Annual Report 2010
Dr. Steven Feder
American Academy of Pediatrics
Community Achievement Award Winner
Lincoln Medical Partners, General Surgeons • Part of a Big Team • Miles in Motion

Greetings,
“I have never had such great customer service from an entire team of
medical professionals as I did in the 16 hours I was at your facility…
No one made me feel like I was putting them out on a holiday and everyone was upbeat and
professional. Your staff went way out of their way to make sure my husband was comfortable
too. You have a great medical team. The atmosphere must be right to have such great
customer service.”
Every year we receive many notes of thanks like this one. Some are from yearround
residents and many are from seasonal visitors, but each note mentions our highquality
care and our compassionate, personal attention.
Both Miles and St. Andrews enjoy some of the highest patient satisfaction rates in
Maine and in the U.S. While you could easily attribute this to our top-notch medical
staff and employees, ultimately it’s the generous support of the community that
has sustained our organizations and helped us achieve our statewide reputation for
excellence.
Thanks to people like you (our donors, our volunteers), and the tireless work of our
Auxiliary and League members, we were able to purchase vital equipment for our
hospitals, including a new nurse call system and programmable I.V. pumps. We were also able to maintain programs such as the CMCC/
LCH Education Center which help us build a brighter future for both our hospitals and our wider community.
Each year we publish an annual report to update our communities on our goals and progress, and to thank the thousands of supporters
who make our work possible. The many notes of thanks, like the one above, are the direct result of your thoughtful generosity.
On behalf of the trustees, administration, medical staff, and employees of the Lincoln County Healthcare organizations, thank you
so much for your generous support. And, as always, please feel free to contact me at (207) 563-4500 with any suggestions, questions or
concerns.
Sincerely,
James W. Donovan
Doing Whatever It Takes
When Pediatrician Steven Feder isn’t seeing
patients at his offices in Boothbay Harbor and
Damariscotta, he can often be found coaching
them on the playing fields.
Dr. Feder has always been a team player, whether
playing football in college and Scotland (he played
a year of professional football for the Fife 49ers)
or working with other providers to insure the
best possible health services in schools as medical
director of the Lincoln Academy and Boothbay
Region High School health centers.
That dedication has earned him accolades from
the Maine Osteopathic Association, which named
him Maine Outstanding Young Physician in 2002
and Physician of the Year in 2006, and from the
American Academy of Pediatrics, which gave him
a 2010 Community Achievement Award.
Now Dr. Feder, who lives in Boothbay Harbor
with his wife, Amy Winston, has added a new
responsibility.
As the first Medical Staff President of Lincoln
Medical Partners, Dr. Feder will represent the
medical staffs of Miles and St. Andrews Hospitals
on the Lincoln County
Healthcare Board of
Trustees.
Dr. Feder said his top
priorities include working
to insure cohesion
between doctors in both
communities (including
those employed by the
hospital and those in
private practice) and
to bring the benefits of
MaineHealth, the state’s
largest integrated health
care system, to Lincoln County Healthcare
patients.
A more integrated health care system in Lincoln
County means more cost-effective services,
and it also means developing the cooperative
relationships that are critical to responding
effectively to health threats of all description, from
the H1N1 flu to childhood obesity.

Ricki Waltz, RN, School
Nurse at Lincoln Academy
in Newcastle, has worked
with Dr. Feder since he
first came to the county in
1998. He is also her two
sons’ physician.
She trusts Dr. Feder as a
doctor, and she also knows
she can count on him as
medical director for the
Lincoln Academy Health
Center to keep students
healthy.
“The really big thing that sticks out to me about
Steve is he is willing to do whatever it takes to
provide services,” said Waltz.
If a student can’t make it to an appointment after
school, Dr. Feder sometimes stops by the school to
see them in the morning, she said. Often, students
will ask for him by name because they know and
trust him, said Waltz.
That sense of trust has long-term consequences,
said Waltz. “It establishes a relationship with
health care people that they are going to need at
some point in their life, even if they are healthy.”
Dr. Feder said he learned the value of being an
active participant in his patients’ lives from his own
pediatrician, who was also the coach of Dr. Feder’s
youth football team and a mentor when he first
considered a medical career.
That lesson has stayed with him throughout
his own medical career. For Dr. Feder, taking
good care of his patients is a job that extends far
beyond regular office hours, whether that means
volunteering, teaching health classes, coaching or
working with others to insure good services for
children.
