If the guide will be fixed to the jaw during surgery usingosseosynthesis screws, all necessary adaptations to aid this fixing should be done beforesurgery, outside the surgery theatre. Also disinfection and sterilisation of these acrylic devicesshould be done according to the manufacturer’s recommendations.6. Preparation of surgery according to planning.As all components to be used during surgery are known beforehand, because all parametersare incorporated in the design of the guide, surgery should be prepared to prevent anymistakes in equipment during surgery. It might be helpful to position the needed componentsin a structured way onto the dynamic surgical tray. The dynamic surgical tray is designed torepresent the sixteen tooth positions of a regular patient jaw. The different componentsneeded for each implant site can be positioned easily at the correct tooth position. It isimportant to position the correct drills and implant holders for each implant site. If the samecomponents are needed for multiple tooth positions, the components can be positioned atany of these positions. Also general components, like screwdriver, handpiece connector forthe implant holders and/or trephine can easily be positioned in this dynamic surgical tray.By transferring the planning completely to the surgical tray, the actual transfer to the surgery ismuch easier.SURGICAL PROCEDURESFig. 2: Surgical tray prepared for placement of six implants7. Surgical Procedure1. Standard precautions for SurgiGuideUse the standard procedures for these type of devices, which are indicated by themanufacturer. Position the guide in the patient’s mouth according to the recommendations forthe specific guide type. Check their fit and stability. If there is any reason to believe that theSurgiGuide does not transfer accurately into the implant positions or does not fit sufficientlyon the supporting surface, the predictability offered by the SAFE System cannot beguaranteed.2. Final positioning of the SurgiGuideIf the SurgiGuide is approved and will be used for surgery, stabilize the guide on its finalposition. If no tissue punch is needed, it is recommended to have the internal drilling tubesalready screwed in before the final positioning. In some situations, the guide might be fixed tothe jaw by using small osseosynthesis screws. If the guide will not be fixed, take all necessaryactions to avoid movement of the device during surgery. As only one guide will be used forthe complete implant placement process, including drilling and the actual placement, the finalpositioning of the SurgiGuide is extremely important.
