in</strong>itiatives taken by <str<strong>on</strong>g>India</str<strong>on</strong>g>n Nurs<strong>in</strong>gCouncil. Dr. Cecilia Thangarajan, Professor and Headof Department of Social Work, Stella Maris College, Chennai, made a presentati<strong>on</strong> <strong>on</strong>collaborative efforts of schools of social work <strong>in</strong> Tsunami disaster. Dr. Thulsidasan, AIDSC<strong>on</strong>trol Society, Andaman and Nicobar Islands, presented <strong>on</strong> psychosocial support resp<strong>on</strong>seto Tsunami <strong>in</strong> Andaman and Nicobar. Dr. Sudha Bhogle, Department of Psychology,Bangalore University, presented <strong>on</strong> role of Centre for Psychological Counsell<strong>in</strong>g,Department of Psychology, Bangalore University <strong>in</strong> tra<strong>in</strong><strong>in</strong>g and counsel<strong>in</strong>g services toTsunami affected areas.Group 3 discoursed <strong>on</strong> Relief and Rehabilitati<strong>on</strong>. Dr. SureshBadamath, Assistant Professor, Department pf Psychiatry, NIMHANS, presented <strong>on</strong> activitiesof NIMHANS team at Andaman and Nicobar dur<strong>in</strong>g the early phase of Tsunami disaster.Dr. Srilatha Juvva, Reader of Social Work, Tata Institute of Social Sciences, Mumbai,presented a report of the TISS <strong>in</strong>itiatives <strong>in</strong> Nagapatt<strong>in</strong>am. The deliberati<strong>on</strong>s and recommendati<strong>on</strong>s of Group 1 were presented by Ms. Vanitha C.,M. Phil. Tra<strong>in</strong>ee, Department of Psychiatric Social Work, NIMHANS. The key po<strong>in</strong>tsthat emerged from the discussi<strong>on</strong> were:• Tra<strong>in</strong>ers should come from with<strong>in</strong> the community rather than from outside, <strong>in</strong> orderto ma<strong>in</strong>ta<strong>in</strong> susta<strong>in</strong>ability and culture specificity.• C<strong>on</strong>tents of tra<strong>in</strong><strong>in</strong>g should be tailor-made based <strong>on</strong> target populati<strong>on</strong>.• Educati<strong>on</strong> of workers must be c<strong>on</strong>ducted at grass root level• Tra<strong>in</strong><strong>in</strong>g manuals should <strong>in</strong>clude issues of alcohol and addicti<strong>on</strong>, HIV/AIDS andother high risk behaviours that could be fall out of disasters.• Tra<strong>in</strong><strong>in</strong>g should be cost-effective, mak<strong>in</strong>g use of resources available <strong>in</strong> the community.• There should be focus <strong>on</strong> life skills and stress management.• Network<strong>in</strong>g of tra<strong>in</strong>ed pers<strong>on</strong>nel must be improved to facilitate easy mobilizati<strong>on</strong>dur<strong>in</strong>g crisis.• NIMHANS should be the nodal centre for tra<strong>in</strong><strong>in</strong>g.The discussi<strong>on</strong> after the presentati<strong>on</strong> stressed <strong>on</strong> need for co-ord<strong>in</strong>ati<strong>on</strong> cells <strong>in</strong> every statewhere lists of tra<strong>in</strong>ed pers<strong>on</strong>nel is ma<strong>in</strong>ta<strong>in</strong>ed and it becomes part of disaster preparedness.Sensitizati<strong>on</strong> of the media was another aspect where it was felt that negative projecti<strong>on</strong>smust be reduced and focus must be <strong>on</strong> resp<strong>on</strong>sible reportage.The recommendati<strong>on</strong>s of Group 2 were presented by Dr. Ramasubramaniam, Professor ofPsychiatry, MMC, Madurai.• Co-ord<strong>in</strong>ati<strong>on</strong> for disaster management must be d<strong>on</strong>e by Panchayats al<strong>on</strong>g withgovernmental agencies.• A board must be c<strong>on</strong>stituted for collaborati<strong>on</strong> c<strong>on</strong>sist<strong>in</strong>g of NGOs, media,government agencies.• There needs to be a centralized co-ord<strong>in</strong>ati<strong>on</strong> centre for resource management,fund<strong>in</strong>g and support to avoid duplicati<strong>on</strong> of services or neglect of isolated pockets.• Competitiveness am<strong>on</strong>g local and state level NGOs must be avoided.• Preference should be given to local organizati<strong>on</strong>s who are better equipped tounderstand local needs.The discussi<strong>on</strong> raised the po<strong>in</strong>t of an active dialogue that must be established between thegovernment and NGOs <strong>in</strong> order to promote trust and accountability and improve coord<strong>in</strong>ati<strong>on</strong>.In this c<strong>on</strong>text, the lack of co-ord<strong>in</strong>ati<strong>on</strong> between government departmentswas brought to light. A sense of resentment would be seen when there was overlap <strong>in</strong> roles.Officials must work towards better communicati<strong>on</strong>. NGO co-ord<strong>in</strong>ati<strong>on</strong> cells were alsoseen as a must <strong>in</strong> all districts <strong>in</strong> order to ensure that all populati<strong>on</strong>s are reached andappropriate k<strong>in</strong>ds of help are given. The cells could also work towards shar<strong>in</strong>g of<strong>in</strong>formati<strong>on</strong> and meet<strong>in</strong>gs with government officials for plann<strong>in</strong>g.9