“I see it as part of the whole package. Teaching
kids leadership, teaching them character, helping
kids be healthy in a lot of different ways,” said Dr.
Feder.
The Best Care is With
Those Who Know You Best
Lincoln Medical Partners General Surgeons Matthew
Zadrowski, MD; Mark Mainella, DO; and Garth Miller, MD.
Certified Registered Nurse Anesthetist Wendy Nichols
noticed the difference the first shift she worked at
Miles Memorial Hospital ten years ago.
6
“The surgeons and the staff and everybody else really
care about the patients. They care about every aspect of
the patient,” said Nichols, an advanced practice nurse
who lives in Hawaii but works on a temporary basis at
hospitals across the United States.
At other hospitals, the emphasis is often on
performing surgery on as many patients as possible or
using the latest technology, said Nichols.
At Miles Memorial and St. Andrews Hospitals,
where Lincoln Medical Partners general surgeons
practice, the focus is simply on the patient.
That focus and the professionalism of the surgical
team she works with is the reason Nichols twice
chose to have her own surgery performed by Lincoln
Medical Partners surgeons, although her regular doctor
is in Hawaii.
That sort of decision by a member of a surgical staff is
as good an indicator of quality as any statistic, she said.
“If you want to know where to have surgery, ask the
staff,” said Nichols. The people she works with won’t
have surgery anywhere else, she added.
“Our surgeons look at the patient as somebody they
are going to be sitting next to in church or standing
next to in the line at the grocery store,” said Nichols.
Surgeon Garth Miller, MD, has been operating in
Lincoln County for approximately twenty years, often
on people he has known for years.
Dr. Miller joined Lincoln Medical Partners last
year because he wanted to spend more time with
his patients and less time handling burdensome
administrative duties.
A specialist in laparoscopic (minimally invasive
surgery), Dr. Miller has been gratified by the steady
improvement in laparoscopic techniques and
instruments over the years. Laparoscopic techniques
allow surgeons to perform many common surgeries
using minimally invasive techniques that cause less
pain and disability.

A new Lincoln County Healthcare three-year contract with
Olympus, a maker of surgical optics, means Miles and St. Andrews
surgeons will have access to state-of-the art optics as they become
available.
The new equipment offers surgeons an optical improvement
roughly the same as the increase in clarity between an old cathode-
ray television set and a new high-definition T.V.
To a surgeon, that increase in clarity can be critical. The better
optics help them see abnormalities at an earlier stage and work
more precisely around vital structures like blood vessels or nerves.
“In many ways it will help operations go more smoothly and more
quickly and safely,” said Miller.
New electronic
medical records
systems at both St.
Andrews and Miles
also mean that
surgeons have access
to previous test results,
diagnoses and other
information about
patients previously
treated at Miles and
St. Andrews, that
can help prevent
medical errors and
also reduce costs by
avoiding unnecessary
duplication of tests.
“We know that we
can’t do everything
that a big hospital can
do and we try to limit our practice to what we can do well,” said Dr.
Miller.
What it does mean, however, is that patients needing many
common surgeries, including gallbladder surgery, hernia repairs and
colonoscopies, can be confident that they can have their operation
locally without any sacrifice in quality.
“You can get your surgery here and be assured you are getting just
as good care as you can get anywhere else, or better, because we
know you,” said Dr. Miller.
Part of a Big Team
Susan Endicott was examining a sump pump in a
friend’s water-filled basement last summer when she felt
a horrible pressure in her chest.
Healthy and physically active at 56, Susan recognized
the symptoms of a heart attack, but at first had difficulty
confronting the reality of her condition.
“Heart attack went through my mind, but I thought
instantly ‘I can’t afford that, so this has to be something
else,’ ” she said.
When she went upstairs to lie down, however, her
husband, Robert Mitchell, quickly found his wife an
aspirin and got her to the car.
Moments later, Endicott was passing
through the familiar doors of the St. Andrews
Hospital Emergency Department. She had
been in the Emergency Department several
times over the years, but always as a caregiver,
never as a patient. She realized her situation
was serious, but she also knew that she was in
a good place.
“They were flying around taking care of me.
All the angels were in place, they were just
completely efficient,” said Endicott.
