REQUEST FOR PRE-QUALIFICATIONS (RFQ) - Build LACCD

All addenda issued shall become part of the Prequalification documents, and Applicants shallacknowledge, in writing, receipt and incorporation of all addenda and clarifications in theirPrequalification Submittal. Failure of the Applicant to receive Addenda shall not relieve theApplicant from any obligation under its Prequalification Submittal as submitted. TheApplicant shall identify and list in its Prequalification Submittal all Addenda received andincluded in its Prequalification Submittal; failure to do so may be asserted by the District as abasis for determining the Prequalification Submittal non-responsive.4.11 Preparation.The portions of the Prequalification Submittal prepared or filled in by the Applicant must beeither typed or completed in ink. Responses in pencil will be deemed unacceptable. For “Yes” or“No” answers, indicate either “Yes” or “No” by putting a check mark or “X” mark in the boxnext to the “Yes” or “No”. If there is a requirement that the Applicant or any Applicant Membersubmit any additional documents, they shall be packaged as part of the Applicant’sPrequalification Submittal.4.12 Submission.The proposer shall submit to the District no later than the Due Date (See cover page): One (1) hard copy, Enclosed in a sealed opaque envelope clearly marked with the proposer’slegal name, address, and the words: "Prequalification Submittal Respondingto Structured Cabling Systems Prequalification #40J-5J05.05CONFIDENTIAL” Signed by the individual or official of the firm who has the power to bind thefirm contractually One (1) CD containing an electronic version of the Prequalification Submittal inMicrosoft Word format or PDF.Mail or hand deliver Prequalification Submittals to this Build-LACCD, 915 Wilshire Blvd,suite 810, Los Angeles, CA 90017 (see cover page). Prequalification Submittals submitted viafax, telephone or e-mail will not be accepted. If hand-delivered, ample time should be allowedfor delays caused by downtown Los Angeles traffic and parking.Proposer assumes full and sole responsibility for timely receipt of its PrequalificationSubmittal and its security. Any Prequalification Submittal received after the stated deadline,may, at the sole discretion of the District, be returned without consideration. Delays in themail service or any other delivery method shall not be considered a valid excuse for latePrequalification Submittals. Late Prequalification Submittals will only be considered in theevent that a selection cannot be made from timely Prequalification Submittals.All Prequalification Submittals must be firm offers subject to acceptance by District and maynot be withdrawn for a period of ninety (90) calendar days following the last day to submitPrequalification Submittals.
