Delivering on health targets7How we will implement that change“Going Home”All children born in the Midlands area will become part of the Going Home initiative which seeks tocoordinate the discharge of mother and baby into a supportive environment.Establishing Duty of Care (5 eyes on Under 5s)Once allocated no child can be taken off the list unless they move to an address outside theMidlands area. Primary Care will take their role as a pair of eyes seriously. All Midlands staff will havea Child Protection Trained staff member they can go to for advice.Well Child MilestonesWell Child Nurses will be allocated to practices and will deliver their service to an enrolledpopulation as part of a General Practitioner lead Family Health Care Team. The service will alter tobecome wrap around rather than independent and dislocated.Public Health Nurse in every Early Childhood CentreEvery Midlands Early Childhood Centre will have a Public Health Nurse attached to it. They will beinformed child health experts that are excited and engaging sources of information on child health.Very Important ProjectAll Midlands four year olds will be part of the Very Important Project - a Project with a Whanau Orakaupapa which works with libraries and primary schools to have every four year old completefive milestones before they turn five (eyes and ears, teeth, B4 School, meet the teacher and jointhe library).Public Health Nurse in every Primary SchoolEvery Midlands Primary School will have a Public Health Nurse attached to it. They will be informedchild health experts that are excited and engaging sources of information on child health.Youth One Stop Shop co-located in high risk areasPopulation analysis will be undertaken to pinpoint key disengaged youth populations. Innovativeand challenging co-location opportunities will be identified and funded in order to bring health careto a disengaged youth sector.Year 4 Health CheckMidlands will establish a new curriculum linked health check for all Year 4s. It will connect with asimilar educational milestone and screen for any core developmental or health issues.School Clinic in every Secondary SchoolEvery high school will have a school nurse.Year 9 Health CheckMidlands will put in place HEADSS screening for all Year 9 students in the region. The programmewill be curriculum linked offering an opportunity for a consistent approach between educationand health.HPV VaccinationAs an information opportunity to encourage young women to begin to self-manage their own health.78