Dr. Phelps Carter, an emergency
physician with 38 years experience at Maine
Medical Center in Portland, ordered an
electrocardiogram and notified Susan that she
was having a heart attack. Within minutes,
a nurse had administered a medication to
dissolve clots blocking the flow of blood to heart muscle,
and Dr. Carter called the coronary catheterization
laboratory at Maine Medical Center to notify them that
Susan was on her way.
As a member of the MaineHealth integrated health
network, which was ranked 26 th in the nation in 2009 by
SDI, a healthcare market insight and analytics firm, St.
Andrews is a member of a network dedicated to using
all of its resources to insure patients at every member
facility receive the best care possible.
MaineHealth’s members include Maine Medical
Center, one of the top centers for cardiac care in the
country.
When Dr. Carter realized that Endicott needed care
that couldn’t be provided at the small rural hospital,
he arranged for her transfer to the large urban medical
center.
8
“That is one of the great things about being part
of a big team. You have access to help if you need
it,” said Dr. Carter. “I can do the initial lifesaving
procedures, but then I am going to need people who can
carry the patient forward in his or her health care.”
When Endicott arrived at Maine Medical Center, she
found herself in a room filled with doctors, nurses and
technicians highly trained in a very specialized field.
Using state-of-the-art x-ray equipment and tiny optical
sensors, the team found the source of the blood clot that
had caused her heart attack and inserted a stent.
After undergoing rehabilitation at Maine Medical
Center, Endicott returned home to an outpouring of
support.
The experience taught Endicott several things: She
learned how much love and affection surrounded
her in the Boothbay community, and that even with
a healthy lifestyle, nobody can take their health for
granted.
It also left her with a deep sense of gratitude. She is
grateful that she survived the heart attack, and also for
her family and everyone who treated her or played a
role in her care that day, including Dr. Carter and the
staff at St. Andrews.
“We love this little hospital and it has gotten better
and better and better all the time,” she said.
Volunteers
Herb Watson (right) and Anne Durcan
(below) joined 233 other volunteers last
year in donating more than 15,000 hours
of service in departments throughout the
Miles organizations.
From the warm smiling face of the helpful
person at the front desk of Miles Memorial
Hospital, to those who maintain the gardens
that provide staff and patients alike with a
place to spend a few peaceful moments in
the course of a day, volunteers are an integral
part of the Miles experience.
No matter where you go at Miles Memorial
Hospital, Coves Edge Long-Term Care or other
Miles organizations, you will find well-trained
volunteers on the front lines of many services and
playing a vital supporting role in others.
On the Miles campus, volunteers visit patients
with therapy dogs and serve patients and visitors
in the Milestone Café. At Coves Edge Long-Term
Care, pastoral volunteers visit residents who are far
from family.
Connie Bright, Miles Director of Volunteer
Services, said that by offering their compassion and
sharing skills often gained in long careers, Miles
volunteers enrich the experience of both patients
and family members by giving of themselves.
“Volunteers give us an irreplaceable resource: the
gift of their time,” said Bright.
Miles in Motion
From vacuum cleaners to collectible silver coins;
picture frames to an antique pipe organ, you can find
almost anything at the Miles in Motion Thrift Shop.
But if you find something you like, don’t hesitate,
(the pipe organ has already been purchased).
The heir of a 60-year-old Miles Memorial Hospital
League Rummage Sale tradition, the new thrift shop
(open 10 a.m. to 4 p.m., Monday – Saturday) compacts
decades of traditions and acres of bargains into just
under 5,000 square feet on two floors in the Wheeler
Building on Church Street in Damariscotta.
After grossing more than $20,000 in its first two
weeks, it has continued to surpass expectations thanks
to the generosity of donors and the hard work of a
growing group of talented volunteers.
The growth of the Thrift Shop is even more
spectacular when the many ways the League supports
the Miles organizations are taken into account.
Through the House & Garden Tour, the Miles Gift
Shop and other efforts, League members raised more
than $95,000 in 2009 to help Miles provide the best
care for our communities. Thanks to their dedication,
Miles was able to purchase vital medical equipment,
enhance services and expand educational opportunities
in 2009.
10
And while the Thrift Shop had big shoes to fill as
the successor to the single largest League effort, with
the help of a supportive community and talented
volunteers it promises to surpass its
popular predecessor.
Like the original rummage sale, the
new thrift shop offers high-quality, often
unique items for very attractive prices and
like the original rummage sale, all of the
items come from generous donors.
