Applies to all <strong>University</strong> <strong>of</strong> <strong>St</strong> <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> <strong>St</strong>udentsLevel Two Academic DishonestyLevel Two misconduct is more serious. These types <strong>of</strong> violations are more intentional in nature. Thefollowing are examples:1. Quoting directly or plagiarizing, to a moderate extent, without acknowledging the source.2. Submitting the same work or major portions there<strong>of</strong> to satisfy the requirements <strong>of</strong> more than one(1) course without permission from the instructor.3. Receiving assistance and failing to acknowledge this in a paper or research study.The instructor in consultation with the appropriate program director deals with most Level Twomisconduct <strong>of</strong>fenses. Actions may include:1. A zero grade <strong>for</strong> an assignment2. Writing a paper on academic ethicsRecords <strong>of</strong> students who commit the above type <strong>of</strong> <strong>of</strong>fense will be maintained in the student’s file untilthe student completes the program.Level Three Academic DishonestyThese violations are the most serious type <strong>of</strong> <strong>of</strong>fense:1. Copying another’s paper or exam2. Plagiarizing a substantial amount <strong>of</strong> text in a written assignment3. Enabling someone else to copy your material during an exam4. Using books, notes, or other <strong>for</strong>bidden aids during an examination5. Giving or receiving in<strong>for</strong>mation about the content <strong>of</strong> an exam6. <strong>St</strong>ealing or distributing a copy <strong>of</strong> an examination7. Submitting the work <strong>of</strong> someone else as your own8. Using purchased or copied manuscripts9. Removing posted or reserved instructional material, or otherwise preventing access to it10. Inventing material (this includes citing artificial sources)11. Using illegal or unethical means <strong>of</strong> acquiring in<strong>for</strong>mation12. Utilizing an Instant Messenger or phone while taking an online exam13. A repeat <strong>of</strong>fender-one who has been caught more than once in an act <strong>of</strong> academic dishonesty<strong>St</strong>udents committing Level Three Academic Dishonesty <strong>of</strong>fenses will be referred to the Pr<strong>of</strong>essionalMisconduct Committee. The Pr<strong>of</strong>essional Misconduct Committee will make appropriaterecommendations to the designated program director. Action may include a failing grade and/orconsequent dismissal from the program.OTHER POLICESNON-DISCRIMINATION AND HARRASSMENT POLICYIt is the policy <strong>of</strong> the <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> that each member <strong>of</strong> the <strong>University</strong>community be permitted to work or attend class in an environment free from any <strong>for</strong>m <strong>of</strong> discriminationincluding race, creed, color, age, disability, gender, marital status, national origin, veteran status, andsexual harassment, as prohibited by state and federal statutes. This policy applies to students, faculty,employees and applicants <strong>for</strong> admission or employment. An individual who believes that a violation <strong>of</strong>this policy has occurred may file a complaint with the Grievance Committee.SECURITY POLICYReporting Emergency Situations and Security Concerns: Emergency situations involving athreat to life or property should be reported to the police (911) and communicated immediately thereafterto the <strong>St</strong>udent Services <strong>of</strong>fice. The <strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> encourages all students, faculty, and staffto be involved in campus crime prevention.<strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> 39
