The proposed sites have been chosen based on arange of factors that include:• interest in and willingness to try new models;• where there is an urgent need to develop new facilities;• fragility of existing services;• high needs populations;• willingness to be flexible with the existing business model or to try alternatives.The sites below share a high level of readiness for change. It is assumed there are a number of othersites within the Midlands Network that will want to shift status as the model evolves.WhitiangaMercury Bay Medical CentreHamiltonNew Plymouth(2x IFHC)1. Carefirst2. Maru Wehi IntergratedWhanau Ora SiteTurangiPihanga HealthGisborne (2x IFHC)HaweraPateaWhile not everyone will be able to move to an Integrated Family Health Centre at once, theIntegrated Family Health Network will provide all practices and providers with access to a greaterflexibility of funding.The health burden maps that follow have been developed using demographic information withinmesh blocks, and disease prevalence ratios, checked against available disease coding data,to project concentrations. Incorporating other patient characteristics of engagement and theircapability for health management against the health burden provides the ability to determine thenature of service delivery to the population within the Integrated Family Health Centre and otherallied primary and secondary health providers. The determination of the health service burden withina particular area allows for far better planning of the health service resources required to support it.50