“Just to look around and realize that
everything is donated is amazing,” said
Thrift Shop Manager Joy Walker. “I think
people give because they care so much
about the community and the hospital.”

So many items are donated that Tent Sales are also held the third Saturday of
every summer month through October, weather permitting, 7:30 a.m. – 3 p.m. at
the Belvedere Rd. drop-off.
Volunteers give their time and expertise to categorize and price items. A jewelry
group, “The Jewelettes” meet once a month to fix, clean and research items before
putting them out for sale.
The dedication of volunteers is a large part of what makes the thrift shop such a
special place, said Walker.
Since Miles in Motion opened, more than 100 people have volunteered their
time at the Wheeler Building or the drop-off at Belvedere Road next to Route 1,
and the list is growing.
“I think they experience the enthusiasm of the other volunteers. They see that
this is a unique kind of venture. It is about shopping but it is also about having a
good time together,” Walker said.
Volunteer Di Silar jokes that while she loves volunteering at the Thrift Shop, it
can be expensive because she is always finding things she wants to buy for herself
or her family.
“What other store can you go to where the merchandise changes daily?” asked
Silar. “We have the quality of 5 th Avenue and the prices of Army Navy.”
But the merchandise is only part of the reason people volunteer, said Silar.
She and her husband have made wonderful friends since they began
volunteering with the League.
“That is a gift that is never going to be measured in dollars,” she said.
The Robert W. Belknap, M.D. Free Bed Fund was established in 1955 to
help needy individuals with their hospital bills at Miles Memorial Hospital.
The fund selects recipients on an annual basis, and pays part or all of their
outstanding hospital bill. From January 1, 2009 - December 31, 2009, the
fund paid $13,000 to Miles on behalf of those who needed it the most.
FY 2009 Lincoln County Healthcare Community Benefits Statement
Lincoln County Healthcare, Inc.’s (LCH) day-to-day operations as a tax-exempt organization include many system-wide initiatives to
provide inpatient, outpatient, chronic and acute health care services to the people of Lincoln County, Maine. With these programs,
LCH hopes to fill existing local gaps, while making a positive impact in the communities we serve. This report will summarize LCH’s
community benefits efforts over the last year. The final section (VII) will also provide a financial summary of charity care, bad debt,
government-sponsored healthcare, and all subsidized community programs and other support.
Lincoln County Healthcare’s mission is to ensure access to high-quality, patient-centered and affordable health care to the people of our
region. As renowned rural healthcare centers, St. Andrews and Miles primary focus is patient care, comfort, and safety provided by a
personable, professional staff, utilizing the finest technology and equipment. We are active members of the MaineHealth® Family of
healthcare providers and function in collaboration with other member organizations throughout southern, western, and central Maine.
The Lincoln County Healthcare Leadership Team, under the governance of a Board of Trustees, is responsible for the coordination, quality
and efficiency of care provided.
• The Lincoln County Healthcare Education Center is a
collaborative effort of Central Maine Community College and
Lincoln County Healthcare. The new school, the first college
degree program in Lincoln County, opened in September 2008 to
eight students. First year students have the option to earn a Licensed
Practical Nursing (LPN) within a year and will have a five year grace
period to complete the requirements for the RN credential, should

they choose, or continue on for a second year to become a Registered
Nurse. The program collaboratively works with colleges, clinical
departments of the hospital, long term care facilities, MaineHealth,
Lincoln Academy, and local businesses. Students receive classroom
training at Central Maine Community College and in classrooms
at the Lincoln County Healthcare Education Center located at
66A Chapman Street in Damariscotta. An agreement with Lincoln
Academy will allow students to use their labs for a percentage of
their work. Clinical training will be at Miles Memorial Hospital,
and at our long-term facilities in both communities. Lincoln County
Healthcare provides administrative services, clinical training,
equipment, supplies, technical support, and two full scholarships to
the school.
Access to Care Programs
CarePartners – The program arranges the provision of donated
healthcare services for low income uninsured Mainers in
Cumberland, Kennebec, Lincoln, and Waldo Counties.
CarePartners also provides administrative support to help serve the
target population, including comprehensive eligibility assessment,
care management, and access to low cost or free pharmaceuticals.
MedAccess – The program provided access to approximately $2.2
million of free medications in FY09. CarePartners provides this
community resource to uninsured and underinsured community
members through the Patient Assistance Programs (PAPs). In
addition to this service, MedAccess offers application assistance for
other prescription access programs (Medicare Part D, etc.), local
low-cost generic programs, and other state and federal programs
either in-person or through a toll-free number.
