Ask your adviser to sign the <strong>University</strong> course adjustment forms and then submit theforms to the Office of Admissions and Records.Withdrawal From the <strong>University</strong>Deadlines: You may withdraw from the <strong>University</strong> any time before the last day ofa semester or session on which regular classes are scheduled to meet. If you withdrawbefore the Friday of the tenth week of classes (or the Friday of the fourth week in a sixweeksummer session, or the Friday of the second week of a three-week summersession) you receive grades of W in all of your courses for that semester or session.After these deadlines, you will receive grades of W in those courses in which youmade satisfactory progress and grades of WU for courses in which your progress hadbeen unsatisfactory.Procedures: 1 . Students who decide to leave WVU should withdraw from allclasses and must do so in accordance with established <strong>University</strong> policy. Students areresponsible for all financial obligations and for following established procedures. Thisincludes the completion of forms and the delivery of the completed forms to appropriateofficials. Students not fulfilling their financial obligations may have difficultywithdrawing from the <strong>University</strong>. A withdrawal becomes official only after the formshave been recorded by the Director of Admissions and Records.2. Students who are unable to withdraw in person because of illness, accident, orother valid reasons still must send notification of their intention to withdraw to theOffice of Student Life. The notice should be verified in writing and the student ID andPRT cards enclosed.3. Students who desire to withdraw from WVU must obtain a withdrawal formfrom the Student Affairs office (or dean's office of an off-campus instructional unit).Withdrawal procedure is explained at that time. Identification (ID) and PRT cards mustbe presented.4. With the help of their academic advisers, students are responsible for determininghow withdrawal from the <strong>University</strong> may affect their future status with the<strong>University</strong>, including such aspects as suspension for failure to make progress toward adegree or violation of established academic probation and eligibility for scholarships,fellowships, or financial aid.Academic Leave of AbsenceWVU offers undergraduate students in good standing, as defined by WVU'suniform suspension policy and not subject to disciplinary action, the opportunity torequest an academic leave of absence. The academic leave of absence is designedfor the student who wishes to be away from his/her academic endeavors at WVU forone or more semesters, but intends to return at a later date. Leave of absence statusmust be requested before the beginning of the semester for which the leave is desired.The academic records of students on an academic leave of absence remain in anactive status. While on an academic leave of absence, the student retains the right touse certain campus facilities such as the Reading Lab, Writing Lab, Math Lab, StudentCounseling Service, and Career Services. When a student decides to return to WVUafter his/her academic leave of absence, application fees are waived. While on anacademic leave of absence, the student receives communications from WVU.
