Applies to all <strong>University</strong> <strong>of</strong> <strong>St</strong> <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> <strong>St</strong>udentsWELLNESS CENTER POLICIES AND PROCEDURES (Applies to Florida and Cali<strong>for</strong>niacampuses only)Mission <strong>St</strong>atementThe mission <strong>of</strong> the <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong>’s Wellness Center is to provide an environment thatpromotes and facilitates a healthy and productive lifestyle in our students, faculty, staff, and ultimatelyour clients and local community. This will be achieved by (a) development <strong>of</strong> programs that will allowthe <strong>University</strong> philosophy on health and wellness to become a reality and (b) providing a state <strong>of</strong> the artwellness facility that will include wellness related testing/screening and quality instructions in the variousdomains <strong>of</strong> wellness.Wellness Center Activity/Lab FeeThe <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> is committed to wellness. The students are expected to share in thatcommitment. The wellness center activity/lab fee is a term-based fee. Although the wellness centeractivity/lab fee will include personal use <strong>of</strong> the wellness center, it also includes activities and labs thatwill be per<strong>for</strong>med in the wellness center throughout the curriculum.On Campus <strong>St</strong>udents Only: Because the wellness center activity/lab fee is a yearly fee, it will be paidwhether or not the student is out on internship. Based on student feedback, the yearly fee is broken downper trimester to be less <strong>of</strong> a financial burden.If the student graduates during a year the fee will be prorated to reflect that. If the student withdraws fromall course work during a trimester or takes a leave <strong>of</strong> absence, the wellness center activity/lab fee will berefunded or credited in accordance with the student activity refund policy in this handbook. Part-timestudents will pay the full yearly fee. See individual program <strong>for</strong> additional in<strong>for</strong>mation.Flex/OPA <strong>St</strong>udents Only: Please see more in<strong>for</strong>mation regarding the fees under the Flex/OPAsections <strong>of</strong> this Handbook.Off Campus <strong>St</strong>udents: The Wellness Center is available to all EdD, DHSc, OTD and t-DPT studentswhile on the campus. Visiting students may purchase a daily pass and must adhere to all rules, regulations,policies and procedures that apply to members.Eligibility <strong>for</strong> Wellness Center:Only eligible participants (see below) will be allowed entrance into the Wellness Center unless prearrangedwith the Director <strong>of</strong> the Wellness Center. This includes “tours” or people desiring to “lookaround.” The Cali<strong>for</strong>nia wellness center does not allow non-USA related individuals to use the facilities.All Wellness Center participants MUST present photo identification to Wellness Center staff and sign inand out <strong>for</strong> each visit using his/her Wellness Center badge identification. It is advised that participantsconsult with a medical physician prior to starting an exercise program. Written clearance from a medicalphysician may be required. Prior to the first visit, participants must complete a Physical ActivityReadiness Questionnaire (PAR-Q) and submit it to the Wellness Center staff <strong>for</strong> review. A participantmay be required to fill out an additional medical history questionnaire and a medical release waiver <strong>for</strong>mprior to participation.<strong>St</strong>udents – Current on-campus students <strong>of</strong> the <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> will use the WellnessCenter <strong>for</strong> lab activities in various courses throughout the curriculum. <strong>St</strong>udents may also use theWellness Center <strong>for</strong> personal fitness. The fee associated with the Wellness Center is included in ayearly activity fee. As a courtesy this fee will be broken down and paid per trimester.Distance Education <strong>St</strong>udents – The Wellness Center is available to all Distance Education studentswhile on the campus. Visiting students may purchase a daily pass and must adhere to all rules,regulations, policies and procedures that apply to members<strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> 47