Lincoln County Healthcare’s Aggregate
Net Community Benefit Investment = $7,108,718.00
Billing and Collection Practices
Lincoln County Healthcare charges all patients the same price for
the same services, regardless of payor source. Individuals are not
required to pay or make arrangements to pay prior to the services
being provided. On average, the first bill is sent to a patient 7-10 days
after services are provided. After that initial billing date, the patient
has 30 days to pay for those services. Before collection action is
taken, three notices must be sent to patients informing them of their
lack of proper payments and continued attempts to communicate
with them about a solution. In the absence of either full payment or
a patient’s attempts to communicate in order to resolve the situation,
LCH does use a responsible and professional collection agency if
necessary. A bill will become classified as “bad debt” if a patient has
not paid the full amount within 120 days. MaineHealth hospitals
may pursue legal action for collecting an outstanding bill only with
prior Board approval. The LCH Board has not voted to pursue such
legal action in the past several years. Member organizations offering
long-term and residential care services have, from time to time,
taken legal action to collect amounts due from patients or residents
who are not covered under the MaineCare program and have the
sufficient resources to pay for the services they received.
Charity Care Policies
Lincoln County Healthcare’s policy of charity care and financial
assistance is easily understood, prominently posted, and publicly
available. A process exists for offering charity care or financial
assistance to patients who are unable to pay after they have already
been billed. In addition to monitoring collection practices, copies
of the charity care policy are made available to patients at the main
entrance and with bill/collection notices. The organization uses
simple application procedures for charity care or financial assistance
that do not intimidate or confuse applicants. LCH’s employees
who work in billing, and accounts receivable are fully informed and
educated about all financial assistance policies. These staff members
identify unpaid bills where persons are unable to pay, and separate
these potential “charity care” bills from other bad debt accounts.
Notice Of Availability Of Uncompensated Services
Miles Memorial Hospital and St. Andrews Hospital are required
by law to give a reasonable amount of its services without charge
to eligible persons who cannot afford to pay for care. To be eligible
to receive uncompensated service, your family income must be at or
below the following levels:
Good Governance and
Executive Compensation Policies
Good Governance
LCH has a Board of 20 community members, the majority of whom
are not practicing physicians, officers, department heads, or other
employees with a financial connection or otherwise affiliated with
the organization itself. The Board meets 12 times a year (on average),
and has a written “conflict of interest” policy in place. The Board
understands the specific mission of the organization, and approves
strategic planning initiatives aimed at carrying out this mission.
Trustees understand their fiscal and other specific responsibilities
while serving on the Board, and further education/information is
provided to Board members if requested. Trustees and Executive
Officers of LCH do not receive loans on behalf of the organization.
The organization ensures that a substantial part of its activities do
not involve attempts to influence legislation, and that it will not
take an official position or provide direct support for or against a
political candidate. Moreover, in addition to the CEO, CFO, or
both officially signing-off on the annual 990s and audited financial
statements, the Board of Trustees must also have final approval of
the annual audited financial statements. The Board has also adopted
and maintains a corporate compliance program that includes a
Code of Conduct for all staff education and training, monitoring for
compliance, and a Helpline for staff to call, all intended to produce
continual compliance with organizational policies and the law.
Executive Compensation
LCH has a formal written compensation policy in place. In consultation with Towers Perrin, The MaineHealth Board Compensation
Committee establishes appropriate compensation parameters for each member organization’s CEO and certain members of their Senior
Management team. Working within those parameters, the LCH Board determines the level of compensation for its CEO. The findings of
the Compensation Committee are made transparent to, and voted on by, the full Governing Board. This “total executive compensation” is filed
publicly by the organization, and includes “total cash compensation” and “total value of all benefits and perquisites associated with position
(such as housing allowances, social club memberships, signing bonuses, etc.).” The Board takes necessary action to prevent the CEO from
voting or directly participating in the final Committee determination of her/his own compensation. The organization’s executive compensation
procedure relies upon appropriate data for comparability (e.g. compensation levels paid by both taxable and tax-exempt similarly situated
organizations and independent compensation surveys by nationally recognized independent firms). Finally, the organization refrains from
allowing executive compensation to ever be based solely on LCH’s revenues or other similar profit-sharing strategies.
