Clinical EducationMedical InsuranceDue to contractual agreements, the Clinical Education Office is responsible <strong>for</strong> the documentation <strong>of</strong>student medical insurance during ALL clinical affiliation experiences. Incoming students are required tosubmit a copy <strong>of</strong> their medical insurance card prior to going out on their first shadowing experience orsite visit, during the first trimester. Currently enrolled students are required to submit a copy <strong>of</strong> theMedical Insurance Verification Form to the Clinical Education Office.If a student’s medical insurance in<strong>for</strong>mation changes at any time, they must submit a copy <strong>of</strong> the newin<strong>for</strong>mation to the Clinical Education Office.Failure to submit the required pro<strong>of</strong> <strong>of</strong> medical insurance in the time frame requested will beconsidered an act <strong>of</strong> pr<strong>of</strong>essional misconduct and will result in subsequent referral to the <strong>University</strong>Pr<strong>of</strong>essional Misconduct Committee.Attendance/Absence PolicyAttendance is mandatory. <strong>St</strong>udents are expected to work a minimum <strong>of</strong> 40 hours per week according t<strong>of</strong>acility operating schedule <strong>for</strong> the entire length <strong>of</strong> their scheduled affiliation as outlined in the <strong>University</strong>Academic Calendar. <strong>St</strong>udents must submit written requests on the “Clinical Education All-Purpose Form”to the FWS/CI and to the AFWC/ACCE FOR ANY DAYS OFF during their clinical affiliationexperiences.1. Illness:If the student is ill and unable to attend the clinical affiliation experience, the FWS/CI, theAFWC/ACCE and the Clinical Education Office must be notified prior to the expected time <strong>of</strong>arrival <strong>of</strong> the student <strong>for</strong> that day. The student returning to the clinical area must consider thenature <strong>of</strong> his/her illness as to whether he/she is safe to practice. If the nature <strong>of</strong> the illness is feltto endanger either the student or client, the clinical instructor and/or AFWC/ACCE may requirethat the student submit written documentation from a physician verifying that the student is ableor unable to return to the clinical area. After reviewing the medical release, the student will bereadmitted to the clinical area at the discretion <strong>of</strong> the FWS/CCCE <strong>of</strong> the Clinical Education Siteand the AFWC/ACCE.2. Travel Between Clinical Affiliation Experiences:<strong>St</strong>udents ARE NOT permitted to change the start and end dates <strong>of</strong> their Clinical Affiliationexperiences without written permission from the FWS/CI and AFWC/ACCE.<strong>St</strong>udents who determine that additional travel days are necessary are to submit their initialrequest in writing to the AFWC/ACCE on the “Clinical Education All-Purpose Form” prior tostarting their clinical affiliation experience. The AFWC/ACCE will be in contact with theFWS/CI during the student's clinical affiliation experience. The student will receive writtenpermission from the AFWC/ACCE regarding which clinical affiliation experience the additionaltravel days will be allotted, the number <strong>of</strong> days that will be allotted, and how this time will bemade up.Written approval <strong>of</strong> this request will be granted by the sixth week and be dependent upon thefollowing:a. The student's clinical per<strong>for</strong>manceb. The opportunities available to make up the daysc. Any hardship it places on the facility to provide the days <strong>of</strong>f3. Holidays:The student will follow the holiday schedule established by the Clinical Education Site,NOT the academic schedule <strong>of</strong> the <strong>University</strong>.
