Applies to all <strong>University</strong> <strong>of</strong> <strong>St</strong> <strong>Augustine</strong> <strong>for</strong> <strong>Health</strong> <strong>Sciences</strong> <strong>St</strong>udentseducation center, or independent testing center;- A librarian or library testing center staff member;- A proctor CANNOT be a relative, personal friend, employee, co-worker,student, spouse or significant other.‣ Complete the Proctor Request Form* and submit it to the university. The universityreserves the right to deny or terminate a proctor at any time, <strong>for</strong> any reason.- For Flex/MOPA students – the Proctor Request Form MUST be turned in at thebeginning <strong>of</strong> each trimester to your Flex/MOPA administrative assistant.‣ The student will arrange a testing location with the proctor. The date and time <strong>of</strong> theexamination will be determined by the course instructor. It will be necessary to assurethis proctor is available <strong>for</strong> the stated time and day <strong>of</strong> the exam and that you will havea quiet testing area.‣ A picture ID must be presented to the proctor prior to testing.‣ All other examinations rules stated in this section apply.• Attempts to gain answers to an exam or provide answers to fellow students is consideredunpr<strong>of</strong>essional and subject to disciplinary action (see Academic Integrity Policy, in thishandbook).* Proctor Request <strong>for</strong>ms are located on the myUSA portal under individual programs.2. Written Proctored Exams• <strong>St</strong>udent will provide their own No.2 pencil• It is recommended that the student complete the examination on the test and then transposethe answers to the ScanTron <strong>for</strong>m. This may reduce the risk <strong>of</strong> transcription errors.• <strong>St</strong>udents are encouraged to move to questions they know and to return to questions they arenot sure <strong>of</strong> later.• Examinations will be sent directly to the proctor and will be returned to the <strong>University</strong> by theproctor.3. Online Proctored Exams• Examinations will only be available after the student returns the completed proctor request<strong>for</strong>m to the <strong>University</strong>.• Once the student has completed the exams the proctor will sign the Proctor Completion Formand return it to the <strong>University</strong>.4. Online Examinations (non-proctored)The purpose <strong>of</strong> the online examination is <strong>for</strong> students to determine their overall comprehensionand application <strong>of</strong> the course content. Working together on an examination is consideredcheating (see Academic Integrity Policy, in this handbook). Instructors have been trained todetect cheating and the plat<strong>for</strong>m can also provide in<strong>for</strong>mation to the faculty and staff in regardsto cheating. Please use the online examination as a method <strong>for</strong> improving the learning process.Most examinations are timed. You will not have time to check answers in textbooks. Enter thetest knowing that you are prepared to take this exam just as you would in a classroom, proctoredsetting. Be sure you have adequate time to complete the test and a quiet, distraction-freeenvironment <strong>for</strong> test-taking.Once you begin the test you will not be able to leave it until you have completed the entire test.In other words, you cannot start the test, take a break and then finish it later. If you do exit theexam without completing all test questions, you will receive no points <strong>for</strong> the questions leftunanswered. The exam is scored automatically by the computer and you may go to your gradebook <strong>for</strong> your score.
