RFQ <strong>document</strong> <strong>for</strong> <strong>development</strong> <strong>of</strong> Four Laning <strong>of</strong> Rewa Ring Road on BOT (Toll + Annuity)basisand certified by the Applicant. Supporting materials, which arenot translated into English, may not be considered. For the purpose<strong>of</strong> interpretation and evaluation <strong>of</strong> the Application, the Englishlanguage translation shall prevail.2.12 Format and signing <strong>of</strong> Application2.12.1 The Applicant shall provide all the in<strong>for</strong>mation sought underthis RFQ. The Authority will evaluate only those Applicationsthat are received in the required <strong>for</strong>mats and complete in allrespects. Incomplete and /or conditional Applications shall beliable to rejection.2.12.2 The Applicant shall prepare 1 (one) original set <strong>of</strong> the Application(together with originals/ copies <strong>of</strong> <strong>document</strong>s required to besubmitted along therewith pursuant to this RFQ) and clearlymarked “ORIGINAL”. In addition, the Applicant shall submit 1(one) copy <strong>of</strong> the Application, along with <strong>document</strong>s requiredto be submitted along therewith pursuant to this RFQ, marked“COPY”. The Applicant shall also provide 2 (two) s<strong>of</strong>t copies onCompact Disc (CD). In the event <strong>of</strong> any discrepancy between theoriginal and the copy, the original shall prevail.2.12.3 The Application and its copy shall be typed or written in indelibleink and signed by the authorised signatory <strong>of</strong> the Applicant whoshall also initial each page in blue ink. In case <strong>of</strong> printed andpublished <strong>document</strong>s, only the cover shall be initialled. All thealterations, omissions, additions or any other amendments made tothe Application shall be initialled by the person(s) signing theApplication. The Application shall contain page numbers and shallbe bound together in hard cover.2.13 Sealing and Marking <strong>of</strong> Applications2.13.1 The Applicant shall submit the Application in the <strong>for</strong>mat specifiedat Appendix-I, together with the <strong>document</strong>s specified in Clause2.13.2, and seal it in an envelope and mark the envelope as“APPLICATION”. The Applicant shall seal the original and thecopy <strong>of</strong> the Application, together with their respectiveenclosures, in separate envelopes duly marking the envelopes as“ORIGINAL” and “COPY”. RFQ <strong>document</strong> <strong>for</strong> <strong>development</strong> <strong>of</strong> Four Laning <strong>of</strong> Rewa Ring Road on BOT (Toll + Annuity)basisin an outer envelope which shall also be marked in accordancewith Clauses 2.13.2 and 2.13.3.2.13.2 Each envelope shall contain:(i)Application in the prescribed <strong>for</strong>mat (Appendix-I) alongwith Annexes and supporting <strong>document</strong>s;“For avoidance <strong>of</strong> doubt, the copies <strong>of</strong> the <strong>document</strong>s to besubmitted shall be either certified or attested copies. Certifiedcopies shall bear the ‘certified true copy’ stamp along withproper signature and stamp <strong>of</strong> the signatory, whereas theattested copies shall be attested by a Notary Public.”(ii) Power <strong>of</strong> Attorney <strong>for</strong> signing the Application asper the <strong>for</strong>mat at Appendix-II;(iii)(iv)(v)(vi)if applicable, the Power <strong>of</strong> Attorney <strong>for</strong> Lead Member <strong>of</strong>Consortium as per the <strong>for</strong>mat at Appendix-III;copy <strong>of</strong> the Joint Bidding Agreement, in case <strong>of</strong> aConsortium, substantially in the <strong>for</strong>mat at Appendix-IV;copy <strong>of</strong> Memorandum and Articles <strong>of</strong> Association, if theApplicant is a body corporate, and if a partnership then acopy <strong>of</strong> its partnership deed; andcopies <strong>of</strong> Applicant’s duly audited balance sheet andpr<strong>of</strong>it and loss account <strong>for</strong> the preceding five years.Each <strong>of</strong> the envelopes shall clearly bear the following identification:“Application f or Qualification: F o u r Laning withpaved shoulder <strong>of</strong> Rewa Ring Road on BOT( T o l l + A n n u i t y ) basis under the State <strong>of</strong> MadhyaPradesh.”and shall clearly indicate the name and address <strong>of</strong> the Applicant.In addition, the Application Due Date should be indicated on theright hand corner <strong>of</strong> each <strong>of</strong> the envelopes.2.13.3 Each <strong>of</strong> the envelopes shall be addressed to:________________________________________________________________________________________________________________________________NIT No. MPRDC/BOT (T+A)/PQ/2012/ 7783 Bhopal, dated 27.09.2012 - 27 -