Sessi<strong>on</strong> 5 - Shar<strong>in</strong>g and delibrati<strong>on</strong>s <strong>in</strong> group 3 & 4The presentati<strong>on</strong>s of Group 3 & 4 were chaired by Dr. Redamma, Professor and Head ofDepartment of Nurs<strong>in</strong>g, NIMHANS and Dr. Satish Girimaji, Professor, Department ofPsychiatry, NIMHANS.The deliberati<strong>on</strong>s and recommendati<strong>on</strong>s of Group 3 were presented by Dr. Srilatha Juvva,Reader of Social Work, Tata Institute of Social Sciences, Mumbai.• M<strong>in</strong>imum standards of care <strong>in</strong> relief and rehabilitati<strong>on</strong> as recommended by SPHEREmust be adhered to.• In order to make health care available and accessible, satellite cl<strong>in</strong>ics arerecommended.• Health needs of women, children, elderly and disabled should be given priority.• Capacity build<strong>in</strong>g for relief and rehabilitati<strong>on</strong> services should be given to allstakeholders.• Stakeholders must be sensitized to psychosocial care, which should be mandated bythe government.• Efforts must be aimed at develop<strong>in</strong>g exist<strong>in</strong>g / new organizati<strong>on</strong>s such as specialschools, homes for orphans, destitute, elderly, organizati<strong>on</strong>s work<strong>in</strong>g with women,de-addicti<strong>on</strong> issues.• Curriculum for management of humanitarian emergency and psychosocial care tobe <strong>in</strong>stituted <strong>in</strong> schools, colleges, centres for professi<strong>on</strong>al educati<strong>on</strong>.• Database and other <strong>in</strong>formati<strong>on</strong> regard<strong>in</strong>g service providers should be made availableat Panchayath, Block and District levels.The discussi<strong>on</strong> highlighted the need to put people <strong>on</strong> their feet as so<strong>on</strong> as possible as apowerful means of cop<strong>in</strong>g and rebuild<strong>in</strong>g of lives. The accent must be <strong>on</strong> help<strong>in</strong>g survivorsto help themselves rather than <strong>on</strong>ly provid<strong>in</strong>g different k<strong>in</strong>ds of help. The role ofpsychiatrists was more <strong>in</strong> the acute phase where diagnosis and medical treatment areimportant. But they could c<strong>on</strong>t<strong>in</strong>ue to c<strong>on</strong>tribute to counsel<strong>in</strong>g and follow-up effortsthereafter.The recommendati<strong>on</strong>s of Group 4 were presented by Dr. Jahanara, Assistant Professor,Psychiatric Social Work and IHBAS;• There should be a mix of qualitative and quantitative measures for m<strong>on</strong>itor<strong>in</strong>g andevaluati<strong>on</strong>.• Emphasis should be <strong>on</strong> ethics of research and c<strong>on</strong>fidentiality and also, <strong>in</strong> <strong>in</strong>terventi<strong>on</strong>s.• There should be systems to study the effectiveness of m<strong>on</strong>itor<strong>in</strong>g and evaluati<strong>on</strong>itself.• There is a need to localize and c<strong>on</strong>ceptualize global <strong>in</strong>dicators.• There is a need for strict process <strong>in</strong>dicators <strong>in</strong> the form of budgets and plans.10


The discussi<strong>on</strong> emphasized a need to review m<strong>on</strong>itor<strong>in</strong>g and evaluati<strong>on</strong> as more positiveprocesses rather than polic<strong>in</strong>g or supervisi<strong>on</strong>. They were seen more as a means for sett<strong>in</strong>gstandards of care and to aid replicati<strong>on</strong> of efforts and best practices. They could also betools of empowerment and learn<strong>in</strong>g. The two processes were ma<strong>in</strong>ly c<strong>on</strong>centrated <strong>on</strong> two<strong>in</strong>terests - service and research. But these <strong>in</strong>terests must be balanced properly and a dilemmawas for such efforts to be proporti<strong>on</strong>ate to the psychosocial support work. Evaluati<strong>on</strong>must focus <strong>on</strong> process utilizati<strong>on</strong> and