Applies to all <strong>University</strong> <strong>of</strong> <strong>St</strong> <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> <strong>St</strong>udentsIn some instances a re-take test may be allowed or issued, if you per<strong>for</strong>m poorly on anexamination or if you had a “computer glitch” while taking the examination, please contact youronline instructor about the possibility <strong>of</strong> a retake.For Entry-level <strong>St</strong>udents1. Policy <strong>for</strong> Changing an Exam DateEvery attempt should be made to adhere to the exam dates as listed in the syllabus, courseschedule or stated as a class announcement. In the event that a faculty member needs to changean exam date, he/she should give an explanation to the students and provide at least two (2)options <strong>for</strong> rescheduling the exam. The class votes on the best option and majority rules. If thestudents request an exam change and the course instructor approves the request, then theproposed change is voted on anonymously by the class and 100% <strong>of</strong> the class must agree be<strong>for</strong>e achange is allowed.2. Review <strong>of</strong> Examinations during the Trimester• A review is to advance learning and is not a mechanism to obtain a higher grade.• All written examinations will be reviewed at the earliest possible opportunity.• This review will be part <strong>of</strong> a regular class period and will last a maximum <strong>of</strong> one classperiod.• <strong>St</strong>udents will receive their individual scores be<strong>for</strong>e or during this review period.• No questions will be reviewed with an individual student during this review.A sample examination review is as follows (Note: the actual examination reviewmechanism is at the discretion <strong>of</strong> faculty):a. The examination booklet and the answer key are given to the students at the time <strong>of</strong>the review.b. <strong>St</strong>udents then review the examination individually and endeavor to understand thequestions that they answered incorrectly. The use <strong>of</strong> notes, books and taperecorders is at the discretion <strong>of</strong> the individual faculty member.c. If the student is unable to understand why an answer is incorrect, he or she entersthat question number on the top front <strong>of</strong> the exam. The student is then encouragedto <strong>of</strong>fer support <strong>for</strong> his or her choice <strong>of</strong> answer. Exam booklets are collected at theend <strong>of</strong> the review.• Subsequent to the review, the faculty member will reconsider the questions identified on theexam and adjust the key where appropriate and notify the students.• Further review may be remitted by the faculty at their discretion upon request by thestudent.• <strong>St</strong>udents lose the right to review the exam if they have an unexcused absence on thescheduled date <strong>for</strong> the review.3. Final Examination Review• An opportunity to review a final examination is up to the course instructor.• The course syllabus should state whether the final exam can be reviewed.• The mechanism used to review final exams will be determined by the course instructor.• The review <strong>of</strong> final exams must be completed prior to the second Friday <strong>of</strong> the followingnew trimester.• Review <strong>of</strong> final exams is to advance learning. It is not a mechanism to obtain a highergrade.4. Practical Examination Policy <strong>for</strong> Pr<strong>of</strong>essional Courses• A minimum <strong>of</strong> 80% pr<strong>of</strong>iciency is required on all clinically-related practical examinations.The practical examinations require the student to meet both safety and technical skillper<strong>for</strong>mance competencies. Safety requirements must be met at a 100% competency level.If a student does not per<strong>for</strong>m in a safe manner, the student will receive an “F” grade <strong>for</strong> the<strong>University</strong> <strong>of</strong> <strong>St</strong>. <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> 19