service delivery. Self assessment and evaluati<strong>on</strong> ofeffectiveness as workers was also seen as important.C<strong>on</strong>clud<strong>in</strong>g sessi<strong>on</strong>On completi<strong>on</strong> of the two-day deliberati<strong>on</strong>s, the valedictory functi<strong>on</strong> was presided over byProfessor D. Nagaraja, Director/Vice Chancellor, NIMHANS. Dr. Parthasarathysummarized the proceed<strong>in</strong>gs of the workshop over the two days and shared someobservati<strong>on</strong>s. Dr. G. S. Udayakumar presented the overall recommendati<strong>on</strong>s of the workshopas shown <strong>in</strong> the overleaf.Dr. Vijay Chandra shared his appreciati<strong>on</strong> of the NIMHANS collaborati<strong>on</strong> with <str<strong>on</strong>g>WHO</str<strong>on</strong>g> <strong>in</strong>field work, shar<strong>in</strong>g of expertise and c<strong>on</strong>duct<strong>in</strong>g the workshop. He highlighted the need fordocumentati<strong>on</strong> to create <strong>in</strong>stituti<strong>on</strong>al memory of the psychosocial care efforts. Dr. CherianVarghese spoke of the need for short term as well as l<strong>on</strong>g-term <strong>in</strong>terventi<strong>on</strong>s, but <strong>in</strong> ac<strong>on</strong>textual manner. He urged agencies to also document their work to build a range ofresources. He thanked Prof. Nagaraja and the various organizati<strong>on</strong>s that had participated.Dr. D<strong>in</strong>esh Bhugra felt the workshop was successful, as it had 'raised a lot of questi<strong>on</strong>s'.He stressed <strong>on</strong> the need for a debate <strong>on</strong> the ethics of psychosocial care <strong>in</strong> disaster situati<strong>on</strong>swhere research and <strong>in</strong>terventi<strong>on</strong>s are c<strong>on</strong>ducted <strong>in</strong> c<strong>on</strong>juncti<strong>on</strong> with the community andnot <strong>in</strong> ivory towers. He also spoke about extend<strong>in</strong>g the learn<strong>in</strong>gs from <strong>on</strong>e disaster situati<strong>on</strong>to another.Professor D. Nagaraja appreciated the field workers for their efforts and for engag<strong>in</strong>g <strong>in</strong> aprocess of prepar<strong>in</strong>g for eventualities <strong>in</strong> disaster management. He threw light <strong>on</strong> fund<strong>in</strong>gneeds for psychosocial care efforts and looked forward for c<strong>on</strong>t<strong>in</strong>ued support from agenciesto ensure stability of work. He c<strong>on</strong>cluded by encourag<strong>in</strong>g all present at the workshop toresolve to meet the needs of people <strong>in</strong> disasters.Dr. K Sekar proposed the vote of thanks. He made special menti<strong>on</strong> of the unc<strong>on</strong>diti<strong>on</strong>aland c<strong>on</strong>t<strong>in</strong>ued support of Prof. D. He also thanked variousdepartments <strong>in</strong> NIMHANS, participants, survivors, community level workers, organiz<strong>in</strong>gcommittee members and the press for mak<strong>in</strong>g the workshop a fruitful exercise. He c<strong>on</strong>cludedby stress<strong>in</strong>g <strong>on</strong> the establishment of a network for disaster preparedness.Dr. Parthasarathy thanked Dr. K Sekar for his efforts <strong>in</strong>to organiz<strong>in</strong>g the psychosocial carefor the survivors of Tsunami disaster <strong>in</strong> the South <str<strong>on</strong>g>India</str<strong>on</strong>g>n States as well as the currentworkshop.11


Recommendati<strong>on</strong>s of the workshop:• Capacity build<strong>in</strong>g for psychosocial care services for disaster survivors should<strong>in</strong>corporate Governmental and N<strong>on</strong>-governmental sectors. The <strong>in</strong>volvementof Governmental sector is important for c<strong>on</strong>t<strong>in</strong>uity and l<strong>on</strong>g term careprovisi<strong>on</strong> as various departments are <strong>in</strong>volved <strong>in</strong> meet<strong>in</strong>g the basic needs ofcommunities.• District adm<strong>in</strong>istrati<strong>on</strong>s should keep records of all NGOs work<strong>in</strong>g <strong>in</strong> thearea and c<strong>on</strong>duct a periodical review of the same.• The role of NGOs <strong>in</strong> a broad sense as well as that of each NGO <strong>in</strong> particularmust be def<strong>in</strong>ed. Local NGOs must be given preference over externalagencies to carry out psychosocial care.• <strong>Psychosocial</strong> care should be c<strong>on</strong>sidered a l<strong>on</strong>g term rather than short termprogramme.
